Cardiovascular Disease: Cardioverter-Defibrillators and Left Ventricular Assist Devices.

Sudden cardiac death (SCD) is an abrupt and unexpected death presumed to be due to cardiac causes. The primary etiologic mechanism is cardiac arrest due to an arrhythmia. Cardioverter-defibrillators detect abnormal arrhythmia and abort the rhythm with an electrical shock. Wearable cardioverter-defibrillators are devices in which the leads contact the chest on a vest worn by the patient. Wearable devices are for patients at temporary risk of SCD, when comorbidities prohibit placement of an implantable cardioverter-defibrillator (ICD), or as a bridge to ICD placement or heart transplantation. [Surgical therapy of end-stage heart failure. State of the art 2006].

Cardiac transplantation still remains the gold standard despite recent success in organ-preserving therapy. However, organ shortage forces to process alternative therapies. Cardiac resynchronization therapy and cardiac contractility modulation are new and promising therapies, which are able either to delay or even prevent the need for cardiac transplantation. High-risk cardiac revascularization and valve replacement is another important therapy in especially evaluated patients. With newer organ-protective procedures and novel treatment options like the off-pump bypass surgery, end-stage heart

Direct epicardial assist device using artificial rubber muscle in a swine model of pediatric dilated cardiomyopathy.

PURPOSE: Ventricular assist devices are a potent alternative or bridge therapy to heart transplants for dilated cardiomyopathy patients. However, ventricular assist devices have problems related to biocompatibility, hemocompatibility, and thromboembolic events, especially in younger patients. The present study examined the hemodynamic effects of a direct cardiac compression device using circumferential artificial rubber muscles in a young swine model of dilated cardiomyopathy. METHODS: Dilated cardiomyopathy was established in 6 pigs (6-8 weeks of rapid right ventricular pacing; average weigh

Using Topic Modeling to Understand Patients' and Caregivers' Perspectives About Left Ventricular Assist Device: Thematic Analysis.

BACKGROUND: Heart failure (HF) is a significant global clinical and public health challenge, impacting 64.3 million individuals worldwide. To address the scarcity of donor organs, left ventricular assist device (LVAD) implantation has become a crucial intervention for managing end-stage HF, serving as a bridge to heart transplantation or as a destination therapy. Web-based health forums, such as MyLVAD.com, play a vital role as trusted sources of information for individuals with HF symptoms and their caregivers. OBJECTIVE: We aim to uncover the latent topics within the posts shared by users o

Advancements in left ventricular assist devices to prevent pump thrombosis and blood coagulopathy.

Mechanical circulatory support (MCS) devices, such as left ventricular assist devices (LVADs) are very useful in improving outcomes in patients with advanced-stage heart failure. Despite recent advances in LVAD development, pump thrombosis is one of the most severe adverse events caused by LVADs. The contact of blood with artificial materials of LVAD pumps and cannulas triggers the coagulation cascade. Heat spots, for example, produced by mechanical bearings are often subjected to thrombus build-up when low-flow situations impair washout and thus the necessary cooling does not happen. The form

Severe primary graft dysfunction after heart transplant: trends and outcomes in a contemporary Spanish cohort.

INTRODUCTION AND OBJECTIVES: Severe primary graft dysfunction (PGD) is the leading cause of early mortality following heart transplant (HT). This study analyzed the temporal trends and mortality associated with severe PGD, identified risk factors, and developed a predictive model based on a contemporary cohort. METHODS: A total of 2029 HT performed between 2010 and 2020 in 14 Spanish centers were retrospectively analyzed. Patients with and without severe PGD were compared. Logistic regression was used to identify predictors of severe PGD and to generate a risk score. Model performance was ass

CPR, ECLS, BVAD and successful heart transplantation within 2 months: a single-centre case series in two young, high-urgency listed patients.

INTRODUCTION: In times of organ shortage, death while on the heart waiting-list still represents a major problem. As a consequence, bridging to transplant as well as the decision when to escalate therapy play a very important role. METHODS AND RESULTS: We report on two young patients with dilated cardiomyopathy and acute decompensation who were successfully bridged to heart transplantation with both left and temporary right ventricular assist devices in just 2 months. CONCLUSIONS: As a permanent biventricular assist device (BVAD) would have definitely impaired the patients' outcome after HTX

Molecular Mechanisms of Cardiac Adaptation After Device Deployment.

Cardiac devices have transformed the management of heart failure, ventricular arrhythmias, ischemic cardiomyopathy, and valvular heart disease. Technologies such as cardiac resynchronization therapy (CRT), conduction system pacing, left ventricular assist devices (LVADs), and implantable cardioverter-defibrillators have contributed to abated global cardiovascular risk through action onto pathophysiological processes such as mechanical unloading, electrical resynchronization, or hemodynamic optimization, respectively. While their clinical benefits are well established, their long-term molecular

Arrhythmias after left ventricular assist device implantation: Incidence and management.

The use of mechanical circulatory support has become an increasingly common practice in patients with heart failure, whether used as bridge to transplantation or as destination therapy. The last couple of decades has seen a drastic change in the functioning of the left ventricular assist devices (LVAD), changing from the first generation devices running on pulsatile flow to the current continuous flow devices. Atrial and ventricular arrhythmias are common among heart failure patients, and though the systematic circulation is well supported in patients on mechanical circulatory support, these a

Why Do so Few People with Heart Failure Receive Cardiac Rehabilitation?

Many people with heart failure do not receive cardiac rehabilitation despite a strong evidence base attesting to its effectiveness, and national and international guideline recommendations. A more holistic approach to heart failure rehabilitation is proposed as an alternative to the predominant focus on exercise, emphasising the important role of education and psychosocial support, and acknowledging that this depends on patient need, choice and preference. An individualised, needs-led approach, exploiting the latest digital technologies when appropriate, may help fill existing gaps, improve ac

Alternative Strategy for Biventricular Assist Device in an Infant With Hypertrophic Cardiomyopathy.

We report an infant with hypertrophic cardiomyopathy who underwent biventricular assist device placement with two 15-mL Berlin Heart EXCOR pediatric ventricular assist devices using an alternative atrial cannulation strategy. The systemic circulation was supported by left atrium (LA) to aorta cannulation. The LA was accessed through the right atrium by extending a 6-mm EXCOR cannula with a Gore-Tex graft connected to an atrial septal defect. The pulmonary circulation was supported with cannulation of the right atrium to pulmonary artery. This alternative cannulation strategy facilitated effect

Numerical model of flow in a sac-type ventricular assist device.

This article addresses the growing need for comprehensive tools to investigate the hemodynamics of ventricular assist devices (VADs) in general and sac-type VADs in particular. Numerical simulations can be very helpful in these efforts. However, full simulation of flow inside sac-type VADs poses several key problems, among them simulation of the mechanical heart valves and calculation of the motion of flexible walls. We present a simplified three-dimensional (3D) numerical model of a sac-VAD chamber. The walls in the simplified model are defined to move according to experimental measurements,

Trends and Outcomes of Left Ventricular Assist Device Therapy: JACC Focus Seminar.

As the prevalence of advanced heart failure continues to rise, treatment strategies for select patients include heart transplantation or durable left ventricular assist device (LVAD) support, both of which improve quality of life and extend survival. Recently, the HeartMate 3 has been incorporated into clinical practice, the United Network for Organ Sharing donor heart allocation system was revised, and the management of LVAD-related complications has evolved. Contemporary LVAD recipients have greater preoperative illness severity, but survival is higher and adverse event rates are lower compa

CT of left ventricular assist devices.

Left ventricular assist devices (LVADs) have become an increasingly beneficial option for patients with heart failure, especially in light of the insufficient availability of donor hearts. LVADs have been used effectively in end-stage heart failure as a bridge to heart transplantation, as destination therapy for those ineligible for transplantation, or as a bridge to myocardial recovery. Presently, a wide variety of LVADs are being used therapeutically. Four different LVADs have been used at the authors' institution. The records of 42 patients who underwent implantation of 46 total LVADs durin

Update on the Practical Role of Echocardiography in Selection, Implantation, and Management of Patients Requiring Left Ventricular Assist Device Therapy.

PURPOSE OF REVIEW: Echocardiography is a valuable tool for management of patients with a left ventricular assist device (LVAD). We present an updated review on the practical applications of the role of echocardiography for pre- and postoperative evaluation of patients selected. RECENT FINDINGS: The LVAD is a temporary or permanent option for patients with advanced heart failure who are unresponsive to other therapy. Use of the device has its own risks, and implantation remains a complex procedure. Transthoracic and transesophageal echocardiography are useful tools for patient evaluation and m
