The significance of Helicobacter pylori colonization of the stomach.

Helicobacter pylori (Hp) was discovered in 1982 by the Australians Robin Warren and Barry Marshall. Initially rejected by a skeptical scientific community, it has since gained worldwide recognition as a clinically significant bacterium. The incidence of colonization with Hp increases with age, affecting approximately one third of the world's population. Hp is uniquely capable of surviving in the acid environment of the stomach, and has properties of adherence to epithelial cells that resist parastalsis. Strains of Hp associated with human disease produce specific cytotoxic proteins. After inge…

Clarithromycin for treatment of Helicobacter pylori infections.

BACKGROUND: A better appreciation of the causal relationship between Helicobacter pylori infection and peptic ulcer disease and the benefit conferred by curing this infection has led to the recommendation that all patients with duodenal ulcer disease receive anti-H. pylori treatment. Multi-drug regimens, including bismuth, metronidazole and tetracycline or amoxycillin with an antisecretory agent, are successful in > 90% of treated patients but the emergence of metronidazole-resistant H. pylori has begun to limit their effectiveness. DESIGN: The search for the optimal anti-H. pylori treatment …

Role of Helicobacter pylori in gastric carcinoma.

BACKGROUND: Helicobacter pylori infection has recently been incriminated in the pathogenesis of gastric carcinoma and chronic atrophic gastritis and intestinal metaplasia are considered to be precursors of this condition. Although the incidence of Helicobacter pylori infection in India is high that of gastric carcinoma is low. We, therefore, decided to examine the association between Helicobacter pylori, intestinal metaplasia and gastric carcinoma in a prospective study. METHODS: Fifty patients with carcinoma of the stomach and 50 with non-ulcer dyspepsia underwent upper gastro-intestinal end…

[Atrophy of the gastric mucosa and Helicobacter pylori].

The authors investigated the prevalence of Helicobacter pylori (H. p.) in atrophic gastritis in the antrum and/or in the corpus of the stomach (groups I-IV), as compared with mucosae without atrophy. Criteria for elimination were age under 50 years, a case-history of the use of antibiotics during the last 6 months and chronic ingestion of alcoholic beverages. Group IV was formed by probands with pernicious anaemia who were divided into sub-group IVa with atrophy of the corpus and sub-group IVb with atrophy of the corpus and antrum. It was revealed that in probands with pernicious anaemia the p…

[Helicobacter pylori and the development of atrophic gastritis].

A close relationship between H. pylori infection and atrophic gastritis has been reported, and atrophic gastritis has been thought of as one of the precursors of gastric carcinoma. It was reported that there was a significant positive correlation between the reduction of serum pepsinogen (PG) I/II ratio and the progression of atrophic gastritis. The ratio of PG I/II was significantly decreased in H. pylori positive patients when compared to H. pylori negative patients with chronic gastritis. The PG I/II ratio in gastric cancer was significantly lower than that of chronic gastritis, while the p

[Effective regimen for eradication of Helicobacter pylori].

There is now substantial evidence that Helicobacter pylori (H. pylori) is both a frequent cause of chronic gastritis and an important factor in the recurrence of duodenal ulcer. However, to achieve the eradication of H. pylori is very difficult and although a number of treatment regimens suppress this organism, the stomach usually becomes recolonized shortly after the cessation of treatment. Many reports have shown that polytherapy is more effective than monotherapy. And so, the best results have been achieved with triple therapy using combination of bismuth, metronidazole, or tinidazole, and …

Clinical aspects of infection with Helicobacter pylori.

About 90% of patients with duodenal ulcer and 65% of those with gastric ulcer are infected with Helicobacter pylori. H. pylori infection can be diagnosed by examination of biopsy material obtained at endoscopy; by 14C or 13C-urea breath tests, or by serological means. The relapse rate of duodenal and gastric ulcers can be considerably reduced if the bacteria are eradicated. Some of the current treatment regimens are discussed.

Does the stomach adapt to Helicobacter pylori?

To shed some light on the question of gastric adaptation to Helicobacter pylori infection, an overview is given of the various histopathological and clinical consequences of H. pylori infection in man. H. pylori infection can be considered as a chronic bacterial infection. For many individuals there is a balance between a low degree of infection and a low degree of inflammation. The microbial or host factors in adaptation of this balance are largely unknown. If and how 'downregulation' of the inflammatory/immune response does occur need further study.

The role of Helicobacter pylori in gastroduodenal disease.

Peptic ulcers are the result of a wide variety of factors, with H. pylori probably being one of the more significant. H. pylori has been most strongly associated with gastritis and duodenal ulcer disease. The relationship of H. pylori to gastric ulcers is less substantial. The eradication of H. pylori can be achieved in the majority of patients with triple antibiotic therapy. However, resistance to metronidazole occurs readily. Eradication of H. pylori can change the course of duodenal ulcer disease by decreasing recurrence rate from 80% to zero. Nonetheless, treatment for H. pylori should onl

Helicobacter pylori in dyspeptic patients in Kuwait.

Two hundred and four patients, mainly Arabs, attending for upper gastrointestinal endoscopy at the gastroenterology clinic in Mubarak Al-Kabeer Hospital, Kuwait, were examined for evidence of infection with Helicobacter pylori and associated inflammation. Biopsy specimens of antrum, body, and duodenum; gastric juice; and antral mucosal brushings were investigated by microbiological, cytological, and histopathological methods. Clinical conditions diagnosed at endoscopy included gastritis, gastric ulcer, duodenitis and duodenal ulcer, but half the patients had endoscopically normal gastric and d…

Lack of follow-up in Helicobacter pylori eradication treatment: Results from the European registry on Helicobacter pylori management.

BACKGROUND: The Maastricht VI/Florence consensus recommends the eradication of all Helicobacter pylori (H. pylori) infections. However, retreatment strategies remain inconsistent. AIM: To evaluate the reasons for and factors associated with non-retreatment after H. pylori eradication treatment failure. METHODS: A multicenter, prospective registry (European registry on H. pylori management) was used to evaluate the decisions and outcomes of H. pylori management by European gastroenterologists. Patients with at least one eradication failure between 2013 and 2024 were included and classified in…

Isolated granulomatous gastritis successfully treated by Helicobacter pylori eradication: a possible association between granulomatous gastritis and Helicobacter pylori.

We report two cases of granulomatous gastritis that resolved completely with Helicobacter pylori eradication. Two Japanese women, one 61 years old and one 58 years old, presented to our hospital with epigastralgia. Endoscopy revealed multiple shallow ulcerative lesions in the gastric corpus of both patients. Biopsy specimens demonstrated acute and chronic inflammation with multiple small, noncaseating granulomas at the level of the foveolar isthmi-the junction between the pits and the glands. Both specimens were positive for Helicobacter pylori. No other causes of organic granulomatous disease…

Ability of Helicobacter pylori to internalize into Candida.

The following are our views regarding the "letter to the editor" (Helicobacter is preserved in yeast vacuoles! Does Koch's postulates confirm it?) by Alipour and Gaeini, and the response "letter to the editor" (Candida accommodates non-culturable Helicobacter pylori in its vacuole-Koch's postulates aren't applicable) by Siavoshi and Saniee. Alipour and Gaeini rejected the methods, results, discussion, and conclusions summarized in a review article by Siavoshi and Saniee. The present article reviews and discusses evidence on the evolutionary adaptation of Helicobacter pylori (H. pylori) to thri…

Helicobacter pylori and acne vulgaris: is there a relationship?

BACKGROUND: Helicobacter pylori (H. pylori) is a gastric Gram-negative, spiral-shaped microaerophilic pathogen. H. pylori may play a potential pathogenic role in extra-intestinal diseases such as hepatobiliary, respiratory, and dermatological disorders. The latter included chronic urticaria, psoriasis and rosacea. The first report in literature on the relationship between H. pylori and acne vulgaris (AV), found association between severe AV and H. pylori infection. There are very limited data in AV patients addressing the impact of H. pylori infection on various severities. In this context, th…
