Myelodysplastic syndrome diagnosed by genetic testing for hereditary cancer: a case report.

Genetic testing for solid tumor syndromes typically uses peripheral blood leukocytes (PBL) as the source of germline DNA. This approach has shortcomings in certain situations, such as somatic mosaicism and hematologic malignancies. Here we describe a case where germline genetic testing on PBL revealed an unsuspected diagnosis of myelodysplastic syndrome (MDS). A 68-year-old male with a history of three solid tumors and a significant family history of cancer underwent germline genetic testing with a 76-gene hereditary cancer panel. Initial testing using PBL revealed deletions of the entire APC

Cyclic severe thrombocytopenia following heparin-free hemodialysis in the era of highly biocompatible hemodialysis membranes: a case report.

BACKGROUND: With advances in the biocompatibility of hemodialysis membranes, severe dialyzer-associated thrombocytopenia following heparin-free hemodialysis is uncommon and is not easily detected without postdialysis regular blood tests or in the absence of postdialysis bleeding symptoms. CASE PRESENTATION: Herein, we present the case of a 58-year-old East Asian patient on hemodialysis who experienced cyclic episodes of severe thrombocytopenia following heparin-free hemodialysis, which led to significant thrombocytopenia even on nondialysis days, resulting in the cancellation of scheduled kid

Hematological abnormalities in liver cirrhosis.

Hematological abnormalities are common in cirrhosis and are associated with various pathophysiological mechanisms. Studies have documented a prevalence of thrombocytopenia, leukopenia, and anemia in patients with compensated cirrhosis of 77.9%, 23.5%, and 21.1%, respectively. These abnormalities carry significant clinical implications, including considerations for invasive procedures, infection risk, bleeding risk, and prognosis. Previously, cirrhosis was believed to predispose patients to bleeding due to alterations observed in classical coagulation tests such as prothrombin time, partial thr

Optical genome mapping for structural variation analysis in hematologic malignancies.

Optical genome mapping (OGM) is a technology that is rapidly being adopted in clinical genetics laboratories for its ability to detect structural variation (e.g., translocations, inversions, deletions, duplications, etc.) and replace several concurrent standard of care techniques (karyotype, fluorescence in situ hybridization, and chromosomal microarray). OGM can dramatically simplify lab workflow by reducing multiple tests (conventional karyotype, fluorescence in situ hybridization [FISH], and chromosomal microarray) into one test. The superior ability to detect structural variation across th

[Antibacterial activity in vitro and clinical use of sulperazon].

In order to approach antibacterial activity in vitro and the efficacy of Sulperazon (SPZ) (sulbactam/cefoperazone), the sensitivity tests of 1,372 strains from clinical isolated bacteria to 17 antibiotics including SPZ were determined. The Gram negative bacteria occupied 1,035 strains (75.4) and Gram positive 337 strains (24.6%). 50 episodes of infections of major respiratory system in 43 patients were treated by SPZ. 58% of bacterial infections occurred in hematologic malignant diseases and solid tumors patients. 24% of 50 episodes were in neutropenia status. The positive rate of bac-terial c

Senile dry skin type Sjögren's syndrome.

BACKGROUND: Elderly patients may show asteatotic dermatitis complicating Sjögren's syndrome (sjs) that we have named "senile dry skin type sjs." To define the condition further, we have examined a group of patients with sjs and have recorded clinical and laboratory findings in these patients. PATIENTS AND MATERIALS: Of the 36 patients, 11 were men and 25 were women, with a mean age of 72 years. In addition to the subjective sensation of dryness of the eyes and oral cavity and objective signs of dryness revealed by the Schirmer test, the rose bengal test, minor labial gland biopsy, and sialogr

[Targeted newborn screening for sickle-cell anemia: Sickling test (Emmel test) boundaries in the prenatal assessment in West African area].

BACKGROUND: Newborn screening for sickle cell anemia is necessary in Africa where the disease is more frequent. Hemoglobin electrophoresis is used for screening, but is limited by a high cost and difficult access. Sickling test (Emmel test), which is more affordable and technically more accessible, is often requested for prenatal assessment of pregnant women in West African areas to reserve screening for newborns from mothers in whom the positive sickling test attests the presence of hemoglobin S. This study aims to evaluate the number of undetected sickle cell anemia newborns by a screening p

The importance of incidentally detected coagulation abnormalities in children.

BACKGROUND: A history and physical examination can predict most bleeding disorders. This study aimed to reveal possible hemostatic disorders in patients referred to the pediatric hematology department due to the incidentally detected prolonged prothrombin time (PT) or activated partial thromboplastin time (aPTT). METHODS: Pediatric patients without known hematologic disease and referred to investigate the incidental prolonged PT and/or aPTT were included. Coagulation factor activities were analyzed in these patients; antiphospholipid antibodies and lupus anticoagulant tests were studied in pa

Diagnostic challenges in malaria detection: A comparative diagnostic performance of HRP2-based rapid diagnostic tests, microscopy, and PCR at Bichena primary hospital, Northwest Ethiopia.

BACKGROUND: Accurate malaria diagnosis is crucial for effective case management, strong surveillance, and progress toward elimination. However, in highland regions, diagnostic tools are underutilized or yield suboptimal performance. While hematological alterations are frequently observed in malaria, their role remains largely supportive rather than diagnostic. This study aimed to evaluate diagnostic challenges by comparing the performance of HRP2-based rapid diagnostic tests, microscopy, and PCR at Bichena Primary Hospital, Northwest Ethiopia, with hematological profiles examined as supportive

Human cytomegalovirus infection among febrile hematological cancer patients at the Uganda Cancer Institute.

UNLABELLED: Hematological cancers, including Leukemias and Lymphomas, and their associated chemotherapy and disease-specific factors, are linked to impaired granulocyte function and numbers, increasing the risk of opportunistic infections, often presenting as fever. Human cytomegalovirus (HCMV) is one of the significant opportunistic infections in these patients, but limited data exists on its seroprevalence and active infection burden among febrile hematological cancer patients in Uganda. We conducted a cross-sectional study from June to August 2017 at the Uganda Cancer Institute (UCI). Blood

Association Between Environmental Factors and Toxigenic Clostridioides difficile Carriage at Hospital Admission.

IMPORTANCE: Clostridioides difficile infection is the most frequent health care-associated infection in the United States. However, exposure to this organism might occur outside the health care setting. OBJECTIVE: To examine whether exposure to environmental factors, such as livestock farms, is associated with a higher probability of being colonized with C difficile at hospital admission. DESIGN, SETTING, AND PARTICIPANTS: This retrospective cohort study was conducted from May 1, 2017, to June 30, 2018, at a teaching-affiliated hospital in Milwaukee, Wisconsin. All consecutive patients under

An overview of genetic predisposition to familial hematological malignancies.

Genetic predisposition has been always noted in the context of familial hematological malignancies. Epidemiological studies have provided evidence consisting of an increased risk to develop blood cancer in relatives diagnosed with the same pathology and characterized by early age at diagnosis and higher severity compared to sporadic forms. With the emergence of new genomic testing approaches, the prevalence of familial aggregations of hematological malignancies seems to be under estimated. The heterogeneity of clinical features explains the wide number of genes' mutations reported to date and

Pembrolizumab-Induced Aplastic Anemia Successfully Treated With Thrombopoietin Receptor Agonist in a Patient With Lung Adenocarcinoma: A Case Report and Literature Review.

A 71-year-old female with postoperative recurrence of lung adenocarcinoma was treated with chemotherapy plus immunotherapy (carboplatin, pemetrexed, and pembrolizumab). On day eight after the first cycle, she presented with fever and loss of appetite. Laboratory tests showed severe pancytopenia. Bone marrow biopsy revealed hypocellular marrow consistent with aplastic anemia (AA), which is considered an immune-related adverse event (irAE). Treatment with pulse glucocorticoid (GC) therapy, granulocyte colony-stimulating factor, and cyclosporine had a limited effect. On day 24, eltrombopag, a thr

Evaluation of the dyspneic patient in the office.

Dyspnea is a nonspecific symptom of any disease involving the respiratory system. Although diseases of the lungs, chest wall, pleura, diaphragm, upper airway, and heart are most common, diseases of many other organ systems (eg, neuromuscular, skeletal, renal, endocrine, rheumatologic, hematologic, and psychiatric) may involve the respiratory system and present with dyspnea. Dyspnea should be evaluated systematically, and a thorough history and physical examination and baseline tests of heart and lung function are necessary to establish a complete database. More sophisticated testing may be nee

Targeting the Ras pathway in pediatric hematologic malignancies.

PURPOSE OF REVIEW: Ras pathway mutations are one of the most common type of alterations in pediatric hematologic malignancies and are frequently associated with adverse outcomes. Despite ongoing efforts to use targeted treatments, there remain no Food and Drug Administration (FDA)-approved medications specifically for children with Ras pathway-mutated leukemia. This review will summarize the role of Ras pathway mutations in pediatric leukemia, discuss the current state of Ras pathway inhibitors and highlight the most promising agents currently being evaluated in clinical trials. Cobalt hip prosthesis intoxication mimicking an autoimmune disease.

Cobalt-containing hip prosthesis may cause systemic toxicity due to the release of cobalt from metal-on-metal (MoM) joint arthroplasty into the bloodstream. High cobalt blood levels can lead to a variety of clinical manifestations, mimicking other disorders, especially autoimmune, hematologic, and infectious diseases. Our purpose is to describe a clinical case of cobalt hip prosthesis intoxication mimicking an autoimmune disease, with systemic inflammation signs, arthro-myalgias unrelated to overt synovitis, and multiple autoantibody positivity. A 69-years-old woman presented with a 1-year his

Blood Cells and Interferon-Gamma Levels Correlation in Latent Tuberculosis Infection.

The Mycobacterium tuberculosis (M. tb) infection is largely spread in world's population. Most infected individuals develop latent tuberculosis infection (LTBI). Tuberculin skin test (TST) and interferon-gamma release assays (IGRAs) are the available tests to detect the infection. It has been reported that some individuals take a longer period of time to develop the infection than others with the same exposure level. It is suggested that the innate immunity, in which neutrophils have an important protective role, is responsible for this. Many hematologic abnormalities have been described as co

When Inflammation Mimics Malignancy: Recognizing VEXAS Syndrome in Oncology Practice.

INTRODUCTION: VEXAS syndrome is a recently identified clonal hematoinflammatory disorder caused by acquired somatic mutations in the UBA1 gene in hematopoietic stem and progenitor cells. Its discovery established a paradigm linking clonal hematopoiesis with systemic inflammation and progressive bone marrow dysfunction. Patients commonly present with persistent cytopenias and multisystem inflammatory manifestations that overlap with myelodysplastic syndromes, other clonal hematologic disorders, and complications encountered in oncology practice. As a result, affected individuals often are evalu
