Preparing for the real world. Program helps rehabilitation patients perform everyday tasks.

St. Francis Health Care Centre in Green Springs, OH, decided in 1991 to enlist local merchants in a program in rehabilitative medicine. The program, named for Green Springs's main street, is "Broadway: A Route Home." Broadway is a program for persons who, because of severe illness or injury, require extended rehabilitation. Along with care for continuing physical or cognitive problems, such patients often need help in performing tasks--buying groceries, cashing a check, renting a video--that most people take for granted. Under the Broadway program patients can practice these tasks safely in sh

Discharge Against Medical Advice in the United States, 2002-2011.

OBJECTIVE: To describe the national frequency, prevalence, and trends of discharge against medical advice (DAMA) among inpatient hospitalizations in the United States and identify differences across patient- and hospital-level characteristics, overall and in clinically distinct diagnostic subgroups. PATIENTS AND METHODS: We conducted a retrospective, cross-sectional analysis of inpatient hospitalizations (≥18 years), discharged between January 1, 2002, and December 31, 2011, using the Nationwide Inpatient Sample. Descriptive statistics, multivariable logistic, and joinpoint regression were us

[The occupational risk in IRCCS of Neurosciences].

The results of meta-analysis carried out on all documents for the valuation of the risk, recorded by IRCCS of Neuroscience, has led to identify a risk matrix for the medical personnel of the Scientific Institute of Neuroscience. Such analysis allowed to tackle the day to day risk faced by hospital staff. As a general rule the IRCCS of Neurosciences distinguishes themselves as structures of mainly neurologic typology and as structures of more typical psychiatric interest. In the first case the analogy with the traditional Hospital structures are more noticeable, in the second case instead the k

Using non-verbal communication to support de-escalation in patients experiencing acute psychosis in the emergency department.

Emergency nurses are frequently the first point of contact for patients presenting to the emergency department (ED) with the symptoms of acute psychosis. Altered perception, fear and disorganised thinking can impair communication and precipitate distress, which may lead to agitation or aggressive behaviour, placing patients and staff at increased risk of harm. In these circumstances, verbal reasoning alone may be ineffective. Evidence indicates that non-verbal communication, such as body posture, eye-level alignment, proximity and tone of voice, can have a pivotal role in these situations by p

The impact of knowledge brokering on nurses' empathy with patients receiving cardiac care: an experimental study.

BACKGROUND: Patients with cardiovascular diseases often experience fear of death, depression and anxiety, all of which are linked to a heightened risk of future cardiac events. Research indicates that improved empathy is associated with a reduced risk of such events, making the enhancement of empathy among cardiac nurses crucial. Knowledge brokering, a strategy that utilises various interventions to strengthen practice, is key to achieving this. PURPOSE: This study aims to examine the impact of knowledge brokering on nurses' empathy towards patients receiving cardiac care. METHODS: This expe

Dispensing Methadone at Hospital Discharge: One Hospital's Approach to Implementing the "72-hour Rule" Change.

OBJECTIVES: Methadone for opioid use disorder treatment in ambulatory settings is restricted to federally licensed opioid treatment programs (OTPs) in the United States. However, these restrictions do not apply during hospitalization. A recent change to the rule governing methadone in non-OTP settings created an opportunity to dispense methadone at hospital discharge for up to 72 hours. METHODS: Here, we describe one hospital's approach to dispensing methadone at discharge in alignment with the "72-hour rule," including implementation challenges and considerations for other hospitals planning

Peer recovery coaches and emergency department utilization in patients with substance use disorders.

BACKGROUND: Although Emergency Departments (ED) frequently provide care for patients with substance use disorders (SUD), there are many barriers to connecting them with appropriate long-term treatment. One approach to subside risk in this population is the Peer Recovery Coach (PRC). PRCs are individuals with a lived experience of the rehabilitation process and are a powerful resource to bridge this gap in care by engaging patients and their families and providing system navigation, self-empowerment for behavior change, and harm reduction strategies. The purpose of this project is to describe a

Healthcare Utilization of Patients with Opioid Use Disorder in US Hospitals from 2016 to 2019: Focusing on Racial and Regional Variances.

BACKGROUND: There is a lack of US population-based research on healthcare utilization differences caused by opioid misuse. OBJECTIVE: The aim of this study was to explore disparities in healthcare utilization by type of opioid use disorder, race, region, and other patient factors for a more targeted prevention and treatment program. METHODS: The National Inpatient Sample of the United States was used to identify patients with opioid use disorder (n = 101,231, weighted n = 506,155) from 2016 to 2019. Type of opioid use disorder was defined as opioid dependence/unspecified use, adverse effects

Dynamics of nurses' authority in the inpatient care of adolescent consumers with anorexia nervosa: A qualitative study of nursing perspectives.

Nurses caring for adolescent consumers with anorexia nervosa in the inpatient setting are challenged in a unique way, in that they are caring for people with whom they do not have a mutually held concept of well-being. Their efforts to ensure weight gain are frequently against the wishes of the consumer. This dissonance results in challenging interactions, where nursing care and authority may be undermined. This study investigated the dynamics of nurses' authority within this context. Interviews with nurses (n = 10) were conducted and analysed through thematic analysis. Nurses reported that co

Nursing the physically disabled in a general hospital ward.

Progress in medical treatment over the last three decades has meant that many more people survive either traumatic birth situations or diseases/injuries which would have previously resulted in death, and are leading normal lives with some form of physical disability. Until quite recently, little attention has been given to their longer-term health needs. The following article looks at the attitudes of nurses towards physically disabled patients, the experiences and attitudes of these patients, the interaction between nurse and patient, practical outcomes and future directions. Literature on th

Comprehensive integrated NGS-based surveillance and contact-network modeling unravels transmission dynamics of vancomycin-resistant enterococci in a high-risk population within a tertiary care hospital.

Vancomycin-resistant E. faecium (VRE) are an important cause of nosocomial infections, which are rapidly transmitted in hospitals. To identify possible transmission routes, we applied combined genomics and contact-network modeling to retrospectively evaluate routine VRE screening data generated by the infection control program of a hemato-oncology unit. Over 1 year, a total of 111 VRE isolates from 111 patients were collected by anal swabs in a tertiary care hospital in Southern Germany. All isolated VRE were whole-genome sequenced, followed by different in-depth bioinformatics analyses includ

The birth of a child with a congenital anomaly. Part I. Some difficulties experienced by parents in the maternity home.

An investigation study of 90 cases in which a baby had been born with cystic fibrosis, oesophageal atresia, Hirschsprung's disease or an anorectal malformation was undertaken to establish the duration of the prediagnostic phase and the difficulties experienced by parents before discharge from the maternity home. The congenital anomalies requiring immediate surgical intervention were diagnosed very soon after birth. A wider diversity of problems were encountered in the case of those conditions with a longer prediagnostic phase. The problems encountered by parents, viz. lack of communication wit

Hospital admissions and deaths due to congestive heart failure in New Zealand, 1988-91.

AIMS: Congestive heart failure is believed to be a major public health problem in most Western countries; however, little is known about the extent of morbidity and mortality from congestive heart failure in New Zealand. This paper reports data on hospital admissions and mortality due to congestive heart failure in New Zealand during the years 1988-91. METHODS: All data were obtained from the New Zealand Health Information Service. Deaths from congestive heart failure were identified from ICD-9 codes indicating a primary diagnosis of congestive heart failure. Hospitalisations for congestive h

"They say they listen. But do they really listen?": A qualitative study of hospital doctors' experiences of organisational deafness, disconnect and denial.

The sharing of information and feedback directly from service-providing staff to healthcare organisational management is vital for organisational culture and service improvement. However, hospital doctors report feeling unable to communicate effectively with management to provide evidence and affect improvement, and this can impact job satisfaction, workplace relations, service delivery and ultimately patient safety. In this paper, we draw on data elicited from a Mobile Instant Messaging Ethnography (MIME) study involving 28 hospital doctors working in Irish hospitals, to explore the barriers

The ethics of caring for hospital-dependent patients.

BACKGROUND: Hospital-dependent patients are individuals who are repeatedly readmitted to the hospital because their acute medical needs cannot be met elsewhere. Unlike the chronically critically ill, these patients do not have a continuous need for life-sustaining equipment and can experience periods of relative stability where they have a good quality of life. However, some end up spending months or even years in the hospital receiving resource-intensive care because they are unable to be safely discharged, despite an initial optimistic prognosis. It is hard to reliably identify these patient

Conflict in a paediatric hospital: a prospective mixed-method study.

BACKGROUND: Conflict in healthcare is a well-recognised but under-examined phenomenon. Little is known about the prevalence and causes of conflict across paediatric specialties. OBJECTIVE: To report the frequency and characteristics of conflict in a paediatric hospital. DESIGN AND SETTING: An explanatory sequential mixed-method approach was adopted. A bespoke questionnaire recorded frequency, severity, cause and staff involved in conflict prospectively. Data were recorded for the same two 12-week periods in 2013 and 2014, in one UK children's teaching hospital. Data were analysed using descr

[Analysis of the structure of admissions and the course of hospitalization of drug addicts treated at the I Clinic for Mental Diseases, Medical Academy, in Gdańsk 1982-1988].

The structure of admissions and the course of hospitalization were evaluated in the group of 78 (48 men, 30 women) with diagnosis of psychoactive substance dependence hospitalized in the Ist Clinic of Psychiatry in Gdańsk during the period of 1982-88. The mean duration of hospitalization lasted 24 days. The most numerous (59%) group of opioid dependent patients was characterized by the shortest time of hospitalization. The longest duration of the abstinence symptoms was observed in the group of multiple-substance+ dependent patients. 62% of them were taking the drugs during hospitalization. In
