Ischaemia in tuberculous meningitis: update on pathophysiology, clinical predictors and management.

PURPOSE OF REVIEW: Tuberculous meningitis (TBM) is a severe manifestation of Mycobacterium tuberculosis infection, associated with high mortality and long-term neurological disability. Cerebral ischaemia and infarction are major contributors to poor outcomes, yet the underlying mechanisms remain incompletely understood. This review summarises current understanding of the pathophysiology, predictors, and emerging therapies for TBM-associated ischaemia, highlighting critical research priorities. RECENT FINDINGS: Stroke in TBM reflects a complex interplay of neuroinflammation, immunothrombosis,

Management of Sepsis and Septic Shock: What Have We Learned in the Last Two Decades?

Sepsis is a clinical syndrome encompassing physiologic and biological abnormalities caused by a dysregulated host response to infection. Sepsis progression into septic shock is associated with a dramatic increase in mortality, hence the importance of early identification and treatment. Over the last two decades, the definition of sepsis has evolved to improve early sepsis recognition and screening, standardize the terms used to describe sepsis and highlight its association with organ dysfunction and higher mortality. The early 2000s witnessed the birth of early goal-directed therapy (EGDT), wh

Genetic and pharmacological inhibition of inflammasomes reduces the survival of Mycobacterium tuberculosis strains in macrophages.

Mycobacterium tuberculosis infection causes high rates of morbidity and mortality. Host-directed therapy may enhance the immune response, reduce tissue damage and shorten treatment duration. The inflammasome is integral to innate immune responses but over-activation has been described in tuberculosis (TB) pathology and TB-immune reconstitution syndrome. Here we explore how clinical isolates differentially activate the inflammasome and how inflammasome inhibition can lead to enhanced bacterial clearance. Wild-type, Nlrp3-/-/Aim2-/-, Casp1/11-/- and Asc-/- murine bone-marrow derived macrophages …

How I treat refractory chronic graft-versus-host disease.

Approximately 35% to 50% of patients otherwise cured of hematologic malignancies after allogeneic hematopoietic stem cell transplantation will develop the pleomorphic autoimmune-like syndrome known as chronic graft-versus-host disease (cGVHD). Since in 2005, National Institutes of Health (NIH) consensus panels have proposed definitions and classifications of disease to standardize treatment trials. Recently, the first agent was approved by the US Food and Drug Administration for steroid-refractory cGVHD. Despite these advances, most individuals do not achieve durable resolution of disease acti

Neonatal Systemic AAV Induces Tolerance to CNS Gene Therapy in MPS I Dogs and Nonhuman Primates.

The potential host immune response to a nonself protein poses a fundamental challenge for gene therapies targeting recessive diseases. We demonstrate in both dogs and nonhuman primates that liver-directed gene transfer using an adeno-associated virus (AAV) vector in neonates induces a persistent state of immunological tolerance to the transgene product, substantially improving the efficacy of subsequent vector administration targeting the central nervous system (CNS). We applied this approach to a canine model of mucopolysaccharidosis type I (MPS I), a progressive neuropathic lysosomal storage

Cytomegalovirus infection impairs immunosuppressive and antimicrobial effector functions of human multipotent mesenchymal stromal cells.

Human mesenchymal stromal cells (MSC) possess immunosuppressive and antimicrobial effects that are partly mediated by the tryptophan-catabolizing enzyme indoleamine-2,3-dioxygenase (IDO). Therefore MSC represent a promising novel cellular immunosuppressant which has the potential to control steroid-refractory acute graft versus host disease (GvHD). In addition, MSC are capable of reducing the risk of infection in patients after haematopoietic stem cell transplantation (HST). Recent data indicate that signals from the microenvironment including those from microbes may modulate MSC effector func

Biofilm-host immunometabolic crosstalk in Staphylococcus aureus orthopedic implant infection: mechanisms and translational constraints.

Orthopedic implant biofilms are not only antibiotic-tolerant bacterial communities but metabolically active niches that reshape host immunity. Compared with planktonic infection, mature biofilms impose spatial and metabolic constraints that can uncouple leukocyte recruitment from sterilizing activity. Neutrophils can penetrate and phagocytose portions of Staphylococcus aureus biofilms, but clearance becomes incomplete as biofilms mature and may coexist with degranulation and neutrophil extracellular trap-associated tissue injury. Macrophages show impaired access and bactericidal programming, a

Phage-quinolone synergy: mechanisms, clinical strategies, and translational prospects.

The escalating global crisis of bacterial antimicrobial resistance (AMR) has significantly limited the clinical efficacy of quinolone antibiotics, while the use of phages alone suffers from inherent limitations, including a narrow host spectrum and the propensity to rapidly induce phage resistance. Consequently, combined antimicrobial therapy, defined as phage-antibiotic synergy (PAS), has emerged as a pivotal direction to address the challenges. The specific objectives of this review are to systematically dissect the bidirectional synergistic and antagonistic interactions between quinolone an

The Emerging Role of Immunothrombosis in the Control and Pathogenesis of Mycobacterium tuberculosis.

Greater understanding of the immunopathogenesis of tuberculosis is critical for developing novel therapies. Here, we propose that immunothrombosis plays an important role in the immune response to Mycobacterium tuberculosis. This interplay among macrophages, neutrophils, and platelets leads to microthrombosis at the site of infection, trapping the mycobacterium to prevent dissemination. We explore how dysregulated immunothrombosis might contribute to tuberculosis pathogenesis, with excessive microthrombosis driving drug resistance, leading to lung damage and venous thromboembolism. Further res

SARS-CoV-2 infection remodels the host protein thermal stability landscape.

The severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) is a global threat to human health and has compromised economic stability. In addition to the development of an effective vaccine, it is imperative to understand how SARS-CoV-2 hijacks host cellular machineries on a system-wide scale so that potential host-directed therapies can be developed. In situ proteome-wide abundance and thermal stability measurements using thermal proteome profiling (TPP) can inform on global changes in protein activity. Here we adapted TPP to high biosafety conditions amenable to SARS-CoV-2 handling. We

Chronic GVHD: review advances in prevention, novel endpoints, and targeted strategies.

Allogeneic hematopoietic cell transplantation (allo-HCT) is a curative therapy for many malignant and non-malignant hematologic disorders. Chronic graft-versus-host (cGVHD) disease remains a significant hurdle for long-term survival in patients post allo-HCT, and it remains the leading cause of late non-relapse mortality. The risk factors for development of cGVHD include degree of human leukocyte antigen (HLA) disparity, increasing recipient age, use of peripheral blood stem cells as a source, myeloablative conditioning regimens, prior acute GVHD (aGVHD), and female donor to male recipient. Severe community-acquired pneumonia: an integral approach.

BACKGROUND: Severe community-acquired pneumonia represents the most critical manifestation of pneumonia acquired outside the hospital and remains a major cause of intensive care admission and death worldwide. It is frequently complicated by acute respiratory failure, circulatory shock, and multiple organ dysfunction, with mortality commonly exceeding 25-40%. Despite progress in vaccination programmes, microbiological diagnostics, and critical care support, major uncertainties persist regarding early recognition, pathogen identification, antimicrobial selection and duration, management of co-in

Comprehensive genome editing and signaling pathway engineering evolve potent and safe adenoviral producer cell.

Dramatic attention has been drawn to adenovirus as a vehicle for vaccine and cancer therapy, due to the capabilities of annihilating tumor cells and stimulating host immune response. However, few producer cells are perfect for adenoviral manufacture. Herein, we orchestrate a strategy by combining homology-independent targeted integration and sgRNA library, which not only modulates the cellular gene expression profile systemically that enhance subgroup B/C adenovirus yields by 3 ~ 7 folds, but also substitutes naïve adenovirus sequence in HEK293 cells with a recombinant E1 transgene, drasticall

Engineering Protein-Based HIV Entry Inhibitors: Advances, Challenges, and Translational Strategies.

Human immunodeficiency virus (HIV) is an enveloped virus with a remarkable capacity for genetic diversification, enabling rapid escape from host immune responses and therapeutic interventions. Despite extensive global efforts, the development of an effective vaccine has remained elusive owing to the virus's high genetic variability and antigenic diversity. Consequently, considerable effort has been directed toward the development of therapeutic agents targeting viral entry, reverse transcriptase, integrase, protease, and more recently, capsid. Although antiretroviral therapy (ART) remains the

Allogeneic CD33-directed CAR-NKT cells for the treatment of bone marrow-resident myeloid malignancies.

Chimeric antigen receptor (CAR)-engineered T cell therapy holds promise for treating myeloid malignancies, but challenges remain in bone marrow (BM) infiltration and targeting BM-resident malignant cells. Current autologous CAR-T therapies also face manufacturing and patient selection issues, underscoring the need for off-the-shelf products. In this study, we characterize primary patient samples and identify a unique therapeutic opportunity for CAR-engineered invariant natural killer T (CAR-NKT) cells. Using stem cell gene engineering and a clinically guided culture method, we generate allogen
