Primary endoprosthesis in comminuted humeral head fractures in patients over 60 years of age.

The purpose of this study was to determine the early subjective and functional results after primary endoprosthesis replacement for comminuted humeral head fractures in patients over 60 years old. Twenty comminuted humeral head fractures in 20 patients were treated primarily (within 3.5 days (range 0-17 days) with a Neer II modular shoulder prosthesis, and were then followed up for an average of 32. 5 months (range 17-44 months). The average age of the patients was 77 years (range 63-91 years). The average post-operative Constant score was 52 points (range 30-76). This was significantly less t

Alteration of Baumann's angle by humeral position: implications for treatment of supracondylar humerus fractures.

To determine the effects of limb positioning on Baumann's angle, we made a controlled radiographic study of a 6-year-old cadaver upper extremity specimen. Radiographic measurement of Baumann's angle was obtained at 10 degrees increments of humeral rotation from 40 degrees of internal rotation to 40 degrees of external rotation. These measurements were obtained with the humerus parallel to the x-ray cassette and with the humerus flexed 30 degrees from the cassette. Measurement of Baumann's angle with radiographs obtained with the humerus parallel to the x-ray cassette was associated with less v

Open reduction and internal fixation of humeral shaft fractures. Results using AO plating techniques.

Thirty-six patients with an acute fracture of the humeral shaft were treated by open reduction and internal fixation using AO plating techniques. In thirty-two patients an open fracture or multiple injuries, or both, were the indications for internal fixation. Four patients were treated after non-operative treatment failed to maintain a satisfactory reduction. Follow-up was possible for thirty-four patients and showed that thirty-three fractures united primarily and one failed to unite, necessitating two subsequent procedures. Two superficial wound infections in patients with an open fracture

Compound fracture of humeral shaft associated with two-part fracture dislocation of ipsilateral shoulder: a rare combination.

Humerus fracture is very rarely associated with ipsilateral shoulder dislocation. Here, we have reported probably the first case of compound fracture of humerus associated with a two-part fracture dislocation of ipsilateral shoulder. A 30-year-old female presented to the emergency department after a road traffic accident. The vehicle in which she was travelling skidded off the road and she got trapped. She had degloving injury over her left arm with a fracture of left humerus and anterior dislocation of ipsilateral shoulder and greater tuberosity fracture. She was managed initially with wound

The isolated medial humeral epicondyle fracture treated nonoperatively: does fracture displacement change over time?

This study explores the change in the position of the fragment in isolated, displaced, medial humeral epicondyle fractures in children. In this series, 34 patients (mean age 11.0 years) were treated nonoperatively by cast immobilization with no attempt at closed fracture reduction. A statistically significant association was found between the position of the fracture fragment at the time of initial fracture compared with 3 weeks after fracture (P=0.015). Twenty-six of 34 patients (76.5%) showed spontaneous improvement in fragment position. These findings support the practice of treating signif

Investigation of metallic and carbon fibre PEEK fracture fixation devices for three-part proximal humeral fractures.

A computational investigation of proximal humeral fracture fixation has been conducted. Four devices were selected for the study; a locking plate, intramedullary nail (IM Nail), K-wires and a Bilboquet device. A 3D model of a humerus was created using a process of thresholding based on the grayscale values of a CT scan of an intact humerus. An idealised three part fracture was created in addition to removing a standard volume from the humeral head as a representation of bone voids that occur as a result of the injury. All finite element simulations conducted represent 90° arm abduction. Simula

Nondisplaced and minimally displaced fractures of the lateral humeral condyle in children: a prospective radiographic investigation of fracture stability.

Subsequent displacement of nondisplaced or minimally displaced fractures of the lateral humeral condyle while immobilized in plaster may contribute to severe complications. The possibility of assessing the stability of such fractures may be a help in planning the initial treatment. Our prospective investigation of 112 children aged 1-11 years aimed at describing radiographic criteria for prognosticating the stability of the fractures. According to the radiographic findings, the fractures were allocated to one of three groups representing stable fractures, fractures with undefinable risk, and f

Efficacy of scapular-focused rehabilitation protocol on shoulder function following Proximal Humeral Internal Locking System fixation for proximal humeral fractures: a randomized controlled trial.

BACKGROUND: Proximal humeral fractures (PHFs) are a significant healthcare burden, representing the third most common osteoporotic fracture in older adults. Although open reduction and internal fixation (ORIF) with the Proximal Humeral Internal Locking System (PHILOS) provides a reliable rate of osseous union, postoperative complications such as shoulder stiffness and residual pain remain prevalent. Current rehabilitation protocols are predominantly glenohumeral-centric and often overlook fracture-associated scapular dyskinesis. This randomized controlled trial (RCT) aims to evaluate whether i…

Open reduction internal fixation of lateral humeral condyle fractures in children. A series of 105 fractures from a single institution.

Lateral humeral condyle fractures account for 17 % of the distal humeral condyle fractures. Displaced and/or rotated fractures require appropriate reduction and stabilisation. There are, however, a number of controversies in the surgical management of these patients. The aim of the present study was to review the results of patients with a displaced lateral humeral condyle fracture treated with open reduction and internal fixation (ORIF). We retrospectively reviewed children treated with ORIF of lateral humeral condyle fractures at a single institution over a period of 13 years. All cases were

Cost effectiveness analysis of operative versus nonoperative management of humeral shaft fractures in Denmark.

PURPOSE: This study aims to compare the cost-effectiveness of operative versus nonoperative management in the Danish healthcare system. METHODS: A decision tree model for treatment options was developed. We used DRG fees and expert opinion to determine the total treatment cost in our institution. Mean wages-included as a component of cost during the time from work-and weeks missed were obtained from Statistics Denmark and the existing literature. The DASH survey, used to determine treatment efficacy, was also extracted from existing literature. An economic evaluation was conducted by use of r

Nerve Recovery in Pediatric Supracondylar Humeral Fractures: Assessing the Impact of Time to Surgery.

BACKGROUND: Nerve injuries in pediatric supracondylar humeral (SCH) fractures occur in 2% to 35% of patients. Previous research has suggested that isolated anterior interosseous nerve injuries are not influenced by the time to surgery; however, little is known about other nerve injuries or mixed, motor, and sensory injuries. With this study, we aimed to examine the impact of time to surgery on nerve recovery in patients with traumatic nerve injuries associated with SCH fractures. METHODS: Patients <18 years of age with SCH fractures stabilized using percutaneous pins during the period of Janu

Age does not affect reliability of the Radiographic Union Score for HUmeral fractures score.

BACKGROUND: A key challenge of humeral shaft fracture management is nonunion after nonsurgical treatment. The Radiographic Union Score for HUmeral fractures (RUSHU) assesses early fracture healing; however, the effect of patient age on its reliability remains unclear. METHODS: In this validation study, 143 adult patients with nonsurgically managed humeral shaft fracture were included (nonelderly 18-64 years, n = 71; elderly ≥65 years, n = 72). Six raters with varying experience (medical students, residents, consultants) independently scored 6-week radiographs using RUSHU. Intra-rater and inte…
