[Unreamed locking intramedullary nailing of humeral fractures--basic evaluation of a patient group].

PURPOSE OF THE STUDY: The aim of the work is to provide an overview of the existing experience in Synthes unreamed humeral nail (UHN). MATERIAL: Between August 1996 and February 2000 we treated by means of UHN a group of 37 patients (12 men and 25 women), average age 55 years (range, 18-87 years) with a fracture of humeral shaft. According to AO classification in 10 patients it was a case of a fracture of proximal humerus involving the shaft (11B-3) and in 27 cases a fracture of the shaft (12 A, B, C). The whole group was operated on by 11 surgeons. METHOD: In 26 cases we used the antegrade

[Experiences with primary osteosynthesis in radial head fractures].

Displaced fractures of the radial head are indications for early surgery. The resection is unsatisfactorily leading often to an instable elbow in valgus deformity and to painful troubles in the distal radio ulnar joint. Prosthesis implantation has its own problems. Reconstructive surgery is obviously the best solution. 21 internal fixations in 20 patients were performed using small screws (and 2 plates) between 1968 and 1976. The functional and radiological results are excellent in 15 out of 19 controlled patients. 3 patients have a moderate reduction of the mobility, 1 patient a non-union. -W

Surgical treatment of coronal shear fractures of the distal humerus with an intact lateral epicondyle by the lateral combined approach.

BACKGROUND: Coronal shear fractures of the distal humerus are not only rare and prone to misdiagnosis, but their surgical treatment can be challenging. We aimed to investigate the feasibility of exposing distal humeral coronal shear fractures with a combined lateral approach that preserves the extensors and lateral ulnar collateral ligament (LUCL) and to analyze the clinical efficacy of open reduction and internal fixation in the treatment of these injuries. METHODS: We included 45 patients who sustained coronal shear fractures of the distal humerus with the lateral epicondyle intact and were

Osteochondral flap avulsion fracture in a child with forearm compartment syndrome.

Traumatic elbow dislocations account for approximately 3% to 6% of childhood upper extremity injuries, and 41% of these dislocations are concomitant injuries around the elbow. Most of these injuries are associated with a fall on an outstretched hand. Compartment syndrome is one recognized complication after elbow dislocation and distal humeral and radial neck fracture. Compartment syndrome in the forearm is a devastating complication in children with forearm, elbow, and supracondylar fractures. Compartment syndrome occurs as the result of hypoxic damage caused by interruption of the circulatio

Outcomes Following Surgical Fixation of Upper Extremity Fractures in Patients with Chronic Kidney Disease.

PURPOSE: Moderate to severe (stage III-IV) chronic kidney disease (CKD) and end stage renal disease (ESRD) have been shown to be independent risk factors for sustaining a fragility fracture. High rates of complications and mortality are associated with fracture fixation in patients with CKD, but existing literature is limited. It is unknown how CKD stage III-IV or ESRD affects outcomes in upper-extremity fractures. We hypothesize that patients with CKD stage III-IV or ESRD will have high complication rates after surgical fixation of upper extremity fractures. METHODS: We identified all patien

Designing an Anatomical Plate to Improve Biomechanics and Stability for Distal Humerus Fractures: FEM Investigation and the Influence of Meshing Strategy.

OBJECTIVES: Distal humerus fractures require stable fixation to restore elbow function, yet conventional plates often exhibit a geometric mismatch with bone morphology, potentially compromising biomechanical performance. Our finite element analysis (FEA) compared a novel anatomical plate designed to conform to native humeral geometry with a conventional plate for treating type B1 distal humerus fractures. The influence of meshing strategy on the outcomes was also evaluated. Both adaptive and uniform approaches were considered. METHODS: A CT-based three-dimensional humerus model with heterogen

Delayed radial nerve palsy due to entrapment of the nerve in the callus of a distal third humerus fracture.

A 45-year-old male patient was referred for management of radial nerve palsy. His past medical history revealed that he had been injured in a car accident and broken his left humerus 4 months ago. Primary stabilization of the fracture has been achieved by the application of a long-arm plaster cast. His medical reports displayed that he had experienced no clinical signs of radial nerve palsy at that time. After the cast was removed, he noticed that he could not extend his wrist. Surgical exploration revealed that the radial nerve was encased inside the callus. He had probably not realized the w

Recalcitrant nonunion of the distal humerus: treatment with free vascularized bone grafting.

UNLABELLED: We sought to determine whether open reduction and internal fixation in combination with free vascularized bone grafting and elbow contracture release is an effective treatment for patients with recalcitrant distal humeral nonunions with segmental bone loss. In addition, we wondered whether this treatment strategy has an acceptable complication rate. Five patients, with an average age of 48 years, form the basis of our study. An average of 3.4 surgical procedures were done before the vascularized bone grafting for treatment of nonunion. The average time from injury until the index v

Bone metastases in vulvar cancer: a rare metastatic pattern.

Bone metastases from a vulvar carcinoma are exceptionally rare with only five reported cases in the literature. We report on a patient who was initially treated with radical vulvectomy and bilateral inguinal lymphadenectomy for a vulvar cancer (pT2, pN2 (6/37), M0; G2). Due to a positive nodal status, adjuvant radiation of the vulva and the pelvis was performed additionally. The patient presented 4 months after initial therapy with severe pain in the right humeral shaft due to a pathologic fracture based on an osteoclastic metastasis. During osteosynthetic stabilization histologic and immunohi

Missed Thrower's Fracture of the Humerus in a Pediatric Athlete: A Case Report.

BACKGROUND: Humerus fractures caused by the throwing motion are extremely rare. They have been reported mostly in recreational adult athletes in their third or later decades of life. A pediatric thrower's fracture is even less common, with few reported cases. The pediatric version of this fracture is located in the proximal to midshaft humerus, distinguishing it from the adult type, which occurs in the middle to distal shaft. CASE REPORT: A 12-year-old male pitcher experienced a "snap" in his right arm while throwing a pitch in a baseball game. He presented to the Emergency Department with ri

[Intramedullary stabilisation of metastatic fractures of long bones].

UNLABELLED: Surgical treatment of metastatic fractures of long bones is mostly a palliative one. Intramedullary stabilisation without resection of metastases follows the aim of the palliative therapy concept. PATIENTS AND METHODS: From 01.01.1995 to 30.08.2001 36 manifest and 4 impending long bone fractures were registered in 22 female and 16 male patients, with a mean age of 63 years. In addition, one revision was necessary due to persisting instability after humeral intramedullary bundle nailing. Pathological fractures were found in the humerus (n=11), femoral neck (n=6), per- and subtrocha

Novel Dual Bicolumnar Fibula Graft Reconstruction of Distal Humerus in Pediatric Patients After Massive Bone Resection: A Report of Two Cases.

CASE: We describe 2 case studies, involving a 10-year-old girl with an aneurysmal bone cyst and a 12-year-old adolescent boy with Ewing sarcoma. The patient with Ewing sarcoma was previously managed with wide surgical excision and fibular graft reconstruction and subsequently experienced significant graft resorption, hardware failure, and fracture 24 months after operation. A revision limb salvage attempt was undertaken. In both cases, fibular strut grafts were harvested and fixed with intramedullary k-wires to recreate the medial and lateral columns of the distal humeral triangle. CONCLUSION

Supracondylar osteotomies of posttraumatic distal humeral deformities in young adults - technique and results.

BACKGROUND: Cubitus varus deformity is the most common late complication after distal humeral fractures in children. Typical symptoms are increasing instability especially the posterolateral rotatory instability (POLRI), lateral elbow pain and cosmetic problems. Different ways of correction have been described but a gold standard has not yet been established. METHODS: In this study the clinical outcome 6,5 months after supracondylar closed wedge osteotomy stabilized with locking plates in four young adults was investigated: three with a posttraumatic varus deformity and one with a posttraumat

Preliminary experience with biodegradable implants for fracture fixation.

BACKGROUND: Biodegradable implants were designed to overcome the disadvantages of metal-based internal fixation devices. Although they have been in use for four decades internationally, many surgeons in India continue to be skeptical about the mechanical strength of biodegradable implants, hence this study. MATERIALS AND METHODS: A prospective study was done to assess the feasibility and surgeon confidence level with biodegradable implants over a 12-month period in an Indian hospital. Fifteen fractures (intra-articular, metaphyseal or small bone fractures) were fixed with biodegradable implan

A Biomechanical Study of Tuberosity-Based Locked Plate Fixation Compared with Standard Proximal Humeral Locking Plate Fixation for 3-Part Proximal Humeral Fractures.

BACKGROUND: One of the main shortcomings of current proximal humeral plate designs is their inability to reliably secure the greater tuberosity (GT) or lesser tuberosity, leading to fixation failure, nonunion, and rotator cuff dysfunction. Traditional proximal humeral locking plates (PHLPs) rely on isolated screw fixation or suture repair to maintain reduction of the greater and/or lesser tuberosities. This study evaluates a tuberosity-based plate (TBP) specifically designed to improve tuberosity fixation, which may decrease tuberosity displacement and related clinical sequelae. METHODS: Five

The Humeral Bone Loss (HUMBL) classification identifies patterns of humeral bone loss associated with various revision, instability, and humeral loosening rates in primary and revision shoulder arthroplasty.

BACKGROUND: Humeral Bone Loss (HUMBL) is commonly encountered in primary and revision shoulder arthroplasty and can lead to complications including humeral loosening or instability. The HUMBL classification categorizes bone loss into 5 major patterns of increasing severity: 1) Intact/No substantial bone loss (including bone compromised by prior implants/hardware or abnormal bone quality) 2) isolated greater tuberosity loss or compromise (including acute fracture, nonunion and malunion) 3) segmental bone loss (bone loss distal to the surgical neck) 4) ectatic bone loss (expansion of the cortex

Epiphyseal separation of the trochlea due to stress-throwing injury in a pediatric baseball player: A case report and review of the literature.

Distal humeral epiphyseal separation (DHES) is rare and sometimes difficult to diagnose by radiography. This report presents a rare case of an 8-year-old male baseball player with epiphyseal separation of the trochlea caused by a stress-throwing injury. The patient sustained an elbow injury after throwing a ball. Ultrasound and magnetic resonance imaging confirmed epiphyseal separation of the trochlea and avulsion fracture of the capitellum. Open surgery was performed using 1.5 mm diameter K-wires. After 5 weeks of immobilization, an active range of motion was allowed. Bone union was achieved
