Beclin-1-dependent autophagy protects perivascular adipose tissue function from hyperaldosteronism effects.

Hyperaldosteronism (HA), characterized by excessive production of aldosterone (Aldo), contributes to cardiovascular damage and perivascular adipose tissue (PVAT) dysfunction. Previous studies have shown that Aldo can impair autophagy in various tissues. However, it remains unclear whether this impairment occurs specifically in PVAT and whether it involves disruption of autophagic flux through Beclin-1 (BCN1), a key regulator of autophagosome formation and maturation. We hypothesize that BCN1-dependent autophagy plays a protective role in PVAT by limiting inflammation and preserving its anticon…

Chemogenetic activation of adrenocortical Gq signaling causes hyperaldosteronism and disrupts functional zonation.

The mineralocorticoid aldosterone is produced in the adrenal zona glomerulosa (ZG) under the control of the renin-angiotensin II (AngII) sys … within the adrenal cortex. This transgenic mouse also provides an inducible and reversible model of hyperaldosteronism to investigate PA therapeutics and the mechanisms leading to the damaging effects of aldosterone on the cardiovascular system.

Latent Autonomous Cortisol Secretion From Apparently Nonfunctioning Adrenal Tumor in Nonlateralized Hyperaldosteronism.

CONTEXT: Adrenal tumors (ATs), even those diagnosed as nonfunctioning, may cause metabolic disorders. Some primary aldosteronism (PA) patients with ATs are diagnosed with bilateral PA based on adrenal venous sampling (AVS), and their ATs are apparently nonfunctioning. OBJECTIVE: To clarify the influence of apparently nonfunctioning ATs, we compared hormone levels and clinical complications between bilateral PA cases with and without ATs. DESIGN, SETTING, AND PARTICIPANTS: After retrospectively assessing 2814 patients with PA in the multicenter Japan PA study, bilateral PA cases on AVS were d

Case report: schwannoma arising from the unilateral adrenal area with bilateral hyperaldosteronism.

BACKGROUND: We report a rare case of a juxta-adrenal schwannoma that could not be discriminated from an adrenal tumor before surgical resection and was complicated by bilateral hyperaldosteronism. To the best of our knowledge, this is first case in which both a juxta-adrenal schwannoma and hyperaldosteronism co-existed. CASE PRESENTATION: A 69-year-old male treated for hypertension was found to have a left supra-renal mass (5.8 × 5.2 cm) by abdominal computed tomography. His laboratory data showed that his plasma aldosterone concentration (PAC) was within the normal range, but his plasma reni…

Case Report: Nodule Development From Subcapsular Aldosterone-Producing Cell Clusters Causes Hyperaldosteronism.

CONTEXT: We previously reported that the human adrenal cortex remodels to form subcapsular aldosterone-producing cell clusters (APCCs). Some APCCs were recently found to carry aldosterone-producing adenoma (APA)-associated somatic mutations in ion channel/pump genes, which implied that APCCs produce aldosterone autonomously and are an origin of APA. However, there has been no report describing an APCC-to-APA transitional lesion. CASE DESCRIPTION: A histological examination revealed unilateral multiple adrenocortical micronodules in the adrenals of two patients with primary aldosteronism (PA).

[Target organ damage in hyperaldosteronism versus essential hypertension--the Beer Sheba experience].

BACKGROUND: Primary hyperaldosteronism (PAL) is the main cause for secondary hypertension. PAL also increases the risk for target organ damage. OBJECTIVE: To evaluate the rate of target organ damage in PAL patients versus essential hypertension patients (EHT) and to compare the rate of target organ damage in PAL patients who had adrenal adenoma versus patients with no adrenal adenoma. METHODS: The study population included patients from the outpatient hypertensive clinic at Soroka University Medical Center between the years 2000-2005. The diagnosis of PAL was performed according to standard …

Laparoscopic adrenal-sparing surgery for primary hyperaldosteronism due to aldosterone-producing adenoma.

BACKGROUND: Laparoscopic adrenalectomy has been shown to be safe and effective in the treatment of patients with primary hyperaldosteronism due to aldosterone-producing adenoma. Most laparoscopic adrenalectomies for aldosterone-producing adenomas involve total removal of the adrenal gland, and there have been few reports of laparoscopic adrenal-sparing surgery or partial adrenalectomies. METHODS: A prospective review is performed on eight patients with primary hyperaldosteronism due to aldosterone-producing adenoma who underwent laparoscopic transperitoneal adrenal-sparing surgery in our inst…

Peritoneal carcinomatosis following laparoscopic resection of an adrenocortical tumor causing primary hyperaldosteronism.

A clinical syndrome combining hypertension and hypokalemic alkalosis led to the diagnosis of primary hyperaldosteronism, caused by a right-sided, 2 cm large, apparently benign aldosterone-producing adenoma. The adrenal tumor was completely resected by laparoscopic adrenalectomy. Six months after surgery, the patient exhibited a severe relapse of hyperaldosteronism. Extensive peritoneal metastases of a mixed aldosterone- and cortisol-secreting adrenocortical carcinoma were found at abdominal laparotomy. In the light of this case report, we discuss the possibility that laparoscopic resection of …

Hyperaldosteronism after heart surgery in children. Part I: Treatment with aldosterone antagonists.

The course of postoperative hyperaldosteronism and its effect on fluid and electrolyte metabolism were studied in children undergoing open and closed heart surgery. Serum sodium was transiently depressed and red cell sodium concentration remained unchanged. Serum and red cell potassium concentrations were low. Hematocrit did not change significantly during the postoperative period ruling out overhydration. Therefore, the electrolyte changes are interpreted to indicate body potassium loss. That hyperaldosteronism caused potassium loss is suggested by an inverse relationship between plasma aldos…

Concomitant pheochromocytoma and hyperaldosteronism in a 47-year-old man: a case report.

BACKGROUND: The coexistence of pheochromocytoma and hyperaldosteronism is a rare and clinically significant finding with diagnostic challenges that need to be considered in the workup of patients with hypertension. CASE PRESENTATION: This case report describes a 47-year-old Iranian man who initially presented with cold symptoms, chills, and headaches. Despite being diagnosed with panic disorder, his symptoms worsened, leading to a systolic blood pressure crisis. The results indicated elevated levels of 24-hour urine vanillylmandelic acid, metanephrine, and normetanephrine, suggesting increase…

[Adrenalectomy for primary hyperaldosteronism : retrospective review of 57 patients in CHU of Liege].

Primary hyperaldosteronism is the most common cause of secondary hypertension. Diagnostic clarification is essential because surgery is the treatment of choice for patients with unilateral disease (aldosterone-producing adenoma and unilateral cortical hyperplasia). We report a series of 57 patients that had adrenalectomy for primary hyperaldosteronism between April 1994 to January 2022 by the endocrine surgery team of the University Hospital of Liege. Our aim is to evaluate the diagnostic performance of adrenal venous sampling in lateralizing aldosterone hypersecretion and to compare it with o…

A systematic review of pathophysiology and management of familial hyperaldosteronism type 1 in pregnancy.

PURPOSE: Familial hyperaldosteronism type 1 (FH-1) is a rare autosomal dominant form of primary aldosteronism, which features a marked phenotypic heterogeneity, ranging from mild to severe forms of arterial hypertension that can be complicated by stroke and cardiovascular events at a young age. As affected patients usually reach the fertile age, transmission of the disease to offspring is common. Notwithstanding this, reports of FH-1 in pregnancy are limited and there is a lack of treatment guidelines. METHODS AND RESULTS: We searched the PubMed and EuropePMC databases with a PICO strategy to…

Urinary sodium/potassium ratio as a screening tool for hyperaldosteronism in men with hypertension.

Among individuals with hypertension, the prevalence of secondary hypertension has been reported to be ≈10%. More than half of individuals with secondary hypertension have associated hyperaldosteronism. However, given the current clinical environment, these patients often remain undiagnosed. We hypothesized that the urinary sodium/potassium ratio (Na/K) could be used as a simple, low-cost method of screening for hyperaldosteronism among individuals with hypertension in primary care and health examination settings. We recruited hypertensive individuals aged 30-69 years old who were not taking an…
