Disturbances in plasma sodium in patients with war head injuries.

Polyuria with marked plasma sodium disturbance was present in 39 of 224 patients with isolated craniocerebral war injuries. Twenty-one of these 39 patients had hyponatremia (sodium level < 130 mmol/l) and polyuria. Eight of them (38%) died within 30 days after trauma. The remaining 18 patients developed classic diabetes insipidus syndrome, and 6 of them (33%) died of hypernatremia within 30 days after trauma. The mortality in both patients with hyponatremia and patients with diabetes insipidus was higher in those with greater disturbances of plasma sodium concentration with polyuria and those …

[Acute transient Korsakoff's syndrome caused by hematoma of the floor of the 3rd ventricle].

A forty four year-old woman had had an amenorrhea-galactorrhea syndrome for 3 months when a Korsakoff's syndrome with headache suddenly appeared. Neuroradiological investigations revealed a hematoma involving the anterior part of the IIIrd ventricle. After a short period of neurogenic hypernatremia the lesion disappeared with the development of a spontaneous ventriculocisternostomy. Simultaneously, the Korsakoff's syndrome decreased until it went off almost completely, but a diabetes insipidus appeared and the hyperprolactinemia remained unchanged. Four years later, those are the only symptoms…

Risk factors and clinical impact of sodium and potassium disorders in community-acquired pneumonia.

INTRODUCTION AND AIM: Dysnatremia and dyskalemia are common electrolyte disturbances in patients hospitalized with community-acquired pneumo … n levels (p < 0.001). The need for ICU and MV and in-hospital mortality was higher in patients with hypernatremia and hypokalemia, while 6 months mortality was more frequent in patients with hyperkalemia(p < 0,001) and hospital stay was longer among patients with hypernatremia (p = 0.002). CONCLUSION: Electrolyte disturbances at hospital admission are common i …

Challenges in diagnosing and treating Liddle syndrome in resource-limited settings: A case report from Indonesia.

Liddle syndrome, a rare form of monogenic hypertension, poses significant diagnostic and therapeutic challenges due to its phenotypic variab … ion (190/100 mmHg) and bilateral limb weakness. Laboratory results revealed persistent hypokalemia, hypernatremia, and metabolic alkalosis. Low aldosterone levels, alongside clinical and family history, led to the diagnosis of Liddle syndrome. Genetic testing was not conducted due to resource limitations, and ENaC blockers were unavailable. The patients were ma …

Length and weight trajectories during the first 12 months were assessed. Sodium status of collapsed marathon runners.

CONTEXT: Recommendations for prevention and treatment of medical emergencies in participants in marathon races center on maintenance of adeq … osmolar, and none were hyperosmolar. CONCLUSIONS: Our findings indicate a significant incidence of hypernatremia with hyperosmolality and hyponatremia with hypo-osmolality among collapsed runners despite the new fluid intake recommendations, suggesting that either further educational measures are required or that the new guidelines are not entirely adequate to …

Emergency Palliative Care: Early Assessment of an Older Adult With a Fall and Hip Fracture.

In this segment of the emergency department (ED) palliative care case series, we present a patient who arrives to the ED after a fall resulting in a hip fracture. He is also found to have hypernatremia and an acute kidney injury and develops delirium while in the ED awaiting an inpatient bed. The ED-based integrated geriatric palliative care program is consulted and performs a multidimensional assessment. The geriatric palliative care clinician facilitates discussion with his daughter about surgical intervention based on the patient's goals and values, diagnoses delirium, and worsening depress…

Fluid and electrolyte therapy for children.

The major disorder of water and electrolyte metabolism in children is dehydrating diarrhea. The major advance in the treatment of this condition has been the development of oral rehydration therapy, ie, the enteral administration of a balanced glucose-electrolyte solution. This therapy is effective in patients of all ages, dehydration of all degrees short of hypovolemic shock, with gastroenteritis of all causes, and electrolyte disturbances including hypo- and hypernatremia. This review highlights current experimental and clinical studies that have focused on oral rehydration solutions that ha…

Approach to the Metabolic Implications of Peritoneal Dialysis in Acute Kidney Injury.

During the 1970s and 1980s, peritoneal dialysis (PD) was widely accepted as the standard treatment for acute kidney injury (AKI). However, a … rders related to PD are also limitations.Among the metabolic complications of PD are hyperglycemia, hypernatremia, protein loss into the dialysate, and hypercatabolism. Hyperglycemia is caused by the use of dialysate containing high concentrations of glucose. Hypernatremia is a result of short dialysate dwell times during the rapid exchanges of high-volume PD. …
