Penile squamous cell carcinoma (PSCC) is a rare, aggressive malignancy, with HPV driving approximately 50% of cases and correlating with improved outcomes. In this study, we perform single-cell RNA sequencing on tumor microenvironment (TME) cells from 23 treatment-naive PSCC patients, including 11 cases reported previously, and validate our findings in a larger cohort of 85 patients. Our analysis reveals significant immune cell infiltration in PSCC, with HPV+ tumors exhibiting elevated levels of tumor-infiltrating B cells, plasma cells, tertiary lymphoid structures, and IgA secretion. A subset

Congenital deficiencies of the immune system occur in children or adults and can cause severe or recurrent infections. The overall incidence of these immunodeficiency diseases is estimated at approximately 1 in 10,000, excluding selective immunoglobulin A deficiency, but this estimation is based on population studies, not hospital or clinic populations. The majority of immune defects involve antibody production; these immune deficiencies are found more often in adults than infants and children. In an allergy practice, recurrent infections are common, and determining if an immune defect is like

The aims of the present study were to estimate the celiac disease (CD) prevalence in Denmark and the relevance of the test for human leukocyte antigen DQ2 and DQ8 haplotypes (HLA-DQ2/DQ8) for diagnosing CD. The plasma IgA transglutaminase antibody (TGA-IgA) and HLA-DQ2/DQ8 tests should normally be positive for a CD diagnosis. First, we estimated the CD and HLA-DQ2/DQ8 prevalence in the available blood samples collected at the North Zealand Hospital. Out of a total of 9754 patients, with symptoms suggestive of CD (such as malabsorption, diarrhea, steatorrhea, ion-deficiency anemia, and weight l

Both serology and histology remain cornerstones in the diagnosis of coeliac disease. IgA class antibodies against tissue transglutaminase and endomysium have the highest specificity and sensitivity in comparison with other serological tests. An IgA deficiency in serum must always be excluded. Antigliadin antibodies have, because of their low positive predictive value apart from in early childhood, no value as a diagnostic tool in coeliac disease. Intestinal biopsies should be evaluated according to the revised Marsh criteria. A discrepancy between serology and histology requires a competent ev…

A retrospective study on the skin diseases of Chinese Shar Peis was conducted over a 9-year period. Skin disease was found in 58 (49.2%) of the 118 dogs studied. Folliculitis was the most common clinical finding (43 of 58 dogs). In 6 dogs, there was no apparent reason for the folliculitis; however, it was secondary to allergic dermatitis, demodicosis, IgA deficiency, or hypothyroidism in the other dogs. Approximately 20% of the dogs had more than one of these disorders. Serum IgA concentration was measured in 7 dogs and was low in all 7.

A 12 year old boy suffering from p67-phox deficient chronic granulomatous disease presented with a bullous skin disease and a lung infection with paecilomyces species. The histopathology of a bullous lesion showed subepidermal blister formation and microabcesses containing eosinophils in the dermal papillae. By direct immunofluorescence, linear staining of IgA at the dermal-epidermal junction was detected which confirmed the clinical diagnosis of chronic bullous disease of childhood (linear IgA dermatosis).

Defects of humoral immunity are well documented after bone marrow transplantation (BMT). Immunoglobulin recovery can be impaired and selective deficiencies of IgG subclasses have been reported. The nature of these deficiencies may reflect patterns of infection in the post-BMT period. We studied immunoglobulin and IgG subclass recovery in 20 long term (greater than 100 days) survivors of T depleted allogeneic BMT. Although there was no fall in mean levels of IgG, IgM or IgA for the patient group, 14 patients (70%) developed a deficiency of one or more immunoglobulin isotype at some stage post-B

The function and immunoregulation of human IgA memory B cells producing anti-influenza virus antibody was analyzed in vitro in antigen-stimulated cultures. Peripheral blood mononuclear cells (PBMC) from seven of eight normal adult volunteers naturally immunized to influenza virus produced IgA anti-influenza virus antibody when stimulated in vitro with inactivated A/Aichi/68 [H3N2] influenza virus. This IgA antibody response was approximately one-eighth the IgG antibody response. PBMC from each of five patients with selective IgA deficiency failed to produce any measurable IgA antibody. When to…

The Italian Register for Immune Deficiencies was organized in 1977. Seven hundred ninety-seven cases of primary immunodeficiencies, diagnosed according to the World Health Organization criteria, have been registered up to April 1982. In this paper we report the percentages of the different forms of primary immunodeficiencies and the incidence of neoplastic and autoimmune diseases. A prevalence of humoral defects and selective IgA deficiency was found, followed by T-cell disorders. Two and thirty-eight hundredths percent of the patients developed cancer, and 5.89% developed autoimmune diseases.…

Increased amounts of IgG were transmitted into the salivary secretions of patients with elevated serum levels of this immunoglobulin. Both its glandular and extra-glandular transfer apparently depended upon passive diffusion or epithelial `leakage'. High serum levels of IgM (in macroglobulinaemia), and especially local synthesis of this immunoglobulin (in IgA deficiency), enhanced its transfer into the saliva. The transmission through secretory epithelium (glandular transfer) seemed to be an active or selective process, probably dependent upon specific `transfer sites' in the heavy polypetide …

Complement factor I (CFI) deficiency is a rare primary immunodeficiency that disrupts the classical and alternative complement pathways, potentially causing severe recurrent infections and autoimmune manifestations in pediatric patients. However, the coexistence of both pathways in a pediatric patient is extremely uncommon. We report a seven-year-old patient with a rare primary immunodeficiency disorder who presented with recurrent middle ear infections, paronychia, gastrointestinal infections, and respiratory infections. Genetic testing revealed a previously unreported homozygous variant in t

Patients with classic symptoms of celiac disease are often initially tested for serum tissue transglutaminase-immunoglobulin A (tTG-IgA) and total serum immunoglobulin A (IgA) levels concurrently, as IgA deficiency can lead to falsely low tTG-IgA. There are no guidelines for incidental findings of elevated total serum IgA when testing for celiac disease. In our study, we described the proportion of patients with suspicion of celiac disease who had elevated total serum IgA and the factors that may be associated with these findings. We studied the management of these patients with incidental fin…

This article reviews the most common forms of primary immunodeficiencies with articular involvement: X-linked (Bruton's) agammaglobulinaemia, common variable immune deficiency and selective IgA deficiency. The most commonly observed types of arthritis in these disorders are benign, aseptic, characteristically non-erosive polyarthritis and infective arthritis caused by Mycoplasmas or Ureaplasmas. The authors present the case of a patient with immunodeficiency whose polyarthritis was found to be unusually severe and destructive.

The central nervous system has historically been viewed as an immune-privileged site, but recent data have shown that the meninges-the membranes that surround the brain and spinal cord-contain a diverse population of immune cells1. So far, studies have focused on macrophages and T cells, but have not included a detailed analysis of meningeal humoral immunity. Here we show that, during homeostasis, the mouse and human meninges contain IgA-secreting plasma cells. These cells are positioned adjacent to dural venous sinuses: regions of slow blood flow with fenestrations that can potentially permit

RESULTS: None of the subjects were seropositive to IgA anti-tTG antibodies. No association between CD and elevated transaminases was found. Hepatitis B viral infections were one of the main causes of raised transaminases.
CONCLUSION: Before the e …

BACKGROUND: Red cell distribution width (RDW) has been shown as a distinctive marker of mortality and morbidity in a wide spectrum of conditions related to systemic inflammation or deficiency of antioxidant nutrients.
OBJECTIVE: We aimed to investigate the predictive value of RDW in detection of intestinal atrophy in celiac disease (CD).
METHODS: Iron indices and RDW were studied in 49 patients with CD to evaluate the utilization of RDW as a predictive marker for presence of intestinal atrophy.
RESULTS: Sixty-nine percent of patients had iron deficiency at initial presentation and 89% had a

The hyper-IgM syndrome is a rare immunodeficiency disease in which the ability of B cells to switch immunoglobulin production from IgM to IgG, IgA and IgE is defective. The deficiency of IgG and IgA leads to recurrent infections of the respiratory tract, but some patients are susceptible to infection with opportunistic microorganisms, such as Cryptococcus neoformans, and also are prone to neutropenia and mucocutaneous ulcerations. We report a case of a two-year-old boy that was given the diagnosis of the hyper-IgM syndrome on the basis of low serum concentrations of IgG and IgA and high serum
