PURPOSE: Women undergoing ileal pouch-anal anastomosis (IPAA) are frequently within reproductive years and eager to bear children. Management issues have been raised regarding the effects of pregnancy and delivery on the pouch, particularly with respect to obstetric care. Our experience is updated to search for delayed sequelae of pregnancy and delivery and to establish whether other factors have an adverse effect on pouch function. We report a case of colonic volvulus in a 7-year-old child with normal development. Operative findings at laparotomy showed a 270-degree transverse colonic volvulus (TCV), with colonic ligamentous attachment abnormalities. A loop ileostomy was formed following detorsion, with delayed laparoscopic colopexy. This is the first case to describe decompression by diversion ileostomy as acute management of TCV with delayed laparoscopic colopexy, so avoiding unnecessary bowel resection.

Large-bowel obstruction caused by volvulus is potentially life threatening if not managed promptly. Sigmoid colon is the most common site of volvulus followed by caecum. Synchronous double colonic volvulus is an exceedingly rare entity, with only five cases published previously. We present the case of a 77-year-old man with a synchronous sigmoid and caecal volvulus. He underwent total colectomy with ileostomy formation with a view to reverse the ileostomy at a later date.

Atresia of the colon is among the rare types of all gastrointestinal atresias. Ascending colon is the rarest site of all the colonic atresias. The authors report a case of a 4-day-old male baby who presented with the features of distal intestinal obstruction. At laparotomy type I atresia of the ascending colon, just distal to cecum, was found. Primary ceco-colic anastomosis along with a covering ileostomy was performed. Ileostomy was reversed 3 weeks later.

Forty-two patients of typhoid perforation of the gut have been studied. All the patients were subjected to surgery after resuscitation. Simple closure (with or without serosal patch) and temporary ileostomy were the commonly performed surgical procedures. The overall mortality was 14.2%. Postoperative morbidity included wound dehiscence, fistula, encephalopathy, septicaemia, intra-abdominal abscesses and incisional hernia. Faecal fistula formed in 5 out of 42 cases. Ileostomy was found to be the best procedure with regard to morbidity and mortality.

A case has been described in which the diagnosis of acute intestinal obstruction of the fetal small bowel was made antenatally with the aid of ultrasonography. Cesarean section was done in the 34th week of pregnancy. One hour after birth ileostomy and colostomy were performed after resection of a volvulus in the terminal ileum. Two months later the ileostomy and colostomy were closed, and the baby is doing well. The importance of precise diagnosis of the anomaly in utero is emphasized to avoid unjustified termination of pregnancy.

A 17-year-old male with chronic constipation presented with acute obstipation and abdominal distension. Computed tomography showed a dilated colon with rectosigmoid transition. Initial laparotomy and segmental resection confirmed Hirschsprung disease (HD) on histopathology. One month later, he re-presented with abdominal distension. Urgent total colectomy and ileostomy were performed. He recovered after ICU care and was discharged with a functioning ileostomy. This case underscores complications of incomplete resection in adult HD and the need for vigilant follow-up.
