[Fluctuating hyponatremia following transsphenoidal surgery ‒ "The challenge of correcting without going too far"].

Introduction: sodium disorders following pituitary represent one of the most common complications. Differential diagnosis between syndrome of inappropriate antidiuretic hormone secretion (SIADH) and a triphasic response is critical to avoid overcorrection. Case report: we report a case of a 58-year-old male who underwent surgery for a pituitary macroadenoma and developed a triphasic response: initial polyuria, followed by transient SIADH. Due to a suboptimal response to fluid restriction and the risk of transitioning to a new polyuric phase, oral urea (30 g/day) was started. Gradual and safe s

The Utility of Copeptin Measurement in Hospitalized Pediatric Patients with Syndrome of Inappropriate Antidiuretic Secretion.

UNLABELLED: <p>Introduction: Hyponatremia is common in hospitalized pediatric patients, and in many cases, the diagnosis of the syndrome of inappropriate antidiuretic secretion (SIADH) remains challenging, with no gold standard for diagnosis. We assessed factors associated with hyponatremia in pediatric patients clinically diagnosed with SIADH and examined the validity of copeptin level as a useful tool to distinguish SIADH from non-SIADH causes of hyponatremia. METHODS: This observational study retrospectively analyzed 19 patients admitted to Children's Hospital of Orange County in 2021-2024

Hyponatraemia and cancer.

Hyponatraemia is the most common electrolyte alteration in cancer patients and the main cause is the syndrome of inappropriate antidiuresis. In this context, arginine vasopressin secretion can be due to ectopic secretion by tumoral cells or to drugs, including chemotherapeutics. It is known that hyponatraemia is associated with a worse prognosis in cancer. Conversely, the correction of serum [Na+] is associated with a favourable effect on the disease's outcome. Basic research provided evidence that reduced [Na+] activates several intracellular pathways in cancer cells, which lead to an increas

Tolvaptan and urea in paediatric hyponatraemia.

BACKGROUND: The syndrome of inappropriate antidiuretic hormone (SIADH) is usually treated with fluid restriction. This can be challenging in patients with obligate fluid intake for nutrition or medication. Pharmaceutical treatment with tolvaptan and urea is available but minimal paediatric data are available. We review the efficacy and safety of tolvaptan and urea in paediatric patients with SIADH. METHODS: Retrospective review of paediatric inpatients with clinical diagnosis of SIADH. Patients were identified from pharmacy records based on tolvaptan and urea prescriptions. Relevant informati

The syndrome of inappropriate antidiuresis after vaccination against COVID-19: case report.

BACKGROUND: The Syndrome of Inappropriate Antidiuresis (SIADH) has been described to be associated with a multitude of conditions and medications, including the severe acute respiratory syndrome coronavirus 2. We describe the case of a patient with newly diagnosed and symptomatic SIADH after receiving the second COVID-19 vaccination not explained otherwise. CASE PRESENTATION: A 79-year-old male person was admitted to the emergency department due to a worsening of his general health state expressed by weakness, fatigue and anorexia. Vital signs and clinical findings were normal, in particular

Using Tolvaptan to Treat Hyponatremia: Results from a Post-authorization Pharmacovigilance Study.

INTRODUCTION: Hyponatremia is a common condition of varying etiology among hospitalized patients and is associated with adverse outcomes. Treatment to normalize serum sodium is advisable. Tolvaptan received European Union marketing authorization for hyponatremia secondary to the syndrome of inappropriate secretion of antidiuretic hormone (SIADH). Post-marketing pharmacovigilance activities were required to characterize the safety profile of tolvaptan more fully in this population, which is often elderly with a high burden of comorbid illness. METHODS: This was a prospective, observational, mu

An unusual presentation of varicella zoster virus with acute cerebellitis and SIADH without a rash.

We report a case of varicella-zoster virus (VZV) infection with acute cerebellitis and encephalitis with associated Syndrome of Inappropriate Antidiuretic Hormone Secretion (SIADH) in an elderly man presenting with acute cerebellar ataxia without antecedent rash. Cerebrospinal fluid examination (CSF) revealed a mononuclear pleocytosis, high protein, normal glucose, positive for VZV polymerase chain reaction (PCR). Early acyclovir treatment is beneficial for acute VZV cerebellitis. Clinicians should consider infectious Central Nervous System (CNS) causes for presentations of acute cerebellar at

IBD or strongyloidiasis?

INTRODUCTION: Strongyloides has been shown to infrequently mimic inflammatory bowel disease (IBD) or to disseminate when a patient with IBD and unrecognized strongyloides is treated with immunosupression. CASE REPORT: A man from Ecuador, living in Spain for years, with a history of type 2 diabetes mellitus and psoriasis treated with topical corticosteroids, was admitted to the hospital with an 8-month history of diarrhoea. Blood tests showed hyperglycemia, hyponatremia, elevated CRP and faecal calprotectin. Colonoscopy suggested IBD. The patient improved with steroids, pending biopsy results,

Syndrome of inappropriate antidiuretic hormone secretion in a cat with a putative Rathke's cleft cyst.

An 11-year-old spayed female domestic shorthair cat was evaluated for anorexia, lethargy and weight loss of 6 days' duration. Bilateral mydriasis, absent menace response, slow-to-absent pupillary light reflexes, bilateral retinal detachment, intermittent horizontal nystagmus, intermittent ventral strabismus and systemic hypertension were present. Biochemical analysis revealed severe hyponatremia, severe hypochloremia and mild hypokalemia. Multifocal central nervous system disease was suspected based on optic, trigeminal sensory (ophthalmic branch), vestibulocochlear and possible oculomotor ner

Syndrome of inappropriate secretion of antidiuretic hormone in traumatic brain injury: when tolvaptan becomes a life saving drug.

OBJECTIVE: To outline the role of a new drug, tolvaptan, in treating severe and chronic hyponatraemia. Tolvaptan decreases aquaporin expression in renal collecting ducts, by inhibiting antidiuretic hormone (ADH)-V2 receptors, to promote free water clearance. Given its mechanism of action, this drug seems the ideal treatment for the syndrome of inappropriate antidiuretic hormone secretion (SIADH) when the osmotic imbalance leads to life threatening complications. DATA SOURCES: A case is described of severe hyponatraemia deriving from SIADH secondary to meningoencephalitis in a patient admitted

Antipsychotic-induced hyponatraemia: a systematic review of the published evidence.

Hyponatraemia is known to occur as a rare but clinically important adverse reaction to treatment with different psychotropic drugs, including selective serotonin reuptake inhibitors and antiepileptic drugs. In past decades, reports have been published that describe the development of hyponatraemia in association with antipsychotic drug treatment. Our objective was to systematically review the available evidence on antipsychotic-induced hyponatraemia, focussing on patient characteristics, drug dosage, polydipsia and the syndrome of inappropriate secretion of antidiuretic hormone (SIADH). A sear

A rare case of a pancreatic tumor in association with the syndrome of inappropriate antidiuretic hormone secretion.

The syndrome of inappropriate secretion of antidiuretic hormone (SIADH) is a well-known complication in malignancy, especially small cell carcinoma of the lung. However its association with pancreatic carcinoma is rare. To the best of our knowledge, only 9 cases have been reported in the literature. We present the case of a 90 year-old woman with pancreatic carcinoma who developed SIADH.

Hyponatremia associated with repeated use of levetiracetam.

Levetiracetam (LEV) is increasingly used as adjunctive anticonvulsant therapy because of apparent low toxicity. Somnolence, asthenia, headache, dizziness, and nervousness are the most frequently reported side effects (1). We describe a patient, predisposed to the development of the syndrome of inappropriate antidiuretic hormone secretion (SIADH), in whom hyponatremia developed after two challenges of LEV.

The syndrome of inappropriate antidiuretic hormone secretion after bilateral radical neck dissections.

OBJECTIVE: It is commonly believed that synchronous, bilateral radical neck dissection causes the syndrome of inappropriate secretion of antidiuretic hormone (SIADH), presumably as a result of increased intracranial pressure. This belief is based mainly on the results of an experimental study published in 1978 in which occlusion of the superior vena cava in dogs resulted in increased intracranial pressure and SIADH. The purpose of the present study was to test this assumption in an animal model that more closely resembles the clinical condition of bilateral radical neck dissection, in an attem
