Lateral recumbent head-down posture for epidural catheter insertion reduces intravascular injection.

PURPOSE: The unintentional and unrecognized cannulation of an extradural vein is a potentially serious complication of an epidural anesthetic. The present study was undertaken to assess the incidence of blood vessel puncture related to epidural catheterization performed in three different body positions. METHODS: The study was conducted in 900 (three groups of 300) obstetric patients undergoing continuous epidural analgesia during their labour and who were randomly allocated to three groups. Epidural catheterization was performed with patients in the sitting, lateral recumbent horizontal, or

Single-injection lumbar epidural morphine for postoperative analgesia in children: a report of 175 cases.

BACKGROUND AND OBJECTIVES: Since the first report of epidural opioid administration to pediatric patients, several studies have described the quality of analgesia, doses, pharmacokinetics, and side effects of this procedure. A pediatric series using an easy and cheap single-injection technique of epidural morphine administration for postoperative analgesia is presented. METHODS: Postoperative analgesia was achieved with a single lumbar epidural morphine injection (0.1 mg/kg in 0.2 mL/kg saline), which was given via a 22-gauge intramuscular needle to 153 pediatric patients (aged 4 months-17 ye

Epidural test dose and intravascular injection in obstetrics: sensitivity, specificity, and lowest effective dose.

The authors studied the sensitivity and specificity of several epidural test doses as markers of intravascular injection in laboring patients in a prospective double-blind, randomized study. Fifty-nine parturients were assigned randomly to receive an intravenous injection of either normal saline solution (3 mL, NS group) or 1.5% lidocaine with epinephrine 1:200,000 (1 mL, EPI-5 group; 2 mL, EPI-10 group; or 3 mL, EPI-15 group). The EPI-5 and EPI-10 doses were diluted to 3 mL volume with normal saline solution. All injections were given during uterine diastole. Maternal heart rate was monitored

Correlation between epidural space depth measured with ultrasound and MRI compared to clinical loss of resistance when performing lumbar epidural steroid injection.

BACKGROUND: This prospective study assessed the accuracy of MRI and ultrasound (US) measurements as a preprocedural assessment tool for predicting clinical loss of resistance depth (CLORD) during fluoroscopy-guided lumbar epidural steroid injections (ESIs). MATERIALS AND METHODS: Sixty patients enrolled received lumbar ESIs at an academic chronic pain clinic. The MRI measurement calculated the distance between the skin and the posterior epidural space, while US measurements included transverse and parasagittal oblique views of the interlaminar space. The epidural space measurements were compa

The potential contributing effect of ketorolac and fluoxetine to a spinal epidural hematoma following a cervical interlaminar epidural steroid injection: a case report and narrative review.

Cervical interlaminar epidural steroid injections (ESIs) are commonly performed as one part of a multi-modal analgesic regimen in the management of upper extremity radicular pain. Spinal epidural hematoma (SEH) is a rare complication with a reported incidence ranging from 1.38 in 10,000 to 1 in 190,000 epidurals. Current American Society of Regional Anesthesia (ASRA), American Society of Interventional Pain Physicians (ASIPP), and the International Spine Intervention Society (ISIS) recommendations are that non-steroidal anti-inflammatory drugs (NSAIDs) do not need to be withheld prior to epidu

Variation in Contrast Distribution on Epidurogram Following Interlaminar Epidural Injections in Two Patients With Plica Mediana Dorsalis: A Case Report.

BACKGROUND: Unilateral spread of local anesthetics has been reported in approximately 21% of epidural blocks. Plica mediana dorsalis (PMD) is a connective tissue structure that divides the epidural space along the dorsal midline. While unilateral blocks are the most common complication observed in these cases, there have been a few instances involving PMD where a bilateral distribution of the contrast and injectate was visualized under fluoroscopy. CASE REPORT: We present 2 cases involving PMD, each showing different contrast distributions upon epidurography. CONCLUSIONS: PMD is a potential

Patient Outcomes Following Parasagittal Interlaminar Epidural Steroid Injections for Bilateral Lumbar Radicular Symptoms: Correlation of Contrast Spread With Symptom Relief.

Introduction Bilateral lumbar radicular symptoms are commonly treated with interlaminar epidural steroid injections (ILESIs). The parasagittal approach often results in unilateral contrast spread, which may influence the degree of bilateral symptom relief. This study evaluates whether unilateral contrast spread correlates with symptom improvement in both ipsilateral and contralateral symptoms. Methods A retrospective review of six patients with bilateral lumbar radiculopathy secondary to lumbar degenerative disc disease, spondylosis, or disc herniation was conducted. All patients underwent ILE

Ultrasound-Guided Versus Conventional Radioscopic-Guided Transforaminal Epidural Steroid Injections for Cervical Radicular Pain: A Systematic Review and Meta-analysis.

BACKGROUND: Ultrasound-guided transforaminal (USF) injections have been proposed as a faster and more easily accessible alternative to traditional radioscopic methods for cervical radicular pain, but their efficacy and safety in cervical spine interventions remain uncertain. METHODS: Pubmed, Embase, and Cochrane Library were searched for studies comparing ultrasound (US)-guided cervical transforaminal injections versus traditional radioscopic methods of epidural injection for patients 1104. We computed standardized mean differences (SMD) for continuous pain outcomes, mean differences (MD) for

Comparison of the Oblique Interlaminar and Transforaminal Lumbar Epidural Steroid Injections for Treatment of Low Back and Lumbosacral Radicular Pain.

BACKGROUND: Transforaminal epidural steroid injections (TF-ESIs) effectively deliver small amounts of drugs to inflamed sites via the ventral epidural space. However, there is a high risk of nerve damage as the needle narrowly approaches the spinal nerve. Therefore, we devised an oblique interlaminar (OIL) approach as an alternative method. We compared the efficacy of fluoroscopic-guided OIL-ESIs with that of TF-ESIs in the management of lower back and unilateral lumbosacral radicular pain. MATERIALS AND METHODS: Sixty-six patients were randomized to receive a fluoroscopic-guided ESI either t

Comparison of Clinical Efficacy Between Transforaminal and Interlaminar Epidural Injections in Lumbosacral Disc Herniation: A Systematic Review and Meta-Analysis.

BACKGROUND: Epidural injection (EI) is used to treat back or radicular pain from lumbosacral disc herniation (LDH). Although several reports have stated that the transforaminal approach in EI (TFEI) has an advantage in target specificity and yields better clinical efficacy than the interlaminar approach in EI (ILEI), other studies have indicated that the clinical efficacy of ILEI was not inferior to that of TFEI and that ILEI also has the ability to spread medication into the ventral space to a degree similar to that of TFEI. There has been controversy about whether TFEI is superior to ILEI in
