The impact of major congenital malformations on mortality in a neonatal intensive care unit.

Neonatal mortality due to congenital malformations or genetic disorders has not decreased despite a decrease in overall neonatal deaths with recent advances in medical technology. As a consequence, an increasing percentage of neonatal deaths is attributable to congenital malformations and genetic disorders. This study retrospectively reviewed neonatal deaths associated with congenital malformations over an 11-year period in the neonatal intensive care unit (NICU) at Kosair Children's Hospital, Louisville, Kentucky. Presently, congenital malformations are responsible for approximately 45% (rang

What is stopping us? An implementation science study of kangaroo care in British Columbia's neonatal intensive care units.

BACKGROUND: The goal of the Neonatal Intensive Care Unit (NICU) is to provide optimal care for preterm and sick infants while supporting their growth and development. The NICU environment can be stressful for preterm infants and often cannot adequately support their neurodevelopmental needs. Kangaroo Care (KC) is an evidence-based developmental care strategy that has been shown to be associated with improved short and long term neurodevelopmental outcomes for preterm infants. Despite evidence for best practice, uptake of the practice of KC in resource supported settings remains low. The aim of

A study on blood culture positivity and C-reactive protein variability in neonatal septicaemia at neonatal intensive care unit of a tertiary care hospital.

Neonatal septicaemia is an important cause of neonatal morbidity and mortality. Blood culture is the gold standard for diagnosis of neonatal septicaemia. Several laboratory investigations are available to detect neonatal sepsis, one important is 'sepsis screen' which includes C-reactive protein (CRP), micro -ESR, total WBC count including immature to mature (VT) ratio and absolute neutrophil count. Blood culture was done among 210 samples from neonatal intensive care unit (NICU), RG Kar Medical College with suspected septicaemia along with CRP estimation. Among all the parameters, clinical cor

Prevalence and associated factors of neonatal hypothermia in the neonatal intensive care unit at St. Peter's specialized Hospital, Ethiopia.

Neonatal hypothermia, defined as a body temperature below 36.5 °C, is a global issue that significantly increases neonatal morbidity and mortality, particularly in low- and middle-income nations. Despite global guidelines, the prevalence remains high in many resource-limited settings. This study aimed to assess the prevalence and associated factors of hypothermia among neonates admitted to the intensive care unit of St. Peter's Specialized Hospital. An institution-based cross-sectional study was conducted from July to September 2024 among 222 neonates admitted to the intensive care unit of St

Bacteriological profile and antibiotic susceptibility of neonatal sepsis in neonatal intensive care unit of a tertiary hospital in Nepal.

BACKGROUND: Neonatal sepsis, one of the leading causes of mortality in neonatal intensive care units (NICU) of developing countries like Nepal, is often not extensively studied. In order to decrease the morbidity and mortality associated with neonatal sepsis, neonatologists should have a keen knowledge of the existing bacteriological flora and their antibiotic susceptibility pattern. In this study, we aim to determine the bacteriological profile and antibiotic susceptibility pattern of culture positive neonatal sepsis in the NICU of a tertiary teaching hospital in Nepal. METHODS: This was a r

Evaluation of vancomycin Bayesian dose-optimizing software in a neonatal intensive care unit population.

PURPOSE: Infectious Diseases Society of America 2020 guidelines recommend Bayesian-guided area under the curve (AUC) monitoring for vancomycin dosing in pediatric patients. Our institution's neonatal intensive care unit (NICU) utilizes single-level kinetics to monitor AUC using 2 pharmacokinetic models validated in neonates and infants. Anecdotally, preterm infants corrected to a higher postmenstrual age (PMA) may be better suited to the pediatric dosing model (PDM) versus the neonatal dosing model (NDM). This study aims to identify which model is best suited to the NICU population based on fa

Rapid genome sequencing in neonatal and pediatric intensive care units. Evidence and current situation.

Rapid genome sequencing has been found to be an effective tool for the diagnosis of genetic disorders in neonatal and pediatric intensive care settings, allowing rapid and accurate decision-making and access to personalized care and therapies. Most genetic disorders exhibit significant clinical and genetic heterogeneity, which complicates recognition of the disease and diagnosis by conventional methods. Rapid genome sequencing offers a superior diagnostic yield, improving patient management and reducing diagnostic delay and inpatient care costs. However, there are barriers to its implementatio

Development and implementation of restrictive platelet transfusion thresholds in a neonatal intensive care unit.

OBJECTIVE: Thrombocytopenia is a common problem affecting preterm neonates. Recent studies show increased morbidity and mortality with liberal platelet transfusion thresholds. We sought to standardize thrombocytopenia management through a transfusion guideline to reduce excessive transfusions. STUDY DESIGN: We developed and implemented a guideline using PDSA cycles for infants with birth weights <1000 grams. Platelet transfusions were classified as indicated or non-indicated per the guideline. Severe (grade 3 or 4) intraventricular hemorrhage and pulmonary hemorrhage rates were balancing meas
