A 73-yr-old white woman admitted with lobar pneumonia and congestive heart failure developed progressive colonic pseudoobstruction (Ogilvie's syndrome) 2 days after admission which was unrelieved by diatrizoate meglumine (Gastrografin, Squibb Canada, Montreal) enema and rectal tube. Cisapride, a new gastrointestinal prokinetic agent, was administered intravenously with full resolution of the syndrome. To the authors' knowledge, this is the first reported case of successful treatment of acute colonic pseudoobstruction with cisapride.

In a 12-month prospective study incorporating four neighbouring district general hospitals, 228 patients required a total of 236 admissions with intestinal obstruction. The aetiological factors included adhesions 75 (32 per cent), malignant disease 61 (26 per cent), strangulated hernias 59 (25 per cent), volvulus 10 (4 per cent), acquired megacolon 6 (3 per cent), pseudo-obstruction 4 (2 per cent), faecal impaction 6 (3 per cent) and miscellaneous 15 (6 per cent). The peak incidence for obstruction due to adhesions, malignant disease and strangulated hernias each occurred in the eighth decade.

In this case, we present an 84-year-old patient admitted for small bowel obstruction (SBO) with radiographic evidence of a clear transition point in the proximal small bowel on a computed tomography (CT) scan. Given his prior history of hemicolectomy, surgical adhesions were suspected as the likely cause. Malrotation is a rare aetiological factor of intestinal occlusion in adult. We report our experience of 16 cases collected from 1976 to 1991. The incidence is about 0.2-0.5% with two peaks, at 25 and 55 years respectively. Mean age is of 41 years. The symptoms were occlusion in more than 90%. A contrast enema was performed in acute as well as in chronic situations. The surgical treatment whatever it is, is the treatment of choice but has to be put in balance with age.

In some instances, particularly in the neonate, paralytic ileus affects predominantly the small bowel, thus resembling intestinal obstruction. Localized paralysis of the small bowel occurs secondary to regional events in a segment of gut and resembles mechanical obstruction even more closely. In localized paralysis the bowel lumen is usually dilated without abrupt distal transition to collapsed intestines. If there is radiographic and clinical recognition of localized, transitory bowel paralysis, surgery is usually unnecessary.

Numerous conditions predispose to Ogilvie's syndrome, a process that mimics bowel obstruction. Characteristic radiographic findings of acute pseudo-obstruction include dilatation of the cecum and the remainder of the colon but not the small intestine. Treatment is individualized. Massive cecal dilatation carries the risk of cecal perforation and its attendant high mortality rate. Conservative therapies, such as nasogastric suction, gentle enemas, and use of a rectal tube, may be beneficial. Decompression by colonoscopy or other means may be necessary.

Diverticulitis of the vermiform appendix is an often disregarded disease. Diagnosis follows after appendectomy due to inflammatory complications in the form of diverticulitis and appendicitis. Diverticula can also be found as a reason for complaints in histologically unaltered appendix or as an incidental finding. Diverticula of the vermiform appendix are classified as false (acquired) or true (congenital). We report on the case of a 57-year-old man with a perforated appendix diverticulum as reason for peritonitis and a paralytic ileus of the colon and small intestine.
