Many types of skin tests have evolved from Blackley's early scratch tests. This review highlights both the similarities between current skin tests and their differences. All current skin tests are capable of detecting allergic hypersensitivity, but the tests differ in their sensitivity, specificity, safety, reproducibility, and applications. A 43-year-old woman with a history of lidocaine allergy required dermatologic surgery during pregnancy. To accomplish this without the use of general anesthesia, intradermal allergy testing was performed. BACKGROUND: Intradermal allergen testing (IDAT) is commonly used to formulate allergen-specific immunotherapy, a pillar treatment for canine atopic dermatitis. Many sedatives have shown histaminergic or anti-histaminergic effects and thus been deemed unsuitable for IDAT.
OBJECTIVE: The goal of this study was to determine whether, in healthy dogs, dexmedetomidine (Dexdomitor) or a 1:20 combination of medetomidine and vatinoxan (Zenalpha) will affect intradermal reactions compared to unsedated dogs.

We report the case of a patient with anaphylaxis after ingestion of millet seeds, a previously unreported allergen. Skin testing with freshly prepared millet seed extract produced positive intradermal responses at a concentration of 100 pg/ml. The reaction was mediated by IgE antibodies to millet, as demonstrated by elevated specific IgE antibody levels by radioallergosorbent test (RAST) and by significant in vitro histamine release from leukocytes incubated with millet seed extract.

A 48-year-old man was evaluated for anaphylaxis associated with ingestion of beignets prepared from a commercial mix. Microscopic examination of the patient's beignet mix revealed live Dermatophagoides farinae. Another unopened box from the same source was not infested. Two patients who suffered from allergic rhinitis, conjunctivitis and contact urticaria caused by the two-spotted spider mite (Tetranychus urticae, Koch) are described.
