Persistent pupillary membrane and accessory iris membrane in cataract surgery.

Incomplete involution of the anterior tunica vasculosa lentis is observed in most newborns, including persistent pupillary membrane (PPM) and accessory iris membrane (AIM). PPM is a common congenital ocular anomaly characterized by delicate iris strands along the pupil. AIM is a rare congenital ocular anomaly characterized by iris tissue strands arising from the iris collarette, forming a pseudopupil. Although most of these membranes do not lead to visual impairment, they can pose challenges during cataract surgery. Our case highlights a unique scenario involving the concurrent presence of PPM…

Iris Avulsion During Corneal Wound Hydration After Uneventful Cataract Extraction.

We report a case of spontaneous total iris avulsion that occurred during corneal wound hydration following an uneventful phacoemulsification procedure. An 86-year-old woman underwent cataract surgery on her right eye, during which a single-piece acrylic intraocular lens (Bausch and Lomb Akreos Adapt Advanced Optics, Bausch and Lomb Incorporated, Rochester, NY, USA) was implanted in the bag, and the viscoelastic material was removed. During corneal wound hydration, total iris avulsion was observed, with the iris prolapsing out of the anterior chamber through the main wound. Wound hydration was …

EPHA2 biallelic disruption causes syndromic complex microphthalmia with iris hypoplasia.

Disruption of any of the ocular development steps can result in ocular defects such as microphthalmia, coloboma and anterior segment dysgeneses including aniridia and cataract. All of these anomalies can be isolated or seen in association with each other. Except for aniridia (almost exclusively due to PAX6 mutations), most of these congenital ocular malformations are related to a wide genetic heterogeneity, as hundreds of genes are implied in ocular development. Here we describe a patient presenting with bilateral microphthalmia, congenital cataract, corneal dystrophy and iris hypoplasia, asso…

Reversal of Iris Heterochromia in Adult-Onset Acquired Horner Syndrome.

Iris heterochromia is typically seen in association with congenital Horner syndrome. A man in his 40s with congenital iris heterochromia, blue in the right and brown in the left, presented with left-sided Horner syndrome. This was associated with recent change in color of his brown left iris to blue similar to the right iris. This case demonstrates a unique case of adult-onset Horner syndrome with reversal of iris heterochromia.

Reper intraocular lens with artificial iris: implantation techniques and outcomes.

The authors describe the surgical techniques to implant a novel intraocular lens with artificial iris (Reper) in patients affected by traumatic aphakia and aniridia. Two surgical approaches are proposed: implantation by suture-loaded cartridge and "open sky" implantation combined with penetrating keratoplasty. The method of establishing the exact position of the scleral fixation points and the pre-assembly of the suture of Reper before its implantation are the main novel proposed surgical procedures. The rationale is to minimize surgical procedures in the anterior chamber and to prevent knot l…

Immune-enhanced effect of Iris polysaccharide is protective against leptospirosis.

Leptospirosis, caused by pathogenic Leptospira species, is an essential but neglected zoonosis. There are more than 300 serovars of pathogen … ction and cancer. In this study, we examined the protective effect of polysaccharides isolated from Iris against leptospirosis. To our knowledge, it is the first time to report Iris polysaccharides (IP) as an immunostimulant in treating infection. The results showed that IP treatment significantly increased the survival rate of hamsters challenged by a …

Sclerouvectomy plus intraoperative ophthalmic brachytherapy for iris-ciliary body melanomas.

BACKGROUND: Partial lamellar sclerouvectomy was designed more than 30 years ago by Jerry and Carol Shields, in order to remove melanomas that involved the ciliary body and/or the choroid, while leaving intact the outer portion of the sclera and the overlying sensory retina [1]. METHODS: We present two cases of iris-ciliary body melanoma with different size. The performed surgery was a partial lamellar sclerouvectomy plus iridectomy, complemented by intraoperative juxtalimbar ruthenium-106 brachytherapy. RESULTS: Both cases achieved anatomical success without developing chronic ocular hypoton…

Intraocular amyloidosis with multifocal iris and anterior chamber translucent spherules.

Ocular amylodosis, although a rare entity, is known to affect the conjunctiva, extraocular muscles, orbit, lacrimal gland, and skin around the eyes. Intraocular deposition of amyloid mainly confines to the vitreous and cornea. In this report, we describe two cases of intraocular amyloidosis presenting as multiple iris and anterior chamber cysts. Histopathological examination with special stain like Congo Red and Transmission Electron Microscopy confirmed the diagnosis of amyloidosis. Systemic investigations ruled out systemic association confirming the diagnosis of primary ocular amyloidosis.

Iris Crystals: A Rare Clinical Finding in Chronic Anterior Uveitis.

Iris crystals are a rare clinical entity seen in cases of chronic anterior uveitis (CAU). We report one such case with crystal deposition on iris as well as corneal endothelium and anterior lens capsule. No significant change in the crystals was seen over a period of 11 years despite multiple episodes of exacerbation and resolution. Anterior segment optical coherence tomography (AS-OCT) showed crystals as hyperreflective deposits over the corneal endothelium. On confocal microscopy, the crystals were seen as ill-defined hyperreflective lesions on endothelium between 525 and 604 μm. Reduction i…

IOL repositioning using iris sutures: a safe and effective technique.

This retrospective non-comparative consecutive case series study was conducted at Azienda Ospedaliera Universitaria Careggi, Florence, Italy and describes a useful intraocular lens (IOL) repositioning technique using iris sutures. In our study, 41 consecutive cases of posteriorly dislocated IOLs were surgically treated between January 2015 and May 2017. Six of the cases were post-traumatic luxations, and 20 patients had pseudoexfoliation syndrome. All the patients underwent pars plana vitrectomy and same IOL repositioning using iris sutures. The mean follow-up was 12.2mo. The mean preoperative…

Acetazolamide-Induced Bilateral Ciliochoroidal Effusion Syndrome in Plateau Iris Configuration.

PURPOSE: Our purpose is to describe a 60-year-old male, who has plateau iris configuration and developed bilateral ciliochoroidal effusion syndrome after ingestion of acetazolamide. OBSERVATIONS: Our case was a research participant in a multi-center clinical study (ClinicalTrials.gov NCT01677507). During the course of this study, he was treated with a single dose of acetazolamide (500 mg), and seven days later treated with latanoprost one drop daily at bedtime both eyes for seven days, and then was administered another dose of acetazolamide (500 mg). Several hours later he complained of blurr…

Bilateral acute iris transillumination (BAIT) initially misdiagnosed as acute iridocyclitis.

Bilateral acute iris transillumination (BAIT) is a relatively new clinical entity characterized by bilateral acute loss of iris pigment epithelium, iris transillumination, pigment dispersion in the anterior chamber, and sphincter paralysis. We report the case of a 30-year-old male who was initially diagnosed with acute iridocyclitis in a different clinic and treated with topical and systemic corticosteroids. He was referred to our clinic to seek another opinion because his symptoms did not improve. An ocular examination revealed bilateral pigment dispersion into the anterior chamber, diffuse i…

Treatment of peripheral pigment epithelium iris cyst with SLT laser.

PURPOSE: To report a case of a peripheral pigment epithelium iris cyst treated with selective laser trabeculoplasty laser. CASE REPORT: A 15-year-old girl applied to our clinic with the complaint of painless blurred vision in the right eye. Gonioscopy through a widely dilated pupil revealed a clear, oval, pale brown, semitransparent cyst that was located in the inferotemporal aspect of the iris. The cyst was touching the lens, and the lens was pushed superonasally by the cyst. The iris cyst was treated with selective laser trabeculoplasty laser. A small iris notch emerged at the 8-o'clock pos…

Pediatric intraoperative floppy iris syndrome associated with persistent pupillary membrane.

Intraoperative floppy iris syndrome (IFIS) occurring during cataract surgery in adults has been widely reported in association with tamsulosin and other α-1(a) adrenergic antagonists; however, only one case of pediatric IFIS has been previously reported and was associated with congenital cataract. We report a case of a 1-month old girl with IFIS associated with bilateral persistent pupillary membranes without cataracts and the use of preoperative topical phenylephrine and pilocarpine for IFIS prophylaxis.
