Assessing the reliability of MRI-CBCT image registration to visualize temporomandibular joints.

OBJECTIVES: To evaluate image quality of two methods of registering MRI and CBCT images of the temporomandibular joint (TMJ), particularly regarding TMJ articular disc-condyle relationship and osseous abnormality. METHODS: MR and CBCT images for 10 patients (20 TMJs) were obtained and co-registered using two methods (non-guided and marker guided) using Mirada XD software (Mirada Medical Ltd, Oxford, UK). Three radiologists independently and blindly evaluated three types of images (MRI, CBCT and registered MRI-CBCT) at two times (T1 and T2) on two criteria: (1) quality of MRI-CBCT registration

The Relationship Between Osteoarthritis of the Lumbar Facet Joints and Lumbosacropelvic Morphology.

STUDY DESIGN: Cross-sectional study. OBJECTIVE: To investigate the relation between lumbosacropelvic morphology and the presence and degree of facet joint degeneration. SUMMARY OF BACKGROUND DATA: Osteoarthritis of the facet joints is one of the most common degenerative changes in the spine. It is considered to be formed secondary to repetitive stress or trauma and spinal deformity with secondary overload. The cause(s) of facet joints osteoarthritis, however, have not been clearly identified. METHODS: Abdominal computed tomography (CT) images of 723 patients which were taken between the yea…

Pathological findings from synovium of early osteoarthritic knee joints with persistent hydrarthrosis.

AIM: Nonspecific chronic synovitis of the knee joint was reported by Pollard in 1962 and its pathogenesis is considered to be a physiological reaction to intra-articular disease. In this study, we evaluated the pathological findings of the synovium of early osteoarthritis (OA)-affected knee joints with hydrarthrosis in comparison to typical OA. METHODS: Synovial tissues were harvested from early OA knee joints with hydrarthrosis graded 0-2 according to the Kellgren and Lawrence classification and examined by histopathology. RESULTS: The synovial tissues showed proliferation of fibroblast-lik…

Inflammation: IL-36 has proinflammatory effects in skin but not in joints.

Given the known involvement of IL-36 in psoriasis it might be surprising that the latest mouse models show that inhibiting IL-36 signalling does not alter the course of inflammatory arthritis. Can we now add IL-36 to the list of inflammatory mediators that are not viable DMARD targets?

[Painful swollen joints with weakness: what's behind all this? Case 1/2013].

HISTORY AND ADMISSION FINDINGS: We report on a 58-year-old patient with weight loss, weakness and polyarthralgia. She reported of recurrent drops, painful joints and muscle weakness. INVESTIGATIONS: The patient had swollen joints, clubbing and cachexia. The radiogram of the hands and feet showed a hypertrophic oseoarthropathy. In further investigations we found a tumor in the upper left lung. The histology findings showed a adenocarcinoma of the lung. DIAGNOSIS, TREATMENT AND COURSE: The diagnosis of a secondary hypertrophic osteoarthropathy (Pierre Marie-Bamberger syndrome) due to a adenoca…

Three- to six-year results of metacarpophalangeal joints arthroplasty in psoriatic arthritis.

BACKGROUND: Patients with psoriatic arthritis often need the replacement of the hand joints owing to the high prevalence of hand joints involvement and their often severe destruction and/or function loss. The long-term results of silicone arthroplasty of metacarpophalangeal joints in psoriatic patients are almost unreported in the scientific literature. MATERIALS AND METHODS: We analyzed 3- to 6-year results of 14 metacarpophalangeal joint replacements with silicone implants that we performed in nine psoriatic patients. RESULTS: The metacarpophalangeal joints arthroplasty in all the patients…

Minocycline-induced periarticular black bones in inflamed joints which underwent arthroplastic reconstruction.

BACKGROUND: Minocycline-induced pigmentation of bone (black bone) is well described in tooth-bearing intra-oral bone, but is less known in p … ticular bones during the surgery. There was no pigmentation in the cartilage or soft tissues of the joints. The mean Japanese Orthopaedic Association (JOA) score or Japanese Society for Surgery of the Foot (JSSF) scale for rheumatoid arthritis foot and ankle joints at latest follow-up (case 1, 66; case 2, 87; case 3, 77; case 4, 77; case 5, 80) improved …

The fate of acutely inflamed joints with a negative synovial fluid culture.

PURPOSE: The purpose of this study was to evaluate the management and fate of acutely inflamed joints with a negative synovial fluid culture. METHODS: Between January and December 2009, all the patients who presented to our institution with an acutely inflamed joint and were subjected to microbiological assessment of their synovial fluid, were included in the study. Patients with a positive synovial fluid culture, a prosthetic joint replacement in situ and where an aspirate was obtained for a rheumatological diagnosis were excluded. This cohort was then divided into two groups depending on wh…

Classification and surgical treatment of symphalangism in interphalangeal joints of the hand.

BACKGROUND: Symphalangism is a rare congenital difference characterized by ankylosis of interphalangeal (IP) joints of the fingers and toes. In adults, there were several attempts to restore the stiff joints into mobile ones, but these treatment options resulted in poor outcomes and could not be applied to growing children. Here, we report our experiences on surgical treatment for children who had symphalangism of the hand. METHODS: We treated 36 joints in 17 children with symphalangism of the hand using dorsal capsulotomy and collateral ligament release. The diagnoses were based on history, …

Imaging joints for calcium pyrophosphate crystal deposition: a knock to the knees.

With advanced age, articular calcium pyrophosphate crystal deposition (CPPD) is common. Defining who has CPPD is of growing importance, given increases in longevity in many countries and the frequent association of chondrocalcinosis with osteoarthritis. Chondrocalcinosis detected by plain radiography serves as a major screening tool, but how many and which sites to screen have not been adequately defined in the past. The work of Abhishek and colleagues in the previous issue of Arthritis Research and Therapy sheds new light on the incomplete information from knee radiographs, and helps position

Incidence of medial plica in 3,889 knee joints in the Japanese population.

PURPOSE: This study aimed to document the incidence of medial plicae (plica synovialis mediopatellaris) in a large sample of the Japanese population and classify them in a type-dependent manner. METHODS: The incidence of medial plicae was investigated during arthroscopic surgery of 3,889 knee joints in 3,563 patients (mean age, 35 years); medial plicae were classified according to Sakakibara's classification (type A, a cord-like elevation in the synovial wall; type B, a shelf-like appearance; type C, a large shelf-like appearance covering the anterior surface of the medial femoral condyle; an…

Liposomes act as effective biolubricants for friction reduction in human synovial joints.

Phospholipids (PL) form the matrix of biological membranes and of the lipoprotein envelope monolayer, and are responsible for many of the unique physicochemical, biochemical, and biological properties of these supermolecular bioassemblies. It was suggested that phospholipids present in the synovial fluid (SF) and on the surface of articular cartilage have major involvement in the low friction of cartilage, which is essential for proper mobility of synovial joints. In pathologies, such as impaired biolubrication (leading to common joint disorders such as osteoarthritis), the level of phospholip…

A new technical contribution for ultrasound-guided injections of sacro-iliac joints.

INTRODUCTION: Sacroiliac joint (SIJ) represents a difficult location for local therapies, as intra-articular injections may be hard to execute, especially in particular conditions such as chronic inflammatory diseases. However, in selected patients, local therapies may be considered. Some recent studies demonstrated the feasibility of ultrasound (US)-guided injection of SIJ, but still a complete explanation and definition of the technique is needed. MATERIALS AND METHODS: Seven patients, four males and 3 females, affected by mono or bilateral sacroiliitis entered the study. Each patient recei

Range of movements of lower limb joints in cross-legged sitting posture.

Cross-legged sitting posture is integral to activities of daily living in Eastern and Asian cultures. This posture has not been studied or defined in any significant detail from an orthopedic view point. The movements of right lower limb joints were measured in cross-legged sitting posture in 44 volunteers from the Indian population with no prior history of problems related to the knee or hip joint. Flexion at the hip joint ranged from 82 degrees to 100 degrees , with a mean of 91 degrees . Abduction at the hip joint ranged from 19 degrees to 57 degrees , with a mean of 39 degrees . The extern…

[Magnetic resonance imaging of the joints: a revolution for the practicing rheumatologist].

In the last 15 years, new imaging techniques have changed the life of practicing rheumatologists in terms of both diagnostic approach and knowledge of disease mechanisms. Clinical symptoms, disease signs and the results of physical examination have been more closely related to their anatomical basis. In particular, magnetic resonance imaging allow diagnosis of disease in its early phase and its follow-up with a previously unknown sensitivity. Novel imaging studies have contributed to elucidate several pathogenetic mechanisms in musculoskeletal diseases, such as rheumatoid arthritis, psoriatic

Ultrasound of the small joints of the hands and feet: current status.

The aim of this article was to review the current status of ultrasound imaging of patients with rheumatological disorders of the hands and f … particularly magnetic resonance. The ability to carry out a rapid assessment of many widely spaced joints, coupled with clinical correlation, the ability to move and stress musculoskeletal structures and the use of ultrasound to guide therapy accurately are principal amongst these. The use of colour flow Doppler studies provides a measure of neovascular …

Degenerative and post-traumatic arthritis affecting the carpometacarpal joints of the fingers.

Symptomatic post-traumatic arthritis affecting the finger CMC joints is less common than might otherwise be expected based on the frequency of injury, especially to the fifth CMC joint. For the fifth CMC joint, the shallow concavoconvex articulation combined with the typical fracture location may provide a protective effect. Non-operative measures are typically successful, except in cases of missed fracture/dislocations and symptomatic joint instability. In these instances, re-construction emphasizes stability first, with an eye toward mobility for the ulnar column. It is common to detect a bo…

[Radicular syndrome and synovial cyst of the zygapophyseal joints: two case-reports].

INTRODUCTION: Multiradicular nerve root compression, and long lasting radicular syndrome occur very often. They happen particularly on degenerative spine. Huge synovial cyst of the zygapophyseal joints may account for it, expand in the epidural area and cause radicular syndrome. EXEGESIS: Two cases of huge synovial cysts spreading into the spinal channel are reported here. Diagnostic and therapeutic modalities are discussed. On degenerative spine, facet joints osteoarthritis may result in synovial cysts. Physical examination findings are radicular syndrome. CONCLUSION: Huge synovial cysts ma…
