[Treatment of isolated and multiple ligament injuries of the knee: anatomy, biomechanics, diagnosis, indications for repair, surgery].

Complex knee ligament injuries are characterized by simultaneous rupture of the anterior cruciate ligament (ACL) and/or the posterior crucia … gical repair include meniscus dislocation, entrapment of collateral ligament portions in the joint, knee dislocation with severe knee instability, and displaced bony avulsions. The anatomy of the knee ligaments must be carefully respected in surgical reconstruction. Acute repair of collateral ligament injuries is possible only in the first 2 weeks after trauma. Ac …

A Rare Case of Large Osteochondral Fracture of Patella.

Osteochondral fractures of the patella are relatively common pediatric knee injuries, often missed during the initial evaluation, and almost always associated with acute patella dislocations. We report the case of an adolescent patient with a very large osteochondral fracture of the patella involving almost the whole of the medial patellar facet and without concomitant dislocation of the patella. A 16-year-old adolescent presented to the emergency with pain and swelling in the left knee after sustaining a road traffic accident. On evaluation with an X-ray and a CT scan, a large osteochondral f

The effect of acetabular retroversion on ipsilateral injuries during traumatic hip dislocation.

BACKGROUND: Determine whether native acetabular anteversion angle increased the risk of ipsilateral limb injuries in patients with traumatic hip dislocations. METHODS: Retrospective clinical series completed at a large, tertiary health care system between February 2016-November 2021. Patients with a native traumatic hip dislocation requiring a closed reduction in the operating room or open reduction internal fixation (ORIF) of an associated fracture were included, identified using current provider terminology (CPT) codes 27,250 and 27,252. Standard acetabular version angles were measured on C

Calcaneocuboid joint dislocation: a case report.

Tarsal bone dislocation is a rare entity. It is usually undiagnosed in the emergency department. We present the case of a 44-year-old man who was diagnosed as having calcaneocuboid joint dislocation in the emergency department. The dislocation was reduced in the emergency department, and a below-the-knee cast was applied. Successful clinical and radiologic results were obtained during follow-up. In this case, unlike the previous reports in the literature, conservative management succeeded in the treatment of calcaneocuboid joint dislocation.

Staged bilateral mobile-bearing and fixed-bearing total knee arthroplasty in the same patients: a prospective comparison of a posterior-stabilized prosthesis.

Mobile-bearing total knee arthroplasty (TKA) has several theoretical advantages over fixed-bearing TKA. We conducted a prospective randomized trial to compare the results of mobile-bearing and fixed-bearing posterior-stabilized TKA in the same patients using the same femoral component design of a mobile-bearing prosthesis in one knee and a fixed-bearing prosthesis in the other knee in 25 patients with osteoarthritis. The mean follow-up was 40 months. No significant differences were found in the mobile-bearing and fixed-bearing knees in terms of clinical and radiographic results. No osteolysis,

Length change patterns and shape of a grafted tendon after anatomical medial patellofemoral ligament reconstruction differs from that in a healthy knee.

PURPOSE: Recurrent patellar dislocation is currently treated with anatomical reconstruction of the medial patellofemoral ligament (MPFL), and favourable postoperative outcomes have been reported. However, it is uncertain if healthy MPFL function is restored by anatomical reconstruction. The hypothesis in this study was that stabilization of the patella following MPFL reconstruction would be improved compared with that before surgery, but that function of the grafted tendon would differ from that of a healthy MPFL. The objective was to analyse the length change patterns of the MPFL before surge

Patellofemoral malalignment: a report of 68 cases treated by proximal and distal patellofemoral reconstruction.

A series of 68 knees were reviewed retrospectively at an average of 56 months following surgical treatment for patellar dislocation and subluxation. Fifty-three knees in 48 patients were assessed both subjectively by questionnaire and objectively by examination, and 15 knees were evaluated by questionnaire only. All cases were treated by a proximal and distal reconstruction as described by Trillat (1964). The surgery was performed by one surgeon (MC) and reviewed independently (DW). The results were excellent in 48 knees (71 per cent), good in 12 (17 per cent) and poor in 8 (12 per cent). In n

Acute Irreducible Rotational Patellar Dislocation with Patellar Tendon Rupture: A Case Report.

CASE: In this report, we present a case of acute patellar dislocation with both vertical and longitudinal rotation of the patella. As a result of rotation in the longitudinal axis, the articular surface of the patella faced outward, and the dorsal surface faced the femoral condyle. The patient underwent open reduction. One year after open reduction and soft-tissue repair, the patient had acceptable knee function and no knee pain. CONCLUSION: This case shows how similar cases may be treated with an open reduction to properly address the patellar dislocation.

Lateral patellar dislocation: mechanism of disease, radiographic presentation, and management.

Lateral patellar dislocation is a common injury occurring in young active adults. The mechanism is that of twisting injury to the knee on a planted foot with valgus stress. Several predisposing factors, including femoral trochlear dysplasia, patella alta, and lateralization of the tibial tuberosity, contribute to patellar instability and lateral patellar dislocation. Magnetic resonance (MR) imaging of the knee is the modality of choice to evaluate underlying bone contusion patterns, associated soft-tissue injuries, and additional complex ligamentous and osteochondral injuries, many of which ar

Predictors of Osteochondral Fractures Following Patellar Dislocation in the Pediatric Emergency Department.

BACKGROUND: Patellar dislocations are frequently seen in the emergency department (ED). Almost all children with a reduced patellar dislocation will have a knee radiography, while only 10% have a fracture identified on x-ray. OBJECTIVE: The primary purpose of the study was to identify factors predicting osteochondral fractures among children with patellar dislocation reduced in the ED. METHODS: This was a retrospective cohort study of all children aged between 1 and 18 years old with patellar dislocation who needed a reduction in a tertiary care pediatric ED between 2019 and 2024. The primar

Long-term results of arthroscopic medial reefing for patellar instability.

BACKGROUND: The purpose of the study was to investigate the long-term outcomes of the all-inside arthroscopic medial reefing (AAMR) procedure for patellar instability and the factors that affect successful outcome. METHODS: In this retrospective study, AAMR with suture was performed in 16 knees of 15 patients who had at least one patellar dislocation and did not experience a decrease in pain and who did not have a major radiological bony abnormality. Preoperatively, Tegner and Lysholm scales were used; for the final evaluation Tegner, Lysholm, Kujala and Knee injury and Osteoarthritis Outcome

Current Classification Systems for Knee Ligament Injuries Are Limited to Ligament Involvement and Do Not Consider Injury Mechanism or Sequence of Injury: A Scoping Review.

PURPOSE: To systematically assess published classification systems for knee ligament injuries and to propose a comprehensive, multiplanar, s … ence of injuries, involvement of bone marrow edema (except one study), and lack of correlation with knee dislocations. No clinical validation was performed. A knee classification system was created that included sequence of injuries, correlation with knee dislocation and injury mechanism, and bone marrow edema. CONCLUSIONS: Knee injury classification systems lack …

Medial patellofemoral ligament reconstruction fixed with a cylindrical bone plug and a grafted semitendinosus tendon at the original femoral site for recurrent patellar dislocation.

BACKGROUND: The medial patellofemoral ligament (MPFL) is the most important factor for stabilizing the patella and preventing lateral patellar dislocation. Medial patellofemoral ligament reconstruction is an accepted surgical technique to restore patellofemoral stability after lateral patellar dislocation. The authors recently developed a new anatomical MPFL reconstruction method using a cylindrical bone plug and grafted semitendinosus tendon at the anatomical femoral attachment site to mimic the native MPFL. This study evaluated the new technique for stabilizing recurrent patellar dislocation

Operative versus closed treatment of primary dislocation of the patella. Similar 2-year results in 125 randomized patients.

To assess whether initial surgery is beneficial for patients with primary dislocation of the patella, we carried out a prospective randomized study. Knee stability was examined under anesthesia, and associated injuries were excluded by diagnostic arthroscopy. 55 patients then had closed treatment and 70 patients were operated on with individually adjusted proximal realignment procedures. Surgery gave no benefit based on 2 years of follow-up. The subjective result was better in the non-operative group in respect of mean Houghston VAS knee score (closed 90, operative 87), but similar in terms of

Proximal tibiofibular dislocation.

Proximal tibiofibular joint dislocation is an uncommon injury, which may be easily missed on plain radiography. If recognised, it can be treated in the emergency department, avoiding surgery and long term problems. The case is presented of a 22 year old male rugby player who was tackled from the left hand side while turning to the left. He heard a "pop" from his knee as he fell to the ground. Clinical examination revealed a prominence in the area of the fibular head. There was no evidence of peroneal nerve injury. Plain x rays confirmed a clinical suspicion for anterior dislocation of the prox
