The effect of pulsed electromagnetic fields on osteoporosis at the knee in individuals with spinal cord injury.

The purpose of this study was to determine the effects of pulsed electromagnetic fields on osteoporotic bone at the knee in individuals with chronic spinal injury. The study consisted of 6 males with complete spinal cord injury at a minimum of 2 years duration. Bone mineral density (BMD) was obtained at both knees at initiation, 3 months, 6 months, and 12 months using dual energy X-ray absorptiometry. In each case, 1 knee was stimulated using The Bone Growth Stimulator Model 3005 from American Medical Electronics, Incorporated and the opposite knee served as the control. Stimulation ceased at

MR imaging appearance of the extensor mechanism of the knee: functional anatomy and injury patterns.

Trauma to the extensor mechanism of the knee, a common clinical problem, can be accurately evaluated with magnetic resonance (MR) imaging. The extensor mechanism consists of the quadriceps muscle and tendon, patella, patellar tendon, and patellar retinacula. Injuries of these structures can be classified into partial and complete tears. Acute injuries are associated with edema, hemorrhage, and fluid collections; chronic injuries often demonstrate redundancy, atrophy, and retraction of the affected structures. MR imaging is useful in differentiating partial and complete tears and in evaluating

Can muscle co-contraction protect knee ligaments after injury or repair?

A computer-based model of the knee was used to study forces in the cruciate ligaments induced by co-contraction of the extensor and flexor muscles, in the absence of external loads. Ligament forces are required whenever the components of the muscle forces parallel to the tibial plateau do not balance. When the extending effect of quadriceps exactly balances the flexing effect of hamstrings, the horizontal components of the two muscle forces also balance only at the critical flexion angle of 22 degrees. The calculations show that co-contraction of the quadriceps and hamstring muscles loads the

[A complex injury of the knee joint associated with a popliteal artery lesion--case report].

Injuries of the popliteal artery are frequent in the dislocations of the knee joint, and an early diagnosis of a lesion of the popliteal artery is the base for the successful treatment. Generally, in all the traumas in the region or proximity of the knee joint, it should always be taken into consideration a possibility of arterial injury. In our case there was a posterior knee joint luxation with evidently contused and thrombosed popliteal artery. The surgery was done in an inappropriate time (more than 14 hours after the injury), and consisted in a Goretex graft interposition, and following a

Comparing walking with knee-ankle-foot orthoses and a knee-powered exoskeleton after spinal cord injury: a randomized, crossover clinical trial.

Recovering the ability to stand and walk independently can have numerous health benefits for people with spinal cord injury (SCI). Wearable exoskeletons are being considered as a promising alternative to conventional knee-ankle-foot orthoses (KAFOs) for gait training and assisting functional mobility. However, comparisons between these two types of devices in terms of gait biomechanics and energetics have been limited. Through a randomized, crossover clinical trial, this study compared the use of a knee-powered lower limb exoskeleton (the ABLE Exoskeleton) against passive orthoses, which are t

The association between knee airbag deployment and knee-thigh-hip fracture injury risk in motor vehicle collisions: A matched cohort study.

INTRODUCTION: In the U.S. alone, an estimated 30,000 knee-thigh-hip (KTH) injuries occur annually in frontal motor vehicle collisions. These fractures typically occur through occupant contact with the vehicle's knee bolster. Research has suggested that knee airbags (KABs) can mitigate the forces sustained during this contact, resulting in decreased injury risk; however, previous research has been limited by small sample sizes or by occurring in a controlled setting. The objective of the current study is to determine the effectiveness of KABs on KTH fracture risk using nationally representative

Can magnetic resonance imaging predict posterior drawer laxity at the time of surgery in patients with knee dislocation or multiple-ligament knee injury?

BACKGROUND: Previous studies indicate that isolated posterior cruciate ligament injuries demonstrate magnetic resonance imaging (MRI) and clinical evidence of healing when treated nonoperatively; however, the authors are unaware of any other study that has looked at whether initial MRI can predict posterior cruciate ligament stability at the time of surgery in patients with knee dislocation. HYPOTHESIS: An MRI grading system will predict laxity on posterior drawer testing at the time of surgery in patients with knee dislocations. STUDY DESIGN: Cohort study (prognosis); Level of evidence, 2.

Knee kinematics and kinetics in former soccer players with a 16-year-old ACL injury--the effects of twelve weeks of knee-specific training.

BACKGROUND: Training of neuromuscular control has become increasingly important and plays a major role in rehabilitation of subjects with an injury to the anterior cruciate ligament (ACL). Little is known, however, of the influence of this training on knee stiffness during loading. Increased knee stiffness occurs as a loading strategy of ACL-injured subjects and is associated with increased joint contact forces. Increased or altered joint loads contribute to the development of osteoarthritis. The aim of the study was to determine if knee stiffness, defined by changes in knee kinetics and kinema

Decoding the Knee Injury and Osteoarthritis Outcome Score for Joint Replacement: A Critical Investigation of a Common Orthopaedic Outcome Following Total Knee Arthroplasty.

BACKGROUND: The Knee Injury and Osteoarthritis Outcome Score for Joint Replacement (KOOS JR) is a composite patient-reported outcome measure intended to represent overall knee health. Limited investigations exist to determine the validity of KOOS JR as a marker of joint function among total knee arthroplasty patients. The goal of this study was to evaluate the underlying construct of KOOS JR. METHODS: Data were collected from 156 patients (94 women; mean age 68 years [range, 39 to 83]) who were scheduled to undergo primary elective total knee arthroplasty. The KOOS JR and eight physical perfo

Knee wobbling during the single-leg-squat-and-hold test reflects dynamic knee instability in patients with anterior cruciate ligament injury.

We propose using the single-leg squat-and-hold (SLSH) task with kinematic analysis to objectively measure dynamic knee stability after anterior cruciate ligament (ACL) injury. There are three objectives of this study: to compare the knee kinematics of ACL-deficient patients and healthy controls by capturing knee wobbling during the SLSH task, to detect kinematic changes after ACL reconstruction, and to correlate the kinematic variables with self-reported knee function. Twenty-five ACL-deficient participants and 18 healthy matched participants were recruited. The knee kinematics involving both
