Animal experiments show that intoxication with hydrocarbon solvents influences the vestibulo-oculomotor reflex arc and indicate that the solvents block the inhibition of the vestibulo-oculomotor reflex presumably exerted by the cerebellum. The blood concentrations necessary to elicit oculomotor disturbances in animals were smaller than those that disturbed the vestibular function. Accordingly, in human experiments styrene and toluene exposure did not elicit any positional nystagmus but caused an increased saccade speed and a diminished visual suppression of vestibular nystagmus. Some cerebella

Pediatric neurotology is a subspecialty encompassing the medical and surgical treatment of conditions involving the middle ear, inner ear and lateral skull base. Multiple otologic and neurotologic diseases exist. In the pediatric patient the most common neurotologic conditions are vertigo, complications of otitis media, cholesteatoma, hearing loss and facial paralysis. Vertigo is differentiated into peripheral and central vestibular abnormalities. Peripheral vestibular causes include Meniere's disease, benign paroxysmal positional vertigo, vestibular neuronitis, otitis media and labyrinthitis.

3D analysis of eye movements during off vertical axis rotation (OVAR) was carried out in seven subjects with unilateral labyrinthine loss (ULL). The modulation component (MOC) of these patients was not different from that of normal subjects. However, the horizontal MOC was significantly smaller when the rotation was directed towards the diseased side as opposed to the healthy side. With rotation to the diseased side, most subjects exhibited a horizontal bias component (BIC) to the opposite side as compared with the diseased side in normal subjects. The vertical and torsional BICs were also inf

The effect of osmotic treatment on cases with fluctuating sensorineural hearing loss other than Menière's disease is described in this paper. Complete normalization of the hearing threshold has been obtained in eight patients suffering from cochlear hydrops (one case), sensorineural concussion deafness (five cases), temporary noise threshold shift (one case), sudden deafness (two cases). Relief of tinnitus has been observed in a case of post-stapedectomy deafness. The audiometric pattern of end organ disease in a case of cohlear otosclerosis showed a change into a classical otosclerosis audiog

Intralabyrinthine schwannomas are a rare subgroup of vestibular schwannomas located within the membranous labyrinth of the inner ear and are known for their variable clinical presentations and symptoms. In the present study, we report on a patient with a persistent history of dizziness and positional vertigo, who was misdiagnosed with posterior canalithiasis. As hearing loss was not developed until late in the disease course, the patient was not properly diagnosed until magnetic resonance imaging revealed an intralabyrinthine schwannoma, which was not discovered on earlier imaging. In addition

A procedure for the spectral analysis of nystagmograms obtained by rotary stimulation with the rate of 180 degrees/s for 20 s has been developed with the aim to reveal the most typical changes in the vestibular nystagmus with one or another form of the cochleovestibular pathology. A signal recorded in the analogous form was processed by computer EC-1022. Periodograms were calculated by the method of the fast Fourier transform and periodogram smoothing by averaging of realization sites. In case of Ménière's disease the third harmonic was not recorded on periodograms when right and left labyrint

Endolymphatic sac ablation in the guinea pig can result in a dilated membranous labyrinth and low-frequency hearing loss similar to patients with Meniere's disease. However, this animal model fails to develop analogous symptoms of a Meniere's crisis such as sudden hearing loss and marked vestibular imbalance. Possibly, factors in addition to endolymphatic hydrops must be present to result in a crisis episode. By placing animals in an inverted posture, their intralabyrinthine pressure was elevated, causing additional stress on the inner ears. Standard electronystagmographic techniques were used

A new procedure, the retrosigmoid internal auditory canal (IAC) vestibular neurectomy has been developed and presented. It involves a 3-cm retrosigmoid craniotomy removing the posterior wall of the IAC to the singular canal, with transection of the superior vestibular nerve and posterior ampullary nerve. This produces a complete denervation of the vestibular labyrinth and preserves the patient's hearing. All ten patients with Meniere's disease had their vertigo cured. Hearing was preserved to within 11 dB of the preoperative pure tone average in 9 of 10 cases. There were no serious complicatio

The temporal bones of a patient with unilateral Ménière's disease were studied. Two weeks before death, the patient had an ultrasonic treatment through the lateral semicircular canal. The only histopathological finding attributed to the ultrasound irradiation is the degeneration of the sensorineural epithelium and crista of the lateral and vertical anterior canal ampulla. The other vestibular sense organs are normal, giving rise to several hypotheses. Electron microscopy might have discovered some changes in the utricular and saccular maculae and in the posterior and superior ampulla. The pres

The effects of congenital cytomegalovirus infection vary from severe central nervous system destruction to asymptomatic viuria. Yet children with the latter have a high incidence of unsuspected hearing loss. Histopathology of temporal bones from children who die with cytomegalic inclusion disease shows viral infected cells in non-sensory endolabyrinthine epithelium. Experimental mouse cytomegalovirus causes an age related cranial neuronitis and perilabyrinthitis. Immunopathologic localization of different viral antigens in experimental inner ear infections shows differential vulnerability of l

Eosinophilic granuloma is a benign lytic lesion of bone, probably arising from an abnormal proliferation of histiocytes. It may occur within the temporal bone and be confused with more common disorders such as aural polyps or chronic suppurative otitis media. If untreated, the disease may destroy the bony labyrinth and spread to the middle and posterior cranial fossas. Temporal bone lesions may represent only one manifestation of a more extensive multifocal disorder (i.e., Hand-Schüller-Christian or Letterer-Siwe disease). From 1967 to 1982, 25 patients with eosinophilic granuloma of the skull

OBJECTIVE: The aim of this study was to determine the incidence of inner ear involvement in various forms of acute otitis media (AOM) in ear, nose, and throat (ENT) practices in Germany.
METHODS: Patients who had been diagnosed with various forms of AOM in the years 2010 to 2017 were enrolled in the study from a nationwide, representative practice database (Disease Analyzer, IQVIA). In these patients, the incidence of simultaneous or subsequent inner ear disorders (IED) was determined within 7 days and within 12 months from the date of an AOM diagnosis.
RESULTS: A total of 286 186 patients w

There have been several reports of perisaccular fibrosis of the endolymphatic sac in some autopsied cases of Meniere's disease. We attempted to initiate perisaccular fibrosis of the sac in guinea pigs by injecting a small amount of 10% silver nitrate solution that cauterizes the tissue readily to produce scar, ie, fibrosis. Ninety-nine guinea pigs out of 108 (92%) that were treated with silver nitrate showed the endolymphatic hydrops in varied extent, from slight to profound. The extent of hydrops might depend on the extent of destruction of the sac following the cauterization. The lumen of th

A third inner ear disorder similar to Menière's disease and fluctuant hearing loss seems to be due to endolymphatic hydrops. This disorder is characterised by attacks of vertigo without any cochlear symptoms. During the course of the disease, which lasts up to 8 years, the vestibular organ loses its function in most cases. Clinical and electronystagmographic findings are presented, and the possible pathophysiological mechanisms are discussed.

Ever since the introduction of temporal bone imaging by means of high-resolution CT, it appears that the combination of high spatial resolution, high density resolution and the freedom of patient positioning for scanning of optimal otological planes may play a unique role in the diagnosis and follow-up of a number of otological disorders. Two examples are described here. The first is the possibility of determining whether the vestibular aqueduct in idiopathic Ménière's disease is obliterated or not, and if so, whether it is a bony or a fibrous obliteration. Although the results are preliminary

Endolymphatic hydrops (EH) is a pathological condition which is the final common manifestation of a variety of otologic insults. In this paper we develop a classification which, on the basis of clinical and pathological data, distinguishes symptomatic and asymptomatic forms. Clinical case histories and temporal bone studies are presented to illustrate and substantiate this classification. The symptomatic form becomes evident by the hallmark symptoms of episodic vertigo and fluctuating hearing loss. The asymptomatic form is silent. Interconversion from one form to another may occur over time. E
