Non-literal language deficits in mild cognitive impairment.

BACKGROUND: Verbal language deteriorates in Alzheimer's disease, contributing to dramatic disturbances in the ability to communicate. The presence of language disturbances may be detected at earlier phases of the neurodegenerative process, like mild cognitive impairment (MCI). In daily verbal interactions, people mostly use literal language, but sometimes they employ non-literal language, which requires listeners to interpret messages beyond the plain meaning of the words and can be quite demanding. In the present study, we tested the hypotheses that patients with MCI may have deficits in non-…

Speech and language outcomes of very preterm infants.

Speech and language impairments of both simple and complex language functions are common among former preterm infants. Risk factors include lower gestational age and increasing illness severity including severe brain injury. Even in the absence of brain injury, however, altered brain maturation and vulnerability imposed by premature entrance to the extrauterine environment is associated with brain structural and microstructural changes. These alterations are associated with language impairments with lasting effects in childhood and adolescence and increased needs for speech therapy and educati…

Diffuse cerebral language representation in tuberous sclerosis complex.

INTRODUCTION: Tuberous sclerosis complex (TSC) is a multisystem genetic disorder affecting multiple organs, including the brain, and very often associated with epileptic activity. Language acquisition and development seems to be altered in a significant proportion of patients with TSC. In the present study, we used magnetoencephalography (MEG) to investigate spatiotemporal cerebral language processing in subjects with TSC and epilepsy during a reading semantic decision task, compared to healthy control participants. METHODS: Fifteen patients with TSC and 31 healthy subjects performed a lexico…

Naming abilities in low-proficiency second language learners.

Difficulties in second language (L2) learning are often associated with recognizable learning difficulties in native language (L1), such as in dyslexia. However, some individuals have low L2 proficiency but intact L1 reading skills. These L2 learners experience frequent tip-of-the-tongue states while naming in L1, which indicates that they have a weakness in retrieval of phonological codes of words. The authors hypothesized that if naming ability is shared across languages, this difficulty would reemerge in L2 naming, which was tested using the tip-of-the-tongue experimental paradigm. Consiste…

White matter integrity, language, and childhood onset schizophrenia.

BACKGROUND: The heterogeneity of symptoms and cognitive deficits in schizophrenia can be explained by abnormal connectivity between brain regions. Childhood-onset schizophrenia (COS) is a particularly severe form of schizophrenia, with an onset during a key time period for both cerebral pruning and myelination. METHODS: Diffusion tensor images were acquired from 18 children and adolescents with COS and 25 controls. The COS group was divided into two sub-groups-one with linguistic impairment (LI) and the other without (NLI). The fractional anisotropy (FA), axial (AD), and radial diffusivity (R

Teacher knowledge of basic language concepts and dyslexia.

Roughly one-fifth of the US population displays one or more symptoms of dyslexia: a specific learning disability that affects an individual's ability to process written language. Consequently, elementary school teachers are teaching students who struggle with inaccurate or slow reading, poor spelling, poor writing, and other language processing difficulties. Findings from studies have indicated that teachers lack essential knowledge needed to teach struggling readers, particularly children with dyslexia. However, few studies have sought to assess teachers' knowledge and perceptions about dysle…

Impaired comprehension of nonliteral language in Tourette syndrome.

OBJECTIVE: To investigate theory of mind and the understanding of nonliteral language in patients with Tourette syndrome (TS). BACKGROUND: In TS, striatal dysfunction could affect the functioning of the frontal cortex. Changes in frontal functioning could lead to impairments in theory of mind: the understanding of mental states, such as beliefs, emotions, and intentions. Poor understanding of a speaker's mental state may also impair interpretation of their nonliteral remarks. METHOD: In this study, patients with TS and healthy controls completed tasks to assess their understanding of sarcasm…

Speech and language therapy for aphasia following stroke.

BACKGROUND: Aphasia is an acquired language impairment following brain damage which affects some or all language modalities: expression and understanding of speech, reading and writing. Approximately one-third of people who have a stroke experience aphasia. OBJECTIVES: To assess the effectiveness of speech and language therapy (SLT) for aphasia following stroke. SEARCH STRATEGY: We searched the Cochrane Stroke Group Trials Register (last searched April 2009), MEDLINE (1966 to April 2009) and CINAHL (1982 to April 2009). In an effort to identify further published, unpublished and ongoing tria…

Developmental disorders of language and literacy: special issue.

Language and literacy are cognitive skills of exceptional complexity. It is therefore not surprising that they are at risk of impairment either during development or as a result of damage (e.g. stroke) later in life. Impaired language and literacy can arise from a general learning impairment. However, two developmental disorders, specific language impairment (SLI) and dyslexia, which affect oral and written language, respectively, are 'specific' in that they are not part of a more general learning difficulty. SLI and dyslexia each affect 5-10% of the general population, and they are the focus …

Delayed language development due to infantile thiamine deficiency.

The aim of this study was to investigate the language development of 20 children who had been exposed to thiamine (vitamin B(1)) deficiency in infancy due to feeding with soy-based formula that was accidentally deficient of thiamine. In this case-control study, 20 children (12 males, eight females; mean age 31.8mo [SD 4.1], range 24-39mo) who were fed thiamine-deficient formula in infancy were compared with 20 children (12 males, eight females; mean age 32.2mo [SD 3.9], range 25-39mo) fed with other milk sources and matched for age, sex, and maternal education. Receptive and expressive languag…

Unravelling neurogenetic networks implicated in developmental language disorders.

Childhood syndromes disturbing language development are common and display high degrees of heritability. In most cases, the underlying genetic architecture is likely to be complex, involving multiple chromosomal loci and substantial heterogeneity, which makes it difficult to track down the crucial genomic risk factors. Investigation of rare Mendelian phenotypes offers a complementary route for unravelling key neurogenetic pathways. The value of this approach is illustrated by the discovery that heterozygous FOXP2 (where FOX is forkhead box) mutations cause an unusual monogenic disorder, charac…

Overview of telehealth activities in speech-language pathology.

It is estimated that 10% of the world's population, approximately 650 million people, have some form of disability. Population growth, aging, and medical advances that preserve and prolong life have increased demands for health and rehabilitation services. Recent predictions indicate a shortage of speech-language pathologists and other rehabilitation specialists to provide care for individuals with disabilities. The application of telemedicine and telehealth technologies offers effective solutions to this challenge. An extensive literature review was conducted that included technical reports, …

Is expressive language disorder an accurate diagnostic category?

PURPOSE: To propose that the diagnostic category of "expressive language disorder" as distinct from a disorder of both expressive and receptive language might not be accurate. METHOD: Evidence that casts doubt on a pure form of this disorder is reviewed from several sources, including the literature on genetic findings, theories of language impairments, and the outcomes of late talkers with expressive language delays. Areas of language that are problematic in production but not readily amenable to comprehension testing are also discussed. CONCLUSIONS: The notion of expressive language disord…

Language disorders: autism and other pervasive developmental disorders.

This article summarizes current knowledge about language and communication impairments in children who have autism spectrum disorders. It reviews the language profiles that may be observed during the toddler and preschool years and in school-aged children and discusses receptive and expressive language skills that may be quite variable across the spectrum and the universal impairments in pragmatic aspects of language that are among the defining characteristics of the disorder. It concludes with clinical recommendations for pediatric screening of autism spectrum disorders and for continued moni…

Mainstreaming Sure Start speech and language therapy services.

Mainstreaming is the process of transferring policy, activity or good practice from a community-based initiative, such as Sure Start, into the core service. This paper will explain how one Sure Start programme developed a strategy for speech and language therapy (S&LT) that had an impact on the development of mainstream S&LT provision. The strategy explained here has shown the potential for team working and multi-agency delivery of new services. The new service was based on quality standard setting (as highlighted in government policy); positive multi-agency working based on sharing and skill …

Are specific language impairment and dyslexia distinct disorders?

PURPOSE: The purpose of this study was to determine whether specific language impairment (SLI) and dyslexia are distinct developmental disorders. METHOD: Study 1 investigated the overlap between SLI identified in kindergarten and dyslexia identified in 2nd, 4th, or 8th grades in a representative sample of 527 children. Study 2 examined phonological processing in a subsample of participants, including 21 children with dyslexia only, 43 children with SLI only, 18 children with SLI and dyslexia, and 165 children with typical language/reading development. Measures of phonological awareness and no…

Can language restructure cognition? The case for space.

Frames of reference are coordinate systems used to compute and specify the location of objects with respect to other objects. These have long been thought of as innate concepts, built into our neurocognition. However, recent work shows that the use of such frames in language, cognition and gesture varies cross-culturally, and that children can acquire different systems with comparable ease. We argue that language can play a significant role in structuring, or restructuring, a domain as fundamental as spatial cognition. This suggests we need to rethink the relation between the neurocognitive un…
