Evaluating the Performance of Large Language Models on Palliative Care Test Questions: A Mixed Methods Study.

BACKGROUND: Little is known about large language model (LLM) performance on palliative care (PC)-related knowledge-based tasks. We evaluated two LLMs in answering PC-related test questions and explaining their answer choice rationale. METHODS: LLMs were prompted to answer 25 randomly selected questions from the Fast Facts Quiz and provide their answer choice rationale. Three PC educators ranked and rated LLM-generated answer choice explanations versus the test's answer key explanations. Linear fixed-effect models evaluated reviewer ranking, and ordinal logistic regression evaluated reviewer r

Harnessing large language models in virtual CBT for university students' academic anxiety: a preliminary randomized trial.

Academic anxiety is a common mental health problem among university students. It is often associated with reduced learning efficiency and an increased risk of depression. Cognitive behavioral therapy (CBT) is supported by an established evidence base, but its use in university settings is still limited by factors such as therapist availability, time, and physical space. To explore a new approach to digitally assisted intervention, this study integrated virtual reality (VR), large language model (LLM), and retrieval-augmented generation (RAG) technologies. We developed an LLM-VR-CBT system base

Quality-aware multi-source data fusion and enhancement for Medical Concept Normalization using Large Language Models.

OBJECTIVE: Medical Concept Normalization (MCN) maps informal health phrases to formal clinical concepts. It is a critical task for pharmacovigilance, patient record analysis, and health-related text mining. Extensive MCN research has mainly relied on single-source datasets and overlooked data quality (DQ) issues. This study aims to develop a quality-aware data fusion framework for MCN using Large Language Models (LLMs). METHODS: The methods consist of DQ evaluation and enhancement, LLM-based data augmentation, and multi-source MCN dataset fusion using enriched concept-phrase pairs. We analyze…

Machine learning outperforms large language models for survival prediction in advanced hepatocellular carcinoma: a multicenter study.

Accurate prognostic prediction remains a critical unmet need in advanced hepatocellular carcinoma (HCC). While machine learning (ML) models have demonstrated value in outcome prediction, the ability of large language models (LLMs) to perform structured clinical prognostic tasks remains unclear. In this multicenter retrospective study, 1031 patients with HCC receiving interventional therapy combined with targeted treatment were analyzed and randomly divided into training (n = 717) and test (n = 314) cohorts. Six ML algorithms were developed and compared with two LLMs (ChatGPT-4o and DeepSeek-v3…

Few-Shot Lung Cancer Classification via Electronic Nose Using Large Language Models: A Multicentre Prospective Study.

BACKGROUND AND OBJECTIVE: Electronic Nose (eNose) breathprints are promising non-invasive lung cancer diagnostic tools, but cross-site validation and adaptation remain barriers to clinical applications. It remains unknown whether a natural language processing-pretrained large language model (LLM) can enable few-shot, site-specific classification of lung cancer using eNose breathprints. METHODS: We collected eNose breathprints of lung cancer and non-lung cancer patients from two medical centres in Taiwan. A GPT-2-backbone LLM with parameter-efficient adaptation was compared with convolutional

Evaluation of causal reasoning for large language models in contextualized clinical scenarios of laboratory test interpretation.

This study evaluates causal reasoning in large language models (LLMs) using 99 clinically grounded laboratory test scenarios mapped to Pearl's Ladder of Causation: association, intervention, and counterfactual reasoning. We focused on common lab tests such as Hemoglobin A1c (HbA1c), creatinine, and vitamin D, and paired them with clinically relevant causal factors, including age, gender, obesity, and smoking. Two LLMs, GPT-o1 and Llama-3.2-8b-instruct, were tested, with responses rated by four medically trained human experts. GPT-o1 demonstrated superior discriminative performance (AUROC overa…

Limitations of Large Language Models in Assisting PI-RADS Scoring on Prostate Biparametric MRI Text Reports.

BACKGROUND: Prostate cancer (PCa) is a significant global health challenge, and the prostate imaging reporting and data system (PI-RADS) is crucial for risk stratification using MRI. However, inter-reader variability, especially in the transition zone and among practitioners with differing experience levels, compromises diagnostic consistency. Large language models (LLMs) show potential in medical image analysis, particularly in standardizing reports to improve diagnostic consistency and efficiency. OBJECTIVE: To evaluate the performance of LLMs in assisting PI-RADS scoring based on biparamet…

Benchmarking large language models in breast cancer care: agreement with radiology-led multidisciplinary tumor board decisions.

BACKGROUND: Multidisciplinary tumor boards (MDTBs) play a central role in breast cancer management by integrating imaging findings with clinical and pathological information to guide treatment decisions. The increasing integration of artificial intelligence into clinical workflows has raised interest in the potential role of large language models (LLMs) as supportive tools in oncologic decision-making. The aim of this study was to evaluate the concordance between treatment recommendations generated by LLMs and decisions made by a radiology-led MDTB in newly diagnosed breast cancer patients, an…

Utility of large language models in pediatric trauma triage: An age-stratified analysis of prompt engineering.

BACKGROUND: Pediatric trauma triage is complicated by age-dependent physiologic variability and low case volumes, contributing to persistent undertriage and overtriage despite standardized guidelines. Large language models (LLMs) may offer potential decision-support regardless of pediatric age group. METHODS: We performed a retrospective analysis of 354 EMS-to-hospital pediatric trauma audio recordings from two Level I pediatric trauma centers (2023-2025). Audio recordings were transcribed and evaluated across four LLM prompting strategies (Simple Zero-Shot, L1-biased, L2-biased, Ensemble), t…

Automating standardization of prostate cancer biopsy and histopathology reports with privacy-preserving local large language models.

PURPOSE: Large-scale biomedical analysis in prostate cancer requires structured, tabular datasets, yet most clinical documentation remains in free-text format. The standard of manual data abstraction is time-consuming, error-prone, non-reproducible, and costly. We hypothesized that locally deployed, privacy-preserving large language models (LLMs) combined with traditional natural language processing (NLP) methods could automatically extract structured data from prostate biopsy procedure and pathology reports. MATERIALS AND METHODS: We deployed Mistral 7B locally to process 150 transrectal ult…

Advancing the Prediction and Understanding of Placebo Responses in Chronic Back Pain Using Large Language Models.

BACKGROUND: Placebo analgesia is a widely studied clinical phenomenon, yet placebo responses vary widely across individuals. Prior research has identified biopsychosocial factors that determine the likelihood of an individual to respond to placebo, yet generalizability and ecological validity in those studies have been limited due to the inability to account for dynamic personal and treatment effects. METHODS: We assessed fine-tuned large language models (LLMs) for the prediction of placebo responses in chronic low-back pain using contextual features extracted from patient interviews, as they…
