INTRODUCTION: Potential indications for excimer laser procedures in children include accommodative esotropia, bilateral high ametropia, and severe anisometropia. Treatment of these conditions has traditionally included spectacle or contact lens use. This treatment, however, is often ineffective in children with severe anisometropia or bilateral ametropia, especially those with neuropsychological disorders. We examined three patients who developed keratitis after myopic photorefractive keratectomy. All patients were treated on the same day and in all three cases paraformaldehyde tablets were used for disinfection of the excimer laser iris cone. BACKGROUND: We describe a new technique of inferior off-center ablation with laser in situ keratomileusis (LASIK) to correct both hyperopia and presbyopia.
METHODS: This prospective clinical study was based on the empirical results obtained with LASIK in 16 hyperopic eyes of 8 patients. All patients had a centered ablation in one eye and an inferior decentered ablation in the other eye. A Schwind excimer laser was used and a planned inferior off-center ablation of 1 mm was performed. Maximum follow-up was 22 months (8 eyes).

We present a case of unilateral iatrogenic keratectasia developing 15 months after bilateral laser in situ keratomileusis using a broad-beam excimer laser (Bausch & Lomb Keracor 116) to treat -3.5 -1.5 x 85 diopters of myopia. Preoperative pachymetry in the eye measured 450 microm without topographical changes suggesting keratoconus or forme fruste keratoconus.
