Palliative, end-of-life care for the burn-injured patient.

This review seeks to describe end-of-life trajectories in burns patients, and a holistic approach to care which encompasses the patient, the family and the health care professionals who care for them. It includes relevant considerations in determining the indications for end-of-life care rather than active treatment aimed at cure. Care for the dying burns patient is described in emergency department, intensive care and ward settings. Inclusive communication to ensure appropriate and consensus-based decision-making which is supported by the treating team, patient and family is paramount in deve

Care Needs of Patients With Chronic Cardiovascular Diseases: An Integrative Review.

BACKGROUND AND AIMS: Chronic cardiovascular diseases (CVDs) remain a primary contributor to global mortality. Despite their prevalence, the comprehensive care needs of affected patients are not fully elucidated. This integrative review sought to synthesize current evidence on the multifaceted care requirements of individuals with chronic CVDs. METHODS: A comprehensive literature search was performed using PubMed, Scopus, and Web of Science, targeting studies published between 2010 and February 2025. The Whittemore and Knafl framework guided the methodology, incorporating both quantitative and

An impossible dream?

The extension of assisted dying to those whose natural death is not reasonably foreseeable has very real implications for how we think about life-limiting illness and the systems of care we design to palliate suffering. We now have a new idea of a life-limiting illness; dying is a result of an illness's impact on quality of life, not length of life. In this article, I tell the story of "Sam," whose complex illness and suffering led her to choose an assisted death. I reflect on my own responses to Sam and conclude with a dream for a palliative system of care designed uniquely to support those c

Oral Facial Pain: A spotlight on Trigeminal Neuralgia and its variants for the primary care dental team.

Trigeminal neuralgia (TN) is a debilitating facial pain condition that can significantly impact a patient's quality of life. Typically, pain episodes are unpredictable lasting seconds to minutes, usually presenting in a unilateral distribution of the trigeminal nerve.Patients will often seek help from primary care dental practitioners during an acute attack, in need of pain relief and support in managing the pain. This narrative article aims to highlight the key features of TN and how the primary dental care team can actively support management of patients presenting with an acute episode of pa

Social support, quality of life, immune function, and health in persons living with HIV.

The purpose of this study was to examine the relationship between social support and quality of life in individuals with HIV. Using a descriptive, correlational design, data were collected from 50 HIV-positive individuals who were: (a) participants in support groups at a behavioral medicine unit, (b) inpatient or respite care patients with HIV, or (c) respondents to advertisements at AIDS service organizations. Instruments used for data collection were the Personal Resource Questionnaire 85-Part 2 (Weinert, 1987), measuring perceived social support, and the Quality of Life Index (QLI) (Ferrans

Palliative care, homelessness, and restricted or uncertain immigration status.

BACKGROUND: People experiencing homelessness have limited access to palliative care support despite high levels of ill health and premature mortality. Most research exploring these challenges in the United Kingdom has focused on people living in hostels or temporary accommodation. People with uncertain or restricted immigration status are often unable to access this accommodation due to lack of entitlement to benefits. There is little research about the experiences of those in the United Kingdom who cannot access hostels or temporary accommodation due to restricted or uncertain immigration sta

Patient-Reported Social Impact of Molecularly Confirmed Retinitis Pigmentosa.

Objectives: To evaluate the patient-reported impact of retinitis pigmentosa (RP) in a large patient cohort to identify relevant disease-related disadvantages as key aspects for the improvement of ophthalmic and social care. Methods: Consecutive patients with molecularly confirmed RP older than 18 years of age were identified in two tertiary care centers in Germany. Patients were contacted to participate in an anonymized patient query regarding the impact of RP on their vocational training, professional career, and social, familial, and personal life, as well as their experience with ophthalmol

Evaluating challenges care-partners and older adults confront after major elective surgery: A multimethods study.

BACKGROUND: With the advent of enhanced recovery after surgery pathways, older adults who undergo major elective surgery return home early in the postoperative period and rely more on care-partner support. Little has been done to assess current challenges that older patient/care-partner dyads experience after major elective surgery and to identify intervention targets to improve transitions home after surgery. METHODS: This multimethods study recruited 15 dyads of major elective surgery patients aged 65+ years from 3 surgical specialties and their care-partners (30 participants total). Demogr

Scottish Rite, its physicians, staff, agents, and employees are enjoined from taking any action inconsistent with this order. This Court hopes that the ordeal suffered by all concerned in having to resort to the courts for direction will soon be alleviated by judicial precedent or legislative enactment establishing proper rules and procedures for all involved in a future dilemma such as this one. Adaptation to Climate Change and Climate Protection in Geriatric Care for Older People: Strategies, Infrastructure, and Good Practice Examples.

Anthropogenic climate change presents significant health challenges for older people with care needs across all care settings, from private homes and supported living arrangements to institutional care facilities. Those particularly at risk include individuals with limited mobility, multimorbidity, or social isolation. Care and support systems therefore need to implement measures for both climate change mitigation and adaptation. Mitigation encompasses reducing energy and water consumption, minimising waste, decreasing reliance on fossil fuels, managing chemicals, and strengthening political a

Review article: the current management of acute liver failure.

BACKGROUND: Acute liver failure is a devastating clinical syndrome with a persistently high mortality rate despite critical care advances. Orthotopic liver transplantation (OLT) is a life-saving treatment in selected cases, but effective use of this limited resource requires accurate prognostication because of surgical risks and the requirement for subsequent life-long immunosuppression. AIM: To review the aetiology of acute liver failure, discuss the evidence behind critical care management strategies and examine potential treatment alternatives to OLT. METHODS: Literature review using Ovid

[Borderline personality disorder features and quality of life in young people: the role of social support].

BACKGROUND: Adolescents and young adults with borderline personality disorder (BPD) features, often experience reduced quality of life. Whereas for many young people, friendships are usually supportive, friendships in young people with BPD features can be intense and challenging. Little is known about how support from a best friend might moderate the association between BPD features and quality of life, specifically during a developmental phase when forming friendships is an important developmental task. AIM: To examine the association between BPD features and quality of life in young people

Longitudinal Perinatal Palliative Care for Severe Fetal Neurologic Diagnoses.

Perinatal palliative care (PnPC) focuses on enhancing family and newborn quality-of-life in the setting of a life-limiting fetal condition while simultaneously ensuring medical care delivery that is soundly in line with the family's goals and values. The inclusion of PnPC in the multidisciplinary care of a family facing severe fetal neurologic diagnoses allows for skilled exploration of the values and experience that ultimately drive creation of goals of care. The parental experience of receiving a life-limiting fetal diagnosis pushes parents into an emotional journey, which typically follows

Delivering effective end of life care in the emergency department: a nursing perspective.

The unpredictability of acute care, rising patient multimorbidity and the under-resourcing of community palliative care services mean that more patients are presenting to emergency services in the final stages of illness, with emergency departments (EDs) increasingly becoming places where people experience end of life. Nurses are often the first to recognise a dying patient and initiate essential care in the ED's high-pressure, intervention-focused environment. This narrative review article explores how emergency nurses can provide high-quality care at the end of life when supported by access

"Aged care is more than wiping bums and giving medication" - a qualitative study of the sensory support care needs of older Australians with hearing and/or vision impairment.

PURPOSE: Older adults with hearing and/or vision impairment often face challenges in accessing appropriate support within home care services. This study investigated the hearing and/or vision support care needs of older Australians receiving home care, and assessed whether these needs were met. METHODS: Semi-structured interviews were conducted with 13 home care recipients (mean age 81.2 years, range 71-94) reporting hearing (n = 3), vision (n = 4) or dual sensory impairments (n = 6), and three family members. Interviews explored physical, psychological, and communication needs, healthcare sy

Burden of caregiving of individuals with GM1 and GM2 gangliosidoses in the United States: a qualitative study.

BACKGROUND: GM1 and GM2 (Tay-Sachs and Sandhoff diseases) gangliosidoses are rare, autosomal recessive, potentially life-threatening, disabling disorders characterized by progressive neurodegeneration, with no disease-modifying treatment. This qualitative study aimed to understand the humanistic burden of GM1 and GM2 gangliosidoses from caregivers' perspectives by expanding knowledge on the day-to-day responsibilities of primary caregivers and the impacts experienced while providing care and support. METHODS: Focus groups (90-minute duration) were conducted with caregivers (≥ 18 years) under

[The care pathway for patients with chronic kidney disease: The essential coordination between community health care professionals and care facilities].

Chronic kidney disease (CKD) is a major public health issue, affecting around 10% of the world’s population. Its silent progression and its numerous etiologies (most commonly diabetes and vascular nephropathies) complicate early detection and management. Many patients reach kidney failure without prior nephrological care. Coordination between outpatient care (GPs, pharmacists, nurses) and specialist facilities is therefore essential to improve screening, delay the progression to dialysis, and improve patients’ quality of life. The CKD package, introduced in 2019, provides financial support to

The impact of delirium on withdrawal of life-sustaining treatment after intracerebral hemorrhage.

OBJECTIVE: To determine the impact of delirium on withdrawal of life-sustaining treatment (WLST) after intracerebral hemorrhage (ICH) in the context of established predictors of poor outcome, using data from an institutional ICH registry. METHODS: We performed a single-center cohort study on consecutive patients with ICH admitted over 12 months. ICH features were prospectively adjudicated, and WLST and corresponding hospital day were recorded retrospectively. Patients were categorized using DSM-5 criteria as never delirious, ever delirious (either on admission or later during hospitalization)

Organ donation after medical assistance in dying or cessation of life-sustaining treatment requested by conscious patients: the Canadian context.

In June 2016, following the decision of the Supreme Court of Canada to decriminalise assistance in dying, the Canadian government enacted Bill C-14, legalising medical assistance in dying (MAID). In 2014, the province of Quebec had passed end-of-life care legislation making MAID available as of December 2015. The availability of MAID has many implications, including the possibility of combining this practice with organ donation through the controlled donation after cardiac death (cDCD) protocol. cDCD most often occurs in cases where the patient has a severe neurological injury but does not mee
