Applying Critical Feminist and Critical Race Theory to Address the Cultural and Financial Needs of Black/African American Women With Cancer: A Narrative Review.

Black/African American women with breast and gynecologic cancers face stark end-of-life (EOL) inequities rooted in structural racism, gendered oppression, and financial toxicity. Despite abundant evidence of outcome gaps, theory-driven guidance for equitable, culturally responsive EOL care is limited. This narrative review argues for integrating Critical Race Theory (CRT) and Critical Feminist Theory (CFT) into EOL care frameworks to address the cultural and financial needs of Black/African American women. Drawing on interdisciplinary literature from healthcare, philosophy, sociology, and econ

[Requirements for outpatient care after implantation of a ventricular assist device : Views of patients and their relatives].

Ventricular assist device (VAD) implantation has developed into a well-established option when conservative treatment of terminal heart failure has been exhausted. Figures from 2015 make this clear: only 283 heart transplantations were performed nationwide but 959 VAD systems were implanted. It is noteworthy that the survival times with a VAD are approaching the survival times after heart transplantation. Patients with VADs have a life-long dependency on their proximity to specialists. So far, the requirements for outpatient care have not been systematically recorded from the perspective of VA

Quality of Life and Patient Satisfaction in Adults Treated for a Cleft Lip and Palate: A Qualitative Analysis.

OBJECTIVE: Long-term outcomes of patients born with a cleft lip and palate (CLP) are scarcely investigated. Yet, this patient group is of particular interest, as they can provide a valuable retrospective view upon their treatment experiences and psychological adjustment. Qualitative accounts may be especially useful in understanding the patient journey. DESIGN: The present study set out to evaluate quality of life and satisfaction with treatment in adult patients previously treated for CLP at the Wilhelmina Children's Hospital. Semistructured interviews were performed. PATIENTS: A total of 2

Extracorporeal resuscitation as a further modifier of clinical outcome in patients with left ventricular assist device implantation and Interagency Registry for Mechanically Assisted Circulatory Support level 1.

In cardiogenic shock patients with Interagency Registry for Mechanical Circulatory Support (INTERMACS) level 1, the need for temporary circulatory support is a predictor and modifier of patient outcome. Because this group includes patients with and without cardiopulmonary resuscitation (CPR) and is thus very heterogeneous, we investigated whether a further subclassification is useful. We compared 30-day and 1-year mortality of patients who underwent left ventricular assist device implantation after extracorporeal CPR with the aid of an extracorporeal life support system (CPR+ group; n = 40) wi

District health management teams as champions for implementing the WHO LIVE LIFE suicide-prevention framework in Rural Uganda: a pre-implementation qualitative study.

BACKGROUND: Suicide is a major public-health challenge globally and in Uganda, where criminalization, stigma, and weak surveillance hinder prevention. The WHO LIVE LIFE framework offers evidence-based strategies, but there is limited experience implementing it at the district level in low-resource settings. Guided by the i-PARIHS framework, the "WHO LIVE LIFE with District Health Management Team as Champions for Suicide Prevention across the Lifespan in Rural Uganda (WLLDC-SP)" initiative positions District Health Management Team (DHMT) members as implementation champions. This study explored

Human in the loop in AI-enabled clinical decision support: a systematic scoping review and reporting checklist for lifecycle governance.

OBJECTIVE: To examine the characteristics, implementation strategies, and reported impacts of Human-in-the-Loop (HITL) processes across the lifecycle of AI-enabled Clinical Decision Support Systems (CDSS), and to propose a reporting checklist for HITL in clinical AI research. INTRODUCTION: HITL is a conceptual and technical approach that integrates human expertise into AI systems, widely recognised for its potential to improve transparency, safety, and contextual relevance in clinical settings. While AI-CDSS tools are increasingly embedded in healthcare, HITL practices remain poorly defined a

Updating functional brain units: Insights far beyond Luria.

This paper reviews Luria's model of the three functional units of the brain. To meet this objective, several issues were reviewed: the theory of functional systems and the contributions of phylogenesis and embryogenesis to the brain's functional organization. This review revealed several facts. In the first place, the relationship/integration of basic homeostatic needs with complex forms of behavior. Secondly, the multi-scale hierarchical and distributed organization of the brain and interactions between cells and systems. Thirdly, the phylogenetic role of exaptation, especially in basal gangl

Reorganisation of the Basic Life Support Segment in a Physician-Led Emergency Medical Service: A Retrospective Evaluation of Operational and Clinical Characteristics.

Background: Increasing demand for emergency medical services (EMS) challenges prehospital resource availability, particularly for time-critical emergencies requiring advanced life support (ALS). Although tiered BLS and ALS response models are established in many non-physician-led EMS systems, evidence regarding qualification-based dispatch stratification within physician-led EMS systems remains limited. This study evaluated the clinical and operational characteristics of an expanded BLS dispatch segment in Berlin EMS. Methods: A retrospective observational study analysed 2,132,246 EMS missions

The maternal-placental-fetal interface: Adaptations of the HPA axis and immune mediators following maternal stress and prenatal alcohol exposure.

This review addresses underlying physiological, cellular, and molecular factors that alter the developing fetal brain stress circuits and responses of the hypothalamic-pituitary-adrenal (HPA) axis caused by maternal stress and prenatal alcohol exposure (PAE). An emphasis is placed on the contribution of the placenta following maternal stress separately, and as a co-occurrence with PAE. Altered fetal HPA axis ultimately results in dysregulation of the brain stress-response system long after birth and possibly lifelong. Additional consideration of the role of placentally-derived endocrine and se

Cardiopulmonary support in the intensive care unit.

The cardiopulmonary support system is an extracorporeal device that allows for rapid cardiopulmonary support of the critically ill patient in the intensive care unit. It provides immediate and complete support of cardiac and pulmonary functions to maintain perfusion to vital organs in patients who are severely physiologically compromised (eg, in cardiogenic shock, adult respiratory distress syndrome or pulmonary edema). Successful cardiopulmonary support requires systemic anticoagulation, percutaneous venous and arterial cannulation and careful monitoring by the critical care team to maintain

Recurrent depression: an assessment of family burden and family attitudes.

Recognition that a psychiatrically ill family member has profound effects on family life and significant others has stimulated the need for more precise assessments of family members. The authors evaluated the burden experienced by those in the immediate environment of an individual with recurrent depressive illness as well as the beliefs they hold about the illness. This report presents a preliminary body of data on the level and types of distress shared by family members, especially family members living with the patient. Such information is valuable in designing educational interventions fo

Persistent Financial Burdens and High Donor Satisfaction Among Living Liver Donors: A 24-y Single-center Study.

BACKGROUND: Prior investigations on living donor hepatectomy have primarily focused on medical and psychosocial outcomes, with limited evaluation of financial implications. Financial support systems exist, but these are limited by strict income criteria, leaving some donors ineligible. This study sought to characterize donor-reported quality of life with a focus on postdonation financial burden. METHODS: Over a 24-y period, 402 living donor hepatectomies were performed at a single institution. In 2024, donors were contacted by telephone and surveyed regarding their postdonation experiences, w
