A 79-year-old woman was admitted with a 5-kg weight loss and anorexia. Computed tomography showed diffuse lymphadenopathy, and thickening of the duodenal and ileal walls. The patient then underwent biopsy of these sites. Pathological examination revealed duodenal Epstein-Barr virus (EBV)-positive peripheral T cell lymphoma-not otherwise specified (PTCL-NOS) and EBV-negative ileal diffuse large B-cell lymphoma (DLBCL) to be present simultaneously. Combination chemotherapy including rituximab produced a reduction of the duodenal EBV-positive PTCL-NOS lesion, but had no effect on the EBV-negative

A 92-year-old man with a history of diffuse large B-cell lymphoma treated with chemoradiotherapy, and later with radiotherapy for an isolated cutaneous relapse with complete response, presented with a 1-month history of multiple asymptomatic erythematous-to-violaceous papules and nodules on the face, trunk, and upper limbs. Given his oncological history and previous skin involvement, lymphoma relapse was the leading diagnostic concern, with cutaneous metastases and sarcoidosis also considered. However, skin biopsies showed a non-Langerhans cell histiocytic infiltrate expressing CD45, CD68, and

In this report, we present the case of a patient with an uncommon triple diagnosis of (1) follicular (nodular) lymphoma, (2) nodular lymphocyte predominant Hodgkin lymphoma, and (3) diffuse large B-cell lymphoma non-germinal center B-cell (non-GCB) subtype. Although transformation of follicular lymphoma and nodular lymphocyte predominant lymphoma to more aggressive forms such as diffuse large B-cell lymphoma is possible; it generally happens many years after diagnosis. Moreover, there have been reported cases of follicular lymphoma with transformation and nodular lymphocyte predominant Hodgkin

BACKGROUND: MYC is a heterogeneously expressed transcription factor that plays a multifunctional role in many biological processes such as cell proliferation and differentiation. It is also associated with many types of cancer including the malignant lymphomas. There are two types of aggressive B-cell lymphoma, namely Burkitt lymphoma (BL) and a subgroup of diffuse large cell lymphoma (DLBCL), which both carry MYC translocations and overexpress MYC but both differ significantly in their clinical outcome. In DLBCL, MYC translocations are associated with an aggressive behavior and poor outcome,

Jejunoileal follicular lymphomas (FLs) are rare and have been reported to undergo histological transformation (HT). We report a case of jejunoileal FL transformation into diffuse large B-cell lymphoma (DLBCL). An 82-year-old woman presented with a 5.5 cm ulcerated jejunal mass, identified through double-balloon enteroscopy. The histopathology report revealed diffuse atypical lymphoid cells, which confirmed the presence of DLBCL. Neoplastic follicles confirmed the presence of FL. Genetic analysis revealed a match between the FL and DLBCL. Following a segmentectomy and chemotherapy, the patient

INTRODUCTION: Non-Hodgkin lymphoma (NHL) can occur at any site of the body, however diffuse and extensive involvement of the peritoneal cavity occurs rarely. Diffuse large B-cell lymphoma (DLBCL) is one of the dominant histological subtypes in extra nodal lymphoma involving the peritoneal cavity while Burkitt lymphoma is seen less commonly. We report two cases of DLBCL and one case of Burkitt lymphoma presenting with peritoneal lymphomatosis (PL).
PRESENTATION OF CASE: A retrospective review of two cases involving DLBCL and one case of Burkitt lymphoma with PL was conducted. Findings in all p

Follicular lymphoma (FL) frequently transforms into diffuse large B-cell lymphoma (DLBCL). To clarify the associated clinicopathological prognostic parameters, we examined the correlation of 11 histopathological parameters with progression-free survival (PFS) and overall survival (OS) in 107 consecutive patients who had DLBCL with pre-existing (asynchronous) or synchronous FL. The patients comprised 58 men and 49 women with a median age of 56 years. For DLBCL, the complete response rate was 81%, overall response rate was 88%, and 5-year PFS and OS rates were 55% and 79%, respectively. Immunohi

Multiple different lymphomas in a single person are very rare. The author herein reports the case of a 69- year-old Japanese woman with double gastrointestinal lymphoma. The patient presented with epigastralgia. Endoscopic examination revealed erosions and elevation of the gastric body and a large ulcerated tumor of the terminal ileum. Biopsies were obtained from these lesions. The gastric lesion was MALT lymphoma with monocytoid B-cell proliferation and lymphoepithelial lesions. Light chain restriction was present. Helicobacter pylori were present on Giemsa stain. The gastric lesions did not

It has long been recognized that the border between classical Burkitt's lymphoma (BL) and classical diffuse large B-cell lymphoma (DLBCL) is hard to determine. Instead, both classical lymphoma entities seem to be the extreme ends of a spectrum of diseases that contains a group of lymphomas characterized predominately by the fact that they are hard to assign to the one or the other group. This gray zone has been recently termed "lymphoma, unclassifiable, with features intermediate between DLBCL and BL" by the updated WHO classification. The term "intermediate" resembles that from a recent gene-

Lymphoid malignancies are grouped and characterized by their morphology, immunophenotype, and genetic aberrations to help establish a diagnosis. Use of microarray technology enables the simultaneous determination of expression levels for thousands of genes, providing an additional powerful tool for improving disease classification. In this review, recent studies of diffuse large B-cell lymphoma (DLBCL), primary mediastinal large B-cell lymphoma, mantle cell lymphoma (MCL), and follicular lymphoma are highlighted, and the impact of gene expression profiling on the molecular diagnosis of these d

Primary cutaneous B-cell lymphomas are a heterogeneous group of lymphoid neoplasms primarily occurring in the skin. Although most cases are represented by primary cutaneous follicle center cell lymphoma, primary cutaneous marginal zone lymphoma and leg-type diffuse large B-cell lymphoma, other diffuse large B-cell lymphomas and B-cell lymphoblastic lymphoma may rarely present primarily in the skin. In this setting, the presence of histopathologic and immunohistochemical features of cellular immaturity is exceedingly rare and may represent a diagnostic challenge. We present the first case of a

Human B cell lymphomas often contain CD4 T cells. Here we show that, in diffuse large B cell lymphomas (DLCL), such T cells are oligoclonal. The CDR3 lengths and nucleotide sequences of oligoclonal TCRBV of CD4 T cells in an original and relapsed lymphoma from one patient were compared. Three BV23 sequences were identical (12/17 and 16/16 clones in primary and relapsed lymphomas, respectively), but were absent in CD4 T cells from another patient's DLCL. Two of the repetitive BV23 sequences were found in peripheral blood CD4 T cells (5/17 clones); gamma-irradiated DLCL from this patient stimula

Aberrant MYC activity defines the most aggressive GCB-DLBCLs. We characterized a mechanism of MYC transcriptional activation via a native enhancer that is active in MYC-intact GCB-DLBCL, establishing fitness-sustaining cis- and trans-regulatory circuitry in GCB-DLBCL models that lack MYC enhancer-hijacking rearrangement. See related commentary by Mulet-Lazaro and Delwel, p. 149.

B-cell lymphoma 6 (BCL6) is a transcriptional repressor that regulates the differentiation of B lymphocytes and mediates the formation of germinal centers (GCs) by recruiting corepressors through the BTB domain of BCL6. Physiological processes regulated by BCL6 involve cell activation, differentiation, DNA damage, and apoptosis. BCL6 is highly expressed when the gene is mutated, leading to the malignant proliferation of cells and drives tumorigenesis. BCL6 overexpression is closely correlated with tumorigenesis in diffuse large B-cell lymphoma (DLBCL) and other lymphomas, and BCL6 inhibitors c

Primary ovarian lymphoma is rare. To better understand the clinicopathological and molecular features of these tumours, we studied 14 patients (age range 13 to 74 years, mean 46.9 years) with non-Hodgkin's lymphoma who initially presented with an ovarian tumour. The clinicopathological features were reviewed and tissue samples were studied by immunohistochemistry and for immunoglobulin gene rearrangement, presence of Epstein Barr virus and for MYC and BCL-2 rearrangements by fluorescence in situ hybridisation. The most common symptoms were abdominal pain and abdominal distension. Ten patients

To elucidate the mechanisms underlying chromosomal translocations in diffuse large B cell lymphoma (DLBCL), we investigated the nature and extent of immunoglobulin class switch recombination (CSR) in these tumors. We used Southern blotting to detect legitimate and illegitimate CSR events in tumor samples of the activated B cell-like (ABC), germinal center B cell-like (GCB), and primary mediastinal B cell lymphoma (PMBL) subgroups of DLBCL. The frequency of legitimate CSR was lower in ABC DLBCL than in GCB DLBCL and PMBL. In contrast, ABC DLBCL had a higher frequency of internal deletions withi

BACKGROUND: Anaplastic lymphoma kinase-positive diffuse large B-cell lymphoma (ALK + DLBCL) is a rare and highly aggressive mature B-cell lymphoma with poor responses to conventional chemotherapy and no established standard treatment. Although ALK inhibitors have shown potential activity in ALK-rearranged malignancies, their clinical utility and optimal sequencing in ALK + DLBCL remain unclear.
CASE PRESENTATION: We report a 41-year-old male patient diagnosed with ALK + DLBCL harboring a TFG-ALK rearrangement. The patient experienced progressive disease during the clinical course and showed r

PURPOSE: To describe the evaluation, diagnosis, and treatment of vitreoretinal lymphoma presenting as frosted branch angiitis in a patient with diffuse large B-cell lymphoma (DLBCL).
OBSERVATIONS: A 57-year-old woman with a history of non-Hodgkin lymphoma and recent DLBCL relapse presented with frosted branch angiitis that raised suspicion for an infectious retinitis but was found to be vitreoretinal lymphoma.
CONCLUSIONS AND IMPORTANCE: This case primarily highlights the importance of considering vitreoretinal lymphoma on the differential diagnosis of etiologies of frosted branch angiitis.
