Cholesteatoma Causing a Horizontal Semicircular Canal Fistula.

Horizontal canal fistulas are not uncommon in patients with cholesteatoma. Patients with canal wall down cavities and exposed horizontal canal fistulas develop significant dizziness with wind or suction exposure. Obliteration of mastoid cavities in patients with exposed fistulas can be challenging. We describe a patient with horizontal canal fistula and chronic dizziness from wind exposure who underwent successful mastoid cavity obliteration with preservation of hearing. Patients with horizontal canal fistulas in a canal wall down cavity can be managed with mastoid obliteration for relief of d

Pulsatile tinnitus caused by pneumocephalus after Janneta surgery.

Pulsatile tinnitus of nonvascular origin is rare. We herein present a case of pulsatile tinnitus complicated with Jannetta surgery due to a communication created between the drilled mastoid cells and epidural space. She was successfully cured by otological surgery where the mastoid tip was packed with bone cement. A 68-year-old woman was referred to the previous hospital with complaints of right autophony, aural fullness, hyperacusis to her footsteps, and pulsatile tinnitus for the past three years. She had received Jannetta surgery for right hemifacial spasm seven years before. The computed t

Presigmoid approach preserving the superior petrosal sinus in a pontine cavernous malformation associated to abnormal venous drainage of the brainstem: how I do it.

BACKGROUND: The presigmoid approach classically includes the ligature and section of the superior petrosal sinus to get a wider visibility window to the antero-lateral brainstem surface. In some cases, the separation of this venous structure should not be performed. METHOD: We present our experience getting safely to a pontine cavernous malformation through a conventional mastoidectomy presigmoid approach preserving an ingurgitated superior petrosal sinus because the association with an abnormal venous drainage of the brainstem. CONCLUSIONS: When sectioning the superior petrosal sinus in cla…

IgG4-related pachymeningitis and mastoiditis, associated with cerebral venous thrombosis: A case report.

IgG4-related disease (IgG4-RD) is a multisystem fibroinflammatory condition; this can be a challenging diagnosis that requires clinico-pathologic correlation. We report a young woman, presenting with cranial nerve palsy. The work-up revealed pachymeningitis, cerebral venous thrombosis (CVT), and a destructive lesion in the mastoid. We diagnosed IgG4-RD through mastoidectomy. Thus, a biopsy of asymptomatic, infrequently affected organs, like the mastoid, can meet all histopathological criteria. In neuro-meningeal presentations, CVT may be secondary to the local inflammatory environment of pachy…

Acute Mastoiditis Complicated with Concomitant Bezold's Abscess and Lateral Sinus Thrombosis.

Bezold's abscess is a very rare extracranial complication of acute mastoiditis. Lateral sinus thrombosis is another intracranial complication of acute mastoiditis that can occur, but there are only few reports of concomitant ispilateral Bezold's abscess and lateral sinus thrombosis with favorable outcome. We diagnosed and treated successfully a 14-year-old girl suffering from Streptococcus pneumoniae acute mastoiditis complicated with Bezold's abscess and lateral sinus thrombosis. Surgical treatment included myringotomy, cortical mastoidectomy, and Bezold's abscess incision and drainage. Durin…

First branchial cleft fistula associated with external auditory canal stenosis and middle ear cholesteatoma.

INTRODUCTION: First branchial cleft anomalies manifest with duplication of the external auditory canal. CASE REPORT: This report features a rare case of microtia and congenital middle ear and canal cholesteatoma with first branchial fistula. External auditory canal stenosis was complicated by middle ear and external canal cholesteatoma, but branchial fistula, opening in the zygomatic root and a sinus in the helical root, may explain this feature. A canal wall down mastoidectomy with canaloplasty and wide meatoplasty was performed. The branchial cleft was excised through parotidectomy and faci…

[Necrotizing otitis externa in a 5-month-old infant].

This report presents the case of a healed 5-month-old infant with necrotising (malignant) bilateral otitis externa from acute mastoiditis on the right side and sepsis caused by Pseudomonas aeruginosa infection. Despite of immediately performed mastoidectomy, targeted antibiotics and intensive local treatment, two third of both external auditory canal's epithelium had shown subcutaneous concentric necrosis and ejection which have been removed with repeated necretomies. After the remission of inflammatory symptoms, successful bilateral auditory canal reconstructions were performed. The observed …

Chronic middle ear disease and cochlear implantation.

We have presented three patients with bilateral CSOM who were otherwise suitable for cochlear implantation. These patients had conventional surgical management of CSOM and were implanted after a period of 6 months through apparently healthy middle ears. However, all three had a recurrence of middle ear disease requiring further surgery, and one had a potentially life-threatening complication necessitating removal of the implant. Before considering cochlear implantation in patients with bilateral CSOM, it is essential to eradicate all disease, and radical mastoidectomy with obliteration may be …

Perforated tympanic membrane after blast injury.

The rate of spontaneous healing of tympanic membrane perforations in relation to time after blast injury, size, and location of perforations, was evaluated in 147 patients (210 ears). In 46 patients a paper patch was placed over the perforation. Spontaneous healing was seen in 155 ears (74%), 131 (62%) healed within the first 3 months and 145 ears (69%) healed within 10 months. Large and central kidney-shaped perforations had the least tendency to heal and usually required surgical intervention. Tympanoplasty was performed on 33 ears (15%), ossiculoplasty on 5 and mastoidectomy on 10. Interven…

Extensive craniocervical pneumocranium.

The authors report the first case of extensive craniocervical pneumocranium causing mass effect, without any associated extradural pneumatocele. The patient presented with frontal headaches and unusual sensations in his left ear. He was found to have large pneumocranium involving his left frontal, temporal, parietal, and occipital bones as well as the atlas. He underwent a craniectomy with replacement of the pneumatized bone with titanium mesh, and mastoidectomy with obliteration of the mastoid air cells. This case is discussed in the context of the literature to date. A review of the literatu…

Management of abducens palsy in patients with petrositis.

Our objective was to discuss the management and outcome of abducens nerve palsy in patients with Gradenigo's syndrome. In a retrospective analysis of patients with Gradenigo's syndrome at a tertiary-care center in Houston, Texas, from 1987 to 1995, we identified 2 patients with Gradenigo's syndrome, both female. One had bilateral involvement, so that the total was 3 ears. Both patients had complete recovery of their abducens nerve palsy. In 2 ears with chronic mastoiditis, sixth nerve palsies failed to respond to medical therapy alone, but resolved after mastoidectomy with drainage of the petr…

Tympanoplasty: Factors influencing surgical outcome.

We reviewed the records of 183 patients who had undergone myringoplasty during a six year period at the Asir Central Hospital (ACH). This study was limited to cases of repair of uncomplicated perforation of tympanic membrane that did not require ossiculoplasty or mastoidectomy. One hundred and thirty-four (73.2%) were successful myringoplasty and 78 (75.7%) patients had their hearing improved postoperatively. Various factors liable to influence the success rate such as status of middle ear, size and site of perforations, surgical approach and presence of tympanosclerosis were analyzed. Only st…

[Acute mastoiditis: 16 clinical cases].

Sixteen cases of acute mastoiditis were treated between 1980 and 1984. In infants the course of the mastoiditis was brief. An external retroauricular swelling soon occurred that was generally due to edema without abscess formation, and medical treatment resulted in rapid healing. For the older children, the acute mastoiditis occurred on an acute otitis media that had already been present for some time. Abscess formation occurred in half of the cases, and a mastoidectomy was generally required. For the adult patients, there was always an underlying chronic ear pathology, either a cholesteatoma …

Toxic shock syndrome following septorhinoplasty. Implications for the head and neck surgeon.

Toxic shock syndrome (TSS) is an acute multisystem disease that has recently been recognized to occur in a variety of clinical settings. Postoperative TSS has been described following many different surgical procedures, though, to our knowledge, only one report has been published to date describing TSS following a head and neck procedure. We report a case of classic TSS following septorhinoplasty. The analogous role of postoperative nasal packing and the packing of other cavities (eg, in maxillectomies or mastoidectomies) to the use of vaginal tampons as a possible risk factor for TSS developm

Unusual Presentation of a Rare Disease: A Case Report of Fungal Necrotizing Otitis Media and Mastoiditis.

Acute necrotizing otitis media is a severe middle ear infection which causes necrosis of the tympanic cavity. A 54-year-old female was presented who suffered from diabetes mellitus and end-stage renal disease presenting with severe otalgia, initially thought to be necrotizing otitis externa. She rapidly progressed to total necrosis of the tympanic membrane. Further assessments confirmed a fungal disease caused by Candida tropicalis and Aspergillus flavus. Immediate antifungal therapy with amphotericin B and subsequent radical mastoidectomy managed to control the disease with no sign of necrosi…

Endoscopic Management of Pediatric Cholesteatoma.

Pediatric cholesteatoma occurs in one of two forms: congenital cholesteatoma, developing from embryonic epidermal cell rests or acquired cholesteatoma, associated with a focal defect in the tympanic membrane. This disease has been traditionally managed with the operating microscope, often requiring mastoidectomy for adequate visualization of and access to the middle ear and mastoid cavities. Recently, advances in endoscopic equipment have enabled otologists to manage most cases of pediatric cholesteatoma via a minimally-invasive, transcanal endoscopic approach. This review discusses the curren…

Otologic Wegener's granulomatosis with facial nerve palsy.

Wegener's granulomatosis, characterized by necrotizing granulomas and vasculitis of the respiratory tract and kidney, frequently first presents with otologic symptoms. We report a case of primary otologic Wegener's granulomatosis in a patient who presented with symptoms of acute otomastoiditis and associated facial nerve palsy. The patient subsequently developed neuropathies of various cranial nerves. The patient underwent urgent mastoidectomy with facial nerve decompression. Nonspecific inflammatory disease of the mastoid mucosa delayed the correct diagnosis of Wegener's granulomatosis, which…

[A case of sarcoidosis of the mastoid].

BACKGROUND: Sarcoidosis is a common granulomatous systemic inflammatory disease mainly affecting the interstitial lung tissue and perihilar lymph nodes. Since its first description by Bernier 1889 its etiology has remained obscure. METHOD: Sarcoidosis of the right middle ear in a 38-year-old woman was diagnosed by biopsy. RESULT: After mastoidectomy and treatment with prednisolone the patient has been free of relapse for two years. CONCLUSION: According to our knowledge this is the second reported case of sarcoidosis of the middle ear. In both cases the interstitial lung tissue has been inv…

Massive acquired cholesteatoma of the temporal bone.

Nine patients with massive acquired cholesteatoma of the temporal bone were treated by radical mastoidectomy, facial nerve decompression, and excision of the inner ear. The commonest presenting symptoms were otorrhea and deafness, but otalgia, vertigo, external swelling and facial nerve palsies were also seen. Polytomography was helpful in assessing the degree of bone erosion preoperatively. The large cavity present following cholesteatoma excision was partially obliterated by a superiorly based temporalis muscle pedicle which helped to seal the internal auditory meatus and prevented the devel…
