Medicaid Expansion and Overall Mortality in Lung, Breast, and Colorectal Cancer: A Difference-in-Differences Study.

BackgroundMedicaid expansion under the Affordable Care Act (ACA) was designed to improve access to care for low-income adults, but its impact on cancer survival may vary across diseases depending on differences in screening, stage at diagnosis, and treatment pathways.MethodsWe conducted a retrospective cohort study using the National Cancer Database to identify adults aged 40-64 years diagnosed with lung, breast, or colorectal cancer between 2007 and 2021. States were classified by Medicaid expansion status, and diagnoses were grouped into pre-ACA (2007-2013) and post-ACA (2015-2021) periods, …

Surgical Bailout During TAVR: A Contemporary Analysis of the Centers for Medicare and Medicaid Services Database.

BACKGROUND: Surgical bailout during transcatheter aortic valve replacement (TAVR) may be necessary to manage emergent TAVR complications. With discussions of performing TAVR in U.S. hospitals without in-hospital cardiac surgery support, analysis of TAVR bailout on a national level is needed. OBJECTIVES: This study sought to analyze the annual trends and outcomes of TAVR bailout. METHODS: This study included Medicare beneficiaries who underwent TAVR between 2016 and 2024. Annual trends of bailout were analyzed. The primary outcome was 30-day mortality. Additionally, the impact of hospital sur

Medicaid expansion and thyroid cancer stage at presentation: A comparative study of two US states.

BACKGROUND: The Affordable Care Act (ACA) enabled states to expand Medicaid eligibility, extending coverage to millions, particularly in California. While Medicaid expansion has been linked to earlier diagnoses in several cancers, its impact on thyroid cancer-a slow-growing malignancy lacking formal screening-remains unclear. OBJECTIVE: To assess whether Medicaid expansion influenced thyroid cancer stage at presentation. METHODS: A retrospective cohort study using SEER data (2009-2020) compared adults aged 18-64 diagnosed with thyroid cancer in California (expansion) and Texas (non-expansion…

The Essential Role of Medicaid in Duchenne Muscular Dystrophy Care: A Call to Protect Vulnerable Populations.

BACKGROUND: Duchenne muscular dystrophy (DMD) is the most common fatal pediatric neuromuscular disorder, resulting in premature morbidity and mortality. Multidisciplinary care and new therapies have prolonged quantity and quality of life, for which many families remain dependent on Medicaid for access to standards of care. Proposed Medicaid funding cuts would disproportionately harm children with complex medical conditions, including DMD, jeopardizing their access to essential services and specialized care. Children with DMD rely on comprehensive multidisciplinary care, including neurology, ca…

What Is the Effect of Medicaid Expansions on Preexposure Prophylaxis for HIV Prevention Use among Women?

Pre-exposure prophylaxis (PrEP) is a key biomedical tool for combatting HIV spread, but women account for only 5% of users. Between 2015 and 2019, men saw a 9% decrease in new HIV diagnoses, while rates among women remained constant. Using a difference-in-differences regression model and state level HIV surveillance data (utilizing sex assigned at birth) reported by AIDSVu from 2012 to 2021, we examined the impact of Medicaid expansions under the Affordable Care Act on PrEP outcomes. The study revealed that Medicaid expansions were associated with better PrEP outcomes overall, with increases o…

Medicaid Patients With ED Visits For Overdose: Disparities In Initiation Of Medications For Opioid Use Disorder.

Medications for opioid use disorder (MOUD) after emergency department (ED) visits for overdose can reduce subsequent overdose deaths, but disparities in receiving MOUD persist in the US. Using national Medicaid claims data from the period 2016-20, we examined racial and ethnic disparities in MOUD initiation after ED visits for opioid overdose. Overall, 6.4 percent of Medicaid ED visits were associated with a claim for MOUD within thirty days. This rate was highest among non-Hispanic White (7.3 percent) patients and lowest among non-Hispanic Black (4.3 percent) and Hispanic (4.9 percent) patien…

Moving from Medicaid Expansion Coverage to Medicare Can Be a Burdensome Transition: A Qualitative Study.

BACKGROUND: The Affordable Care Act expanded Medicaid eligibility for low-income adults who are not Medicare eligible while leaving in place states' more restrictive dual eligibility criteria. When Medicaid expansion enrollees turn 65 and transition to Medicare as their primary insurer, they may lose Medicaid and face higher premiums and out-of-pocket costs, yet there is little understanding of how older adults navigate this change in insurance programs. OBJECTIVE: To investigate the experiences of Medicaid expansion enrollees who transitioned to Medicare coverage at age 65. DESIGN: This is …

Medicaid Is an Important Driver of a Healthier North Carolina, But Not Enough to Dismantle Persistent Health Inequities.

Medicaid expansion has improved coverage for low-income North Carolinians, but persistent racial and geographic inequities limit its impact. Despite early gains and innovative social-care pilots, Black and Hispanic communities continue to face barriers to access, continuity, and outcomes. Lasting improvement will require sustained investment in primary care, navigation support, and the social factors that shape health.

Trends in Pregnancy Outcomes in People with Sickle Cell Disease and Medicaid Insurance (2006-2018).

Background: Although the risk of pregnancy-related morbidity and mortality in people with sickle cell disease (SCD) is well established, limitations in data sources and heterogeneity in outcome reporting hinder the ability to make meaningful comparisons between historical and contemporary populations. This study used a national administrative claims database to compare pregnancy outcomes in people with SCD between 2006-2011 and 2012-2018. Materials and Methods: Pregnant females aged 16-44 years with SCD were identified from the Centers for Medicare and Medicaid Service Analytic eXtract, along …

A Contemporary Evaluation of the Centers for Medicare and Medicaid Services High-risk Indicators for Carotid Endarterectomy.

OBJECTIVE: Compare stroke/death outcomes across carotid endarterectomy (CEA), transcarotid artery revascularization (TCAR), and transfemoral carotid artery stenting (TFCAS) using Centers for Medicare and Medicaid Services (CMS) high-risk criterion. BACKGROUND: Existing literature has revealed inconsistencies with CMS risk guidelines. With recent approval for TCAR and TFCAS in standard-risk patients, an updated analysis of guidelines is needed. METHODS: Data from the Vascular Quality Initiative (VQI) (2016-2023) on CEA, TFCAS, or TCAR patients were used. We used inverse probability of treatme…

Comparing Cancer Incidence in an Observational Cohort of Medicaid Beneficiaries With and Without HIV, 2001-2015.

BACKGROUND: Life expectancy among people with HIV (PWH) is increasing, making chronic conditions-including cancer-increasingly relevant. Among PWH, cancer burden has shifted from AIDS-defining cancers (ADCs) toward non-AIDS-defining cancers (NADCs). SETTING: We described incidence of cancer in a claims-based cohort of Medicaid beneficiaries. We included 43,426,043 Medicaid beneficiaries (180,058 with HIV) from 14 US states, aged 18-64, with >6 months of enrollment (with no dual enrollment in another insurance) and no evidence of a prveious cancer. METHODS: We estimated cumulative incidence o…

Implementation of an office-based addiction treatment model for Medicaid enrollees: A mixed methods study.

INTRODUCTION: Medications for opioid use disorder (MOUD) are the most effective treatment for opioid use disorder (OUD) but remain underutilized. To reduce barriers to MOUD prescribing and increase treatment access, New Jersey's Medicaid program implemented the Office-Based Addiction Treatment (OBAT) Program in 2019, which increased reimbursement for office-based buprenorphine prescribing and established newly reimbursable patient navigation services in OBAT clinics. Using a mixed-methods design, this study aimed to describe stakeholder experiences with the OBAT program and to assess implement…

Pregnancy-related thromboembolism in women with sickle cell disease: An analysis of National Medicaid Data.

Pregnancy and sickle cell disease (SCD) both individually carry a risk of thromboembolism (TE). Pregnancy in people with SCD may further enhance the prothrombotic effect of the underlying disease. The objectives of this study were to determine the rate and risk factors for arterial and venous thrombosis in pregnant people with SCD. Administrative claims data from the United States Centers for Medicare and Medicaid Service Analytic eXtract from 2006 to 2018 were used. The study population included people with SCD from the start of their first identified pregnancy until 1 year postpartum and a c…

Is the Affordable Care Act Medicaid Expansion Associated With Reported Incidents of Child Sexual Abuse?

There is substantial evidence that adequate access to healthcare among low-income adults through the Affordable Care Act Medicaid expansion mitigates risk factors associated with childhood maltreatment, including parental financial insecurity, substance use, and poor mental health. Indeed, studies identified reduced reports of child neglect in states that expanded Medicaid, relative to those that did not. However, it is unknown whether Medicaid expansion is associated with reported child sexual abuse (CSA). We present findings from a study evaluating the association of Medicaid expansion with …

Changes in Medicaid Utilization and Spending Associated with Homeless Adults' Entry into Permanent Supportive Housing.

BACKGROUND: There is growing interest in financing housing and supportive services for homeless individuals through Medicaid. Permanent Supportive Housing (PSH), which integrates non-time-limited housing with supportive services for people who are disabled and chronically homeless, has seen rapid growth in the last decade, but clear evidence on the long-term impacts of PSH, needed to guide state efforts to finance some PSH services through Medicaid, is lacking. OBJECTIVE: Assess changes in Medicaid expenditures and utilization associated with receiving PSH. DESIGN: Cohort study using a diffe…
