Disparities in gastrointestinal (GI) care in the United States represent a major health burden, with GI diseases accounting for approximately $135 billion annually in direct medical expenditures. A pronounced geographic disparity exists, with over two-thirds of the 3,149 U.S. counties lacking any GI specialist. This results in an estimated 49-50 million Americans required to travel more than 25 miles for specialty care. For instance, states like Wyoming and Arizona report over 75% of residents are more than 25 miles from GI care, while states such as Alaska and North Dakota average only 1.8 ga

Introduction Coronary heart disease (CHD) remains a leading cause of mortality in the United States. While traditional risk factors are well established, the structural impact of financial toxicity, defined as the inability to afford medical care, on cardiovascular outcomes is undercharacterized in nationally representative populations. Methods A dual-level analysis was conducted. At the individual level, a pooled cross-sectional analysis using 2022-2024 Behavioral Risk Factor Surveillance System (BRFSS) data (N=1,336,111) was performed to calculate prevalence ORs. At the macro level, a Baye

INTRODUCTION: In the US, cardiovascular diseases (CVD) are the leading cause of death and disability. Cost-related medication non-adherence (CRMN) can have serious consequences and worsen CVD outcomes. We examined the relationship between CVD risk factors and CRMN among US adults.
METHODS: CDC's 2019-2021 National Health Interview Survey (NHIS) data were used to examine CRMN among adults, categorized into three groups based on reported risk factors. We used chi-square tests, and logistic regression to determine factors associated with CRMN.
RESULTS: Among 49,464 participants, young, unmarrie

Alabama has the highest cervical cancer mortality and sixth highest incidence rates nationwide. We assessed the acceptability and feasibility of at-home HPV self-testing for cervical cancer screening in Alabama. Eligible women (ages 30-64y, no Pap last 3.5y, not being pregnant, no hysterectomy history, and satisfying either rural residency or being African American) were recruited by community health workers (CHW), foodbanks, churches, and trusted relationships in targeted communities. Participants received a mailed package including an Evalyn® HPV self-test kit (for analysis against high-risk

OBJECTIVES: Antiretroviral therapy is critical for preventing perinatal HIV transmission during pregnancy; however, access is shaped by complex structural factors, including access to prenatal care, insurance status and provincial drug-coverage policies. Despite its importance, little is known about how out-of-pocket antiretroviral therapy costs during pregnancy differ within and across Canada.
METHODS: We performed a secondary analysis of Quebec data from the Canadian Perinatal HIV Surveillance Program collected between 2019 and 2023, which included 225 pregnant women and pregnant people liv

BACKGROUND: Prior work has shown that Medicaid coverage offset reductions in employer-sponsored insurance after COVID-19-related job loss in Medicaid expansion states. However, the effect of Medicaid expansion on health care access is not fully understood.
OBJECTIVE: To estimate the association of unemployment during COVID-19 with health insurance coverage and health care access in Medicaid expansion versus nonexpansion states.
STUDY DESIGN: We used restricted, longitudinal National Health Interview Survey (NHIS) data from 2019 to 2020, focusing on working-age adults (N=5156). Using triple-d

INTRODUCTION: Prenatal oral care is a huge concern due to hormonal changes women undergo during pregnancy, during which pregnant women become susceptible to many oral diseases, such as periodontal disease. Disparities in oral health often affect women who fall into these categories: low-income, uninsured, and minority ethnic/racial groups. Having limited knowledge and a lack of access are significant contributing factors to the adverse effects of pregnancies. This qualitative phenomenological study aims to understand the perspectives of pregnant Black women and dental health care providers wit

INTRODUCTION.—: Prostate cancer is one of the most common cancers, with an approximate global annual incidence of 1.5 million men and nearly 400,000 deaths. Treatment options include surgical resection, radiotherapy, and high-intensity focused ultrasounds, often combined with hormone deprivation therapy. Insurability of prostate cancer patients is evaluated based on tumor severity using the AJCC and TNM classifications, along with PSA levels and Gleason Scores. To control the risk, life insurers use medical selection, albeit many individuals may be excluded from coverage. Regulations, such as

OBJECTIVE: Threat perceptions during evaluation for acute coronary syndrome (ACS) in the emergency department (ED) predict posttraumatic stress symptoms (PSS). It is unknown how health insurance status affects threat perceptions. We tested whether lacking health insurance is associated with higher threat perceptions and PSS in patients with suspected ACS in the ED and whether threat perceptions mediate associations between lack of health insurance and subsequent PSS.
METHOD: Patients in the Columbia University Irving Medical Center ED with suspected ACS enrolled in an observational cohort stu

INTRODUCTION: Medication-assisted treatment (MAT) for opioid use disorder (OUD) is underused in primary care. Little is known about patient demographics associated with MAT initiation, particularly among models with an interdisciplinary approach, including behavioral health integration. We hypothesize few disparities in MAT initiation by patient characteristics after implementing this model for OUD.
METHODS: Electronic health record data were used to identify adults with ≥1 primary care visit in 1 of 2 study clinics in a Pacific Northwest academic health system between September 1, 2015 and A

BACKGROUND: African Americans continue to suffer disproportionately from cancer morbidity and mortality, with emerging evidence suggesting potential quality of life (QOL) disparities in the survivorship period.
OBJECTIVE: The objective of the study was to assess sociodemographic, clinical, and psychosocial factors associated with physical and mental health QOL (PHQOL and MHQOL) among African American and white cancer survivors.
METHODS: Patients were recruited from tumor registries. Telephone interviews were conducted with 248 African American and 244 white respondents with a history of brea

BACKGROUND: Health reform, including Medicaid expansion, is increasing insurance coverage and financial access to breast and cervical cancer screening for low-income women, although services for low-income uninsured women are still needed.
METHODS: American Community Survey and administrative data about Medicaid and health insurance enrollment are used to estimate the number of low-income women who will be uninsured in 2017, focusing on the age ranges 21 to 64, 40 to 64, and 50 to 64.
RESULTS: Assuming that 29 states expand Medicaid (as of June 2015), the national percentage of low-income wo

Differences in implantable cardioverter defibrillator (ICD) utilization based on insurance status have been described, but little is known about postimplant follow-up patterns associated with insurance status and outcomes. We collected demographic, clinical, and device data from 119 consecutive patients presenting with ICD shocks. Insurance status was classified as uninsured/Medicaid (uninsured) or private/Health Maintenance Organization /Medicare (insured). Shock frequencies were analyzed before and after a uniform follow-up pattern was implemented regardless of insurance profile. Uninsured p

INTRODUCTION: Smoking is a public health risk; the prevalence of smoking among adults in Nebraska is 18.4%. Studies indicate that maltreatment of children alters their brain development, possibly increasing risk for tobacco use. Previous studies have documented associations between childhood maltreatment and adult health behaviors, demonstrating the influence of adverse experiences on tobacco use. We examined prevalence and associations between adverse childhood experiences and smoking among Nebraskans.
METHODS: We analyzed 2011 Nebraska Behavioral Risk Factor Surveillance System (Adverse Chi

The Breast and Cervical Cancer Mortality Prevention Act of 1990 authorized CDC to establish the National Breast and Cervical Cancer Early Detection Program (NBCCEDP) to increase screening services for women at low income levels who are uninsured or underinsured. Although the NBCCEDP covers most diagnostic services that women need after receiving an abnormal mammography or Papanicolaou (Pap) test result, the program does not reimburse for breast biopsies. In addition, the Act prohibits the use of NBCCEDP funds for cancer treatment. Participating health agencies must ensure that NBCCEDP clients

INTRODUCTION: Despite sustained efforts to reduce opioid-related overdose fatalities, rates have continued to rise. In many areas, overdose response involves emergency medical service (EMS) personnel administering naloxone and transporting patients to the emergency department (ED). However, a substantial number of patients decline transport, and many EDs do not provide medication for opioid use disorder (MOUD). One approach to filling this gap involves delivering MOUD to overdose patients in the field with trained post-overdose EMS teams who can initiate buprenorphine. In this MOUD field initi
