Predictors of Postdeployment Prescription Opioid Receipt and Long-term Prescription Opioid Utilization Among Army Active Duty Soldiers.

INTRODUCTION: Little is known about long-term prescription opioid utilization in the Military Health System. The objectives of this study were to examine predictors of any prescription opioid receipt, and predictors of long-term opioid utilization among active duty soldiers in the year following deployment. MATERIALS AND METHODS: The analytic sample consisted of Army active duty soldiers returning from deployment to Operation Enduring Freedom, Operation Iraqi Freedom, or Operation New Dawn in fiscal years 2008-2014 (N = 540,738). The Heckman probit procedure was used to jointly examine predic

Longer-term neurological and endocrine/metabolic conditions in the deepwater horizon oil spill coast guard cohort study: A principal component analysis.

PURPOSE: Deepwater Horizon oil spill (DWHOS) responders potentially experienced multiple stressors. We examined associations between DWHOS deployment-related stressor patterns and neurological and endocrine/metabolic conditions among U.S. Coast Guard active duty responders who completed post-deployment surveys (n = 3102). METHODS: Deployment-related stressor patterns were identified using principal component analysis of self-reported stressors. Neurological and endocrine/metabolic outcomes were classified from military health encounter records. Associations between principal components (PCs)

Post-traumatic growth amongst UK armed forces personnel who were deployed to Afghanistan and the role of combat injury, mental health and pain: the ADVANCE cohort study.

BACKGROUND: Post-traumatic growth (PTG) is a positive psychological consequence of trauma. The aims of this study were to investigate whether combat injury was associated with deployment-related PTG in a cohort of UK military personnel who were deployed to Afghanistan, and whether post-traumatic stress disorder (PTSD), depression and pain mediate this relationship. METHODS: 521 physically injured (n = 138 amputation; n = 383 non-amputation injury) and 514 frequency-matched uninjured personnel completed questionnaires including the deployment-related Post-Traumatic Growth Inventory (DPTGI). Healthcare utilization and readiness outcomes among soldiers with post-deployment at-risk drinking, by multimorbidity class.

Although alcohol use disorder (AUD) regularly co-occurs with other conditions, there has not been investigation of specific multimorbidity classes among military members with at-risk alcohol use. We used latent class analysis (LCA) to cluster 138,929 soldiers with post-deployment at-risk drinking based on their co-occurring psychological and physical health conditions and indicators of alcohol severity. We examined the association of these multimorbidity classes with healthcare utilization and military readiness outcomes. Latent class analysis was conducted on 31 dichotomous indicators capturi

Weight-Based Teasing and Metabolic Syndrome Components among Adolescent Military Dependents at Risk for Adult Obesity.

Background: Among adults, weight stigma is associated with markers of poor cardiometabolic health. Although weight-based teasing (WBT) is common among youth with high body weight, few studies have examined its associations with cardiometabolic markers. Owing to unique stressors (e.g., parental deployment and frequent moves), military-dependent youth may be at particularly high risk for obesity, WBT, and poor cardiometabolic health. We, therefore, assessed associations between WBT and cardiometabolic health markers among adolescent military dependents presenting for a weight gain prevention tri

Pregnancy and mental health among women veterans returning from Iraq and Afghanistan.

BACKGROUND: Veterans of Operations Enduring Freedom and Iraqi Freedom (OEF/OIF) may experience significant stress during military service that can have lingering effects. Little is known about mental health problems or treatment among pregnant OEF/OIF women veterans. The aim of this study was to determine the prevalence of mental health problems among veterans who received pregnancy-related care in the Veterans Health Administration (VHA) system. METHODS: Data from the Defense Manpower Data Center (DMDC) deployment roster of military discharges from October 1, 2001, through April 30, 2008, we

War on Two Fronts: The Fight against Parasites in Korea and Vietnam.

The Vietnam War has long been regarded as pivotal in the history of the Republic of Korea, although its involvement in this conflict remains controversial. While most scholarship has focused on the political and economic ramifications of the war - and allegations of brutality by Korean troops - few scholars have considered the impact of the conflict upon medicine and public health. This article argues that the war had a transformative impact on medical careers and public health in Korea, and that this can be most clearly seen in efforts to control parasitic diseases. These diseases were a majo

Mind body medicine in the care of a U.S. Marine with chronic pain: a case report.

Many service members suffer from chronic pain that can be difficult to adequately treat. Frustration has led to more openness among service members to complementary and alternative medicine modalities. This report follows JK, a Marine with chronic pain related to an injury while on combat deployment through participation in a 6-week self-care-based Mind Body Medicine program and for 7 months after completion of the program. JK developed and sustained a regular meditation practice throughout the follow-up period. JK showed a noticeable reduction in perceived disability and improvements in psych

Genital mutilation of young girls traditionally practiced in militarily significant regions of the world.

Very few physicians practicing in the United States have experience in treating female patients who have undergone mutilation of the external genitalia, incorrectly termed female circumcision. This procedure, known as infibulation, consists of removing the clitoris, prepuce, and portions of the labia of young girls, usually younger than 7 years of age. Infibulation has been practiced by lay midwives for centuries in the Horn of Africa and in other African and Middle Eastern countries. This paper discusses infibulation, the techniques, and the recommended medical and obstetric management of pat

The Impact of Military Tobacco Control Policy: A Systematic Review.

INTRODUCTION: Tobacco use is synonymous with the military. Despite that military personnel are trained to follow commands, opportunities exist to implement various tobacco control strategies. We conducted a systematic review to evaluate the impact of tobacco control policy employed in military settings. MATERIALS AND METHODS: We searched for published English articles in Medline, Web of Science, Scopus, and Google Scholar databases using relevant subject headings without year restriction. We included randomized controlled trials, nonrandomized controlled trials, case-control, cohort, controll

Associations of polygenic risk scores with posttraumatic stress symptom trajectories following combat deployment.

BACKGROUND: Identification of genetic risk factors may inform the prevention and treatment of posttraumatic stress disorder (PTSD). This study evaluates the associations of polygenic risk scores (PRS) with patterns of posttraumatic stress symptoms following combat deployment. METHOD: US Army soldiers of European ancestry (n = 4900) provided genomic data and ratings of posttraumatic stress symptoms before and after deployment to Afghanistan in 2012. Latent growth mixture modeling was used to model posttraumatic stress symptom trajectories among participants who provided post-deployment data (n

Leveraging Military-Civilian Partnerships in Crisis: A Case Study of the Las Vegas Response to COVID-19.

The COVID-19 pandemic exposed critical vulnerabilities in civilian healthcare surge capacity, highlighting the potential for military-civilian partnerships (MCPs) to augment public health emergency responses. The Las Vegas Military-Civilian Partnership (LV MCP), a unique collaboration, offers a case study of such integration. The objective of this study was to evaluate the impact of the LV MCP, specifically the workload of US Air Force (USAF) pulmonary and critical care medicine (PCCM) physicians, on the Las Vegas civilian healthcare system during the COVID-19 pandemic. This retrospective revi

Rodent and Ectoparasite-Borne Disease Surveillance Support During the Balikatan Exercise in 2024 and 2025.

Zoonotic diseases are a primary threat to military readiness during deployments in the Asia-Pacific region. To characterize these risks, we conducted a multi-site, multi-year 'One Health' biosurveillance study during the Balikatan (BK) 2024 and 2025 exercises in the Philippines, sampling rodents, ectoparasites, and the environment at four key training locations on Luzon Island. A comprehensive biosurveillance study at four BK training sites revealed distinct, evolving, and site-specific zoonotic disease risks that require tailored health protection strategies. The risk is most acute at Clark A
