Gliadin-specific IgA antibodies in breast milk.

Breast-feeding helps to protect against coeliac disease because of the presence of antigliadin-IgA antibodies (AGA-IgA) in breast milk. The aim of this study was to assess the concentrations of AGA-IgA in breast milk during lactation, and whether these concentrations vary with the socioeconomic status of the women. Samples of serum for determination of IgA albumin and AGA-IgA, and samples of breast-milk for AGA-IgA were collected from 105 healthy mothers (aged 17 - 36 years). Women were divided into two groups: group 1 were of low and group 2 were of high socioeconomic status. No differences w…

Acute gouty synovitis associated with "urate milk".

OBJECTIVE: To analyze the clinical features of acute gouty synovitis associated with thick, milky white, "chalky," urate laden synovial effusions, and to investigate the effects on synovial white blood cell (WBC) counts when leukocyte-rich rheumatoid effusions are incubated with a urate packed milky synovial fluid. METHODS: Five patients (all men, mean age 70.8 years) with acute gouty synovitis (acute arthritis in 3, acute bursitis in 2) associated with "urate milk" were studied between 1993 and 1996. RESULTS: Synovial effusions were thick, "chalky," and appeared "milky" white. The fluids we…

90K (Mac-2 BP) in human milk.

Human 90K, also known as Mac-2 binding protein (Mac-2 BP), is a secreted glycoprotein which is widely expressed, binds to the human macrophage-associated lectin Mac-2, and may have a role in host defence. We have measured the concentrations of 90K in human breast milk from eight healthy mothers delivering mature healthy infants after full-term pregnancy using a specific immunoenzymatic assay. Maximal 90K concentrations were observed on days 2-3 post-partum, and ranged from 13.4 to 79.2 microg/ml. The concentrations of 90K in the milk had no correlation with those in the maternal blood.

Exposure to toxic elements via breast milk.

Breast milk is the ideal nutrient for the newborn, but unfortunately also a route of excretion for some toxic substances. Very little attention has been paid to breast milk as a source of exposure to toxic elements. The dose-dependent excretion is breast milk and the uptake in the neonate of inorganic mercury, methylmercury and lead were studied in an experimental model for rats and mice. The transfer of mercury from plasma to milk was found to be higher in dams exposed to inorganic mercury than to methylmercury. In contrast, the uptake of mercury from milk was higher in the sucklings of dams …

Breast feeding: overview and breast milk immunology.

The transfer of host defence capacity to the human offspring provides a remarkable model of passive transfer of immunity. In fact it may als … as 0.5-1 g of secretory immunoglobulin A (SIgA) antibodies daily, the predominant antibody of human milk. This can be compared to the production of some 2.5 g of SIgA per day for a 60 kg adult. These SIgA antibodies have been shown to protect against Vibrio cholerae, ETEC, Campylobacter, Shigella and Giardia. Furthermore, milk is rich in receptor analo …

Milk of calcium pleural collections: CT findings.

OBJECTIVE: Chest radiography and CT findings of five patients with milk of calcium pleural collections are described. MATERIALS AND METHODS: The diagnosis was based on needle aspiration in four patients and the presence of a fat-calcium level on CT in the remaining one. Four patients had a prior history of pleurisy. RESULTS: Characteristic radiographic findings were homogeneous increased opacity with a double contour at the interface with the lung. Computed tomography showed that the lateral contour was the margin of the milk of calcium and the medial contour was the margin of the thickened …

Milk-induced malabsorption in malnourished African patients.

Fifty malnourished rural African patients were randomly assigned to whole milk (50 g lactose/L), acidified milk (24 g lactose/L), or a commercial lactose-free diet (LFD) as a constant nasogastric infusion for 3 d, starting at 2 L/d and increasing to 3 L/d if tolerated. During the first 2 d mild symptoms of intolerance developed in 63% of patients on whole milk, 37% on acidified milk, and 54% on LFD whereas severe intolerance, necessitating withdrawal, was encountered in 22% receiving whole milk and none receiving LFD. Stool weights and fat excretion on day 3 were greater (P less than 0.02) in …

Milk protection against experimental ulcerogenesis in rats.

The antiulcer activity in pasteurized/homogenized bovine milk and a lipid extract of this milk was tested in an attempt to isolate and identify the active component. Using 0.6 N HCl as a damaging agent in pylorus-ligated rats, the protective property of milk appeared to be related to its phospholipid content and not its protein constituents. With intact (non-pylorus-ligated) rats, milk had demonstrable protective activity against 0.6 N HCl, as well as 100% ethanol and 160 mM taurocholic acid. The increasing phospholipid concentrations in skim, whole, and buttermilk paralleled their antiulcer e…

Milk intolerance and microbe-containing dairy foods.

The relationship between primary lactase deficiency, the amount of lactose in the diet, and symptoms of intolerance continues to be debated. … se loads of up to 12 g are fed, symptoms can be minimal or absent. Tolerance to yogurt, acidophilus milk, and other microbe-containing dairy foods has been suggested and is thought to be due to either a low lactose content or in vivo autodigestion by microbial beta-galactosidase. Up to 20 g of lactose in yogurt is tolerated well by lactase-deficient pe …

Isolation of Listeria monocytogenes from raw milk.

During a recent outbreak of listeriosis, we examined 121 raw milk samples and 14 milk socks (filters). Listeria monocytogenes was recovered from 15 (12%) of 121 milk specimens and 2 (14%) of 14 milk socks. The optimal processing method consisted of cold enriching diluted milk for 1 month with culture to selective broth, followed by plating.

Congenital heart disease and cow's milk intolerance.

Four infants with congenital heart disease showed in their first months of life poor weight gain in spite of the absence of cardiac failure; a diagnosis of intolerance to cow's milk proteins was made on the basis of laboratory findings and subsequently confirmed by the success of the avoidance diet and the positivity of the challenge tests; all four children had previously received nasogastric feeding, and this may represent a favouring factor for sensitization. Intolerance to cow's milk proteins should be considered as a possible cofactor for failure to thrive in infants with congenital heart…

Lactose-poor milk in adult lactose intolerance.

The frequency of lactose intolerance was studied in patients with chronic gastrointestinal disease, mainly peptic ulcer to explore the possibilities of the use of lactose-poor milk. It was found that whole milk caused dyspeptic symptoms in 45% of patients with peptic ulcer; lactose-intolerance was present in 82% of the patients with milk intolerance; lactose poor powdered milk resulted in complete freedom from symptoms. Use of milk with reduced lactose contents is recommended in the diet of adult patients with lactase deficiency.

How much breast milk do babies need?

A reassessment of the breast milk requirements of young babies at different ages has been made from a statistical analysis of energy intake data collected prospectively in Cambridge, U.K. and from data derived from a literature survey of cross-sectional food energy intakes of infants from Canada, Sweden, the U.K. and USA. In contrast to the slow progressive linear decline in energy requirements per kg body-weight that has previously been assumed, measured intakes have been shown to fall more rapidly in the first six months of life. Thereafter they then rise again as the baby becomes more activ…
