From the hospital to the community: reshaping the palliative care paradigm.

Palliative care (PC) has traditionally developed within hospital settings, shaped by biomedical models, specialist hierarchies, and episodic care delivery. However, demographic change, rising multimorbidity, patient preferences, and health system constraints increasingly challenge hospital-centric approaches. In contrast, improving the community palliative care model requires moving beyond the simple decentralization of services toward a more integrated, person-centered, and ethically grounded system of care. At its core, an effective community model must recognize the individuality of people

Development and clinical implementation of a care guideline to support self-management and responsibility transition in adolescents and young adults with biliary atresia.

OBJECTIVE: This study developed, refined, and implemented a care guideline for adolescents and young adults (AYAs) with biliary atresia surviving with their native livers and their parents, based on a responsibility transition process model. We evaluated its clinical usefulness and identified challenges related to sustainable implementation. This study extends a previously developed responsibility transition model by translating it into a clinically implemented guideline and examining its real-world applicability. METHODS: An implementation-focused, multiphase mixed-methods design was used. P

Physical Activity Interventions for Rural Older Adults Through a Social Ecological Model Lens: A Systematic Review.

BACKGROUND: Rural older adults face unique barriers to physical activity (PA), including geographic isolation, limited resources, and social constraints. Despite the well-documented benefits of PA, rural older adults are less likely to meet PA guidelines than their urban counterparts. This systematic review evaluates the effectiveness, implementation, and sustainability of PA interventions for rural older adults through the lens of the social ecological model. METHODS: A comprehensive literature search was conducted in PsycINFO, PubMed, and Web of Science, identifying 1,023 articles. Eleven s

Job embeddedness among young shift-working nurses at a newly established hospital in South Korea: Associations with emotional intelligence, emotional leadership, and work-life balance.

PURPOSE: Newly established hospitals often face difficulty retaining young nurses due to organizational instability; however, traditional compensation-based strategies may be insufficient. This study examined the relative contribution of person-environment fit factors-emotional intelligence, emotional leadership, and work-life balance-to job embeddedness among young shift-working nurses in a newly established hospital. METHODS: This cross-sectional descriptive correlational study included 132 shift-working nurses younger than 30 years who were employed at a newly established university hospit

Social control and workplace drinking norms: a comparison of two organizational cultures.

OBJECTIVE: This article reports on an investigation of the relationship of social control mechanisms at work to drinking practices of 10,000 salaried and hourly employees working in the same U.S. industry, with the same union, but in two different work environments. One work environment reflected an organizational culture that is traditional to U.S. management; the other was based on a nontraditional Japanese transplant model. METHOD: The research team used a combination of methods including in-home surveys (N = 1,723; 1,378 men) and ethnography (110 semistructured interviews and 200 hours of

Results of a job analysis of psychologists working in state hospitals.

The graduate training of psychologists should prepare them for central roles in state hospitals, especially roles involving direct patient care and supervision of colleagues. To what extent do actual job practices match graduate preparation? To find out, 55 psychologists from 12 state hospitals in the Midwest were surveyed regarding their educational backgrounds and time currently spent in various job activities. Results indicated that paperwork consumes more time than any other activity, followed by therapeutic services, psychological assessment, supervision, and case management. Almost 50% o

New graduate nurses' experiences of bullying and burnout in hospital settings.

AIM: This paper is a report of a study conducted to test a model linking new graduate nurses' perceptions of structural empowerment to their experiences of workplace bullying and burnout in Canadian hospital work settings using Kanter's work empowerment theory. BACKGROUND: There are numerous anecdotal reports of bullying of new graduates in healthcare settings, which is linked to serious health effects and negative organizational effects. METHODS: We tested the model using data from the first wave of a 2009 longitudinal study of 415 newly graduated nurses (<3 years of experience) in acute ca

Productivity and employees' organizational justice perceptions in long-term care for the elderly.

We examined the associations between productivity, employer characteristics, and context variables, and the organizational justice perceptions of 330 female employees in long-term institutional elderly people care. The productivity measure used was the proportion of the inpatient days to total costs. Employees working in high productivity units experienced higher procedural justice than those working in low productivity units. Hostile employees experienced both the procedures and management as less fair than non-hostile employees. Unit size and resident turnover were negatively and registered

Predicting lymphatic filariasis transmission and elimination dynamics using a multi-model ensemble framework.

Mathematical models of parasite transmission provide powerful tools for assessing the impacts of interventions. Owing to complexity and uncertainty, no single model may capture all features of transmission and elimination dynamics. Multi-model ensemble modelling offers a framework to help overcome biases of single models. We report on the development of a first multi-model ensemble of three lymphatic filariasis (LF) models (EPIFIL, LYMFASIM, and TRANSFIL), and evaluate its predictive performance in comparison with that of the constituents using calibration and validation data from three case s

Correctional-staff wellness in a statewide jail system: health appraisals, occupational strain and priorities for organizational support.

PURPOSE: Although substantial research has examined correctional officer health, fewer studies have focused on jail staff, and even fewer have done so within a unified, statewide jail system. This study aims to address this gap by examining staff perceptions of health and wellness across Tennessee's county jails, where centralized oversight enables system level insight. DESIGN/METHODOLOGY/APPROACH: Open-ended questionnaire data from 412 correctional personnel were analyzed following survey distribution to staff across all 120 Tennessee county jails. While jails are typically decentralized, Te

Evidence-based construction and validation of an agent-based model of community integrated care for older adults living with dementia.

OBJECTIVE: Community dementia care involves interacting patients, caregivers, and formal care providers whose long-term dynamics are difficult to characterize with single-component empirical studies. We constructed and assessed a baseline agent-based model (ABM) to establish a transparent, inspectable reference scenario against which future computational experiments on organizational configurations can be evaluated. METHODS: A fully rule-based ABM of community dementia care was developed using prior empirical studies, published literature, policy documents, and structured expert consultation.

A Social-Ecological Model Exploring Gestational Diabetes Mellitus Screening Practices Among Antenatal Health Care Providers.

Gestational diabetes mellitus (GDM) is associated with adverse health outcomes for the pregnant individual and their baby. Screening approaches for GDM have undergone several iterations, introducing variability in practice among healthcare providers. As such, our study aimed to explore the views of antenatal providers regarding their practices of, and counseling experiences on the topic of, GDM screening in Ontario. We conducted a qualitative, grounded theory study. The study population included antenatal providers (midwives, family physicians, and obstetricians) practicing in Hamilton, Ottawa

Who mimics nature? Disciplinary asymmetry in biomimetic and bioinspired research teams.

Biomimetics is generally viewed as inherently interdisciplinary, yet the disciplinary composition of biomimetic research teams has not been systematically quantified. We analysed 74 407 biomimetics-related publications from the past two decades and classified 436 010 author-paper affiliation records by biological or technical expertise to reconstruct team composition. Our analysis reveals an asymmetric disciplinary landscape: technical authors outnumber biological researchers by roughly two to one. Team organization is more balanced-while purely technical teams still represent the largest shar

ADMET all-inclusive: Accountability for the full end-to-end model life cycle is key to success.

Drug discovery requires balancing efficacy with pharmacokinetic and safety properties. Although machine learning models for ADMET (absorption, distribution, metabolism, excretion and toxicology) prediction are increasingly sophisticated, their impact is often limited by challenges in deployment, user adoption, interpretability and project-specific applicability. In this perspective, we describe Bayer's predicTeam approach, which assumes end-to-end responsibility for the prediction lifecycle, from endpoint selection and data curation to deployment, user training and performance monitoring. Key

[Work rehabilitation in people with severe mental illnesses].

Work and employment are basic human rights. Work therapy has a long-standing tradition in mental health care. Integrating people with severe mental illness into jobs has always been considered indispensable. Germany has a good legal framework for work rehabilitation and there is solid evidence of effectiveness for a range of interventions. International models of work integration have not been implemented to a sufficient degree and this may be one of the reasons for limited success in work rehabilitation for people with mental illnesses. The need for work rehabilitation measures and the availa

An intersectionality-based policy analysis framework: critical reflections on a methodology for advancing equity.

INTRODUCTION: In the field of health, numerous frameworks have emerged that advance understandings of the differential impacts of health policies to produce inclusive and socially just health outcomes. In this paper, we present the development of an important contribution to these efforts - an Intersectionality-Based Policy Analysis (IBPA) Framework. METHODS: Developed over the course of two years in consultation with key stakeholders and drawing on best and promising practices of other equity-informed approaches, this participatory and iterative IBPA Framework provides guidance and direction

Make the most of what you have! Exploring circularity practices adoption in olive milling through the norm activation model.

The transition toward a circular economy (CE) in agriculture relies not only on technological progress but also on the behavioural readiness of millers to embrace sustainable practices. This study employs the Norm Activation Model (NAM) to examine how psychological and structural factors shape CE practices adoption among olive oil mill business owners in Sicily. Drawing on survey data, we conducted a Partial Least Squares Structural Equation Modeling (PLS-SEM) analysis using actual behaviour as dependent variable. The results indicate that Awareness of Consequences and Ascription of Responsibi

Understanding the Link Between Toxic Leadership and Employee Well-being: The Interplay of Employee Turnover, Organizational commitment, Shift timing & Industry Sector.

Background This study deals with the issue of toxic leadership and employees' well-being, its effect on employees' turnover among various industry sectors in the United Arab Emirates (UAE) and to gauge the moderating effect of organizational commitment, shift timing, and industry sector. Toxic leadership was consistently associated with adverse consequences for the employee but few studies have explored the relationship between both in a cross-sector, UAE-based context. Based on Social Exchange Theory, Psychological Contract Theory, and Conservation of Resources Theory, it elaborates an empiri
