Monte Carlo-based estimation of NIR-PDT efficacy with and without breast compression in early-stage breast cancer.

INTRODUCTION: Near-infrared photodynamic therapy (NIR-PDT) for early-stage breast cancer has attracted attention and it suggests that the depth of the tumor from the skin surface significantly affects treatment efficacy, with shallower tumors responding more effectively to therapy. Therefore, compressing the breast, as is done during mammography, may improve therapeutic outcomes by reducing tumor depth. In this study, we investigated the effectiveness of breast compression in NIR-PDT for early-stage breast cancer using Monte Carlo simulations. METHODS: We evaluated the impact of breast compre

Identification and assessment of urban groundwater pollution based on Monte Carlo health risk modeling and self-organizing map (SOM).

With the acceleration of urbanization, groundwater quality is severely threatened, posing health risks to public health and the environment. In reality, the highly heterogeneous geological background and hydrochemical relationships not only increase the difficulty in identifying pollution, but also pose significant challenges in groundwater risk prevention and control. Taking Guangzhou, a megacity in the bustling Pearl River Delta, as the research object, this study leverages the ability of machine learning to mine complex data and the advantage of Monte Carlo simulation in quantifying random

"Cake causes herpes?" - promiscuous dichotomisation induces false positives.

BACKGROUND: Continuous biomedical data is often dichotomized into two or more groups for analysis, despite long-standing warnings from statisticians that this constitutes bad practice. This dichotomisation is typically discouraged because it reduces statistical power and may obscure important trends. This paper considers another reason to discourage this practice: that dichotomisation is a powerful tool to manipulate data, as dichotomising at an arbitrary yet flexible threshold (which we term 'promiscuous dichotomisation') represents a powerful researcher degree of freedom. METHODS: The motiv

Identifying Sources of Error in Computer-Navigated Total Knee Arthroplasty Using Sensitivity Analyses in Knee Models.

BACKGROUND: Long-term studies comparing computer-assisted total knee arthroplasty (TKA) to conventional techniques have not consistently demonstrated differences in outcomes. Errors may occur at various points of a navigated TKA, including surgeon variability in registering landmarks and when placing the cutting guide. The purpose of this study was to identify the sources of error during navigated TKA and to quantify their impact at each critical step of the procedure. METHODS: Sawbones (n = 6) and three-dimensional-printed (n = 4) knee models were stabilized on a custom wooden platform. Land

K-Edge-Inspired Selenomelanin Hydrogel: An X-Ray Filtering Biomaterial for Skin Protection in Radiotherapy.

Radiotherapy is widely employed in cancer treatment, yet its inevitable side effects, particularly radiation dermatitis (RD), remain a significant concern. Numerous studies focus on alleviating RD symptoms, but approaches to minimize the radiation dose to the skin are underexplored. Shielding skin-damaging photons (10-20 keV) holds promise for skin dose reduction; however, this strategy is hindered by inadequate shielding efficiency of current RD biomaterials and overshielding by conventional high-Z materials which attenuate therapeutic high-energy photons. Here, inspired by the strong absorpt

A simulation study on the effect of penetration of gold nanoparticles in the cytoplasm of healthy eye organs on dose enhancement of brachytherapy.

PURPOSE: Special properties and recent advances in the synthesis and biomolecular functionalization of gold nanoparticles (GNPs) have led to the evolution of their use in biomedical applications such as photon radiotherapy. Simulation-based studies on the effect of various parameters that govern the dose enhancement due to utilizing GNPs have facilitated the progress of knowledge in this field. Due to their flexibility and easier accessibility compared with experimental works, simulations have the potential to be considered for pre-clinical tests and, therefore, should be close to the realisti

MVMRmode: Introducing an R package for plurality valid estimators for multivariable Mendelian randomisation.

BACKGROUND: Mendelian randomisation (MR) is the use of genetic variants as instrumental variables. Mode-based estimators (MBE) are one of the most popular types of estimators used in univariable-MR studies and is often used as a sensitivity analysis for pleiotropy. However, because there are no plurality valid regression estimators, modal estimators for multivariable-MR have been under-explored. METHODS: We use the residual framework for multivariable-MR to introduce two multivariable modal estimators: multivariable-MBE, which uses IVW to create residuals fed into a traditional plurality vali

Monte Carlo implementation of a Gaussian plume model for submersion dose calculation at short downwind distances.

In this article, the submersion dose due to a radioactive cloud of pollutants was evaluated at short downwind distances from an emission stack. The atmospheric transport of contaminants was modelled using the Gaussian plume model (GPM). The algorithm for dose computation and its hypotheses were analysed. Two relevant issues were discussed: the semi-infinite cloud approximation used for pre-calculated dose conversion factors and the lack of a radiation transport model for dose computation outside the radioactive cloud. The GPM-based software HotSpot and GENII V2.10 and a FLUKA Monte Carlo GPM i

Dosimetric feasibility study ("proof of concept") of refractory ventricular tachycardia radioablation using proton minibeams.

PURPOSE: Preclinical data demonstrated that the use of proton minibeam radiotherapy reduces the risk of toxicity in healthy tissue. Ventricular tachycardia radioablation is an area under clinical investigation in proton beam therapy. We sought to simulate a ventricular tachycardia radioablation with proton minibeams and to demonstrate that it was possible to obtain a homogeneous coverage of an arrhythmogenic cardiac zone with this technique. MATERIAL AND METHODS: An arrhythmogenic target volume was defined on the simulation CT scan of a patient, localized in the lateral wall of the left ventr

Feasibility of 4D HDR brachytherapy source tracking using x-ray tomosynthesis: Monte Carlo investigation.

PURPOSE: High dose rate (HDR) brachytherapy rapidly delivers dose to targets with steep dose gradients. This treatment method must adhere to prescribed treatment plans with high spatiotemporal accuracy and precision, as failure to do so may degrade clinical outcomes. One approach to achieving this goal is to develop imaging techniques to track HDR sources in vivo in reference to surrounding anatomy. This work investigates the feasibility of using an isocentric C-arm x-ray imager and tomosynthesis methods to track Ir-192 HDR brachytherapy sources in vivo over time (4D). METHODS: A tomosynthesi

Simulation study of protoacoustics as a real-time in-line dosimetry tool for FLASH proton therapy.

BACKGROUND: Applying ultra-high dose rates to radiation therapy, otherwise known as FLASH, has been shown to be just as effective while sparing more normal tissue compared to conventional radiation therapy. However, there is a need for a dosimeter that is able to detect such high instantaneous dose, particularly in vivo. To fulfill this need, protoacoustics is introduced, which is an in vivo range verification method with submillimeter accuracy. PURPOSE: The purpose of this work is to demonstrate the feasibility of using protoacoustics as a method of in vivo real-time monitoring during FLASH

Linear energy transfer (LET) distribution outside small radiotherapy field edges produced by 6 MV X-rays.

In modern radiotherapy with photons, the absorbed dose outside the radiation field is generally investigated. But it is well known that the biological damage depends not only on the absorbed dose but also on LET. This work investigated the dose-average LET (LΔ,D) outside several small radiotherapy fields to provide information that can help for better evaluating the biological effect in organs at risk close to the tumour volume. The electron fluences produced in liquid water by a 6 MV X-rays Varian iX linac were calculated using the EGSnrc Monte Carlo code. With the electron spectra, LΔ,D calc

Pharmacokinetic/Pharmacodynamic Target Attainment of Different Antifungal Agents in De-escalation Treatment in Critically Ill Patients: a Step toward Dose Optimization Using Monte Carlo Simulation.

Differences in pharmacokinetics/pharmacodynamics (PK/PD) target attainment are rarely considered when antifungals are switched in critically ill patients. This study intends to explore whether the antifungal de-escalation treatment strategy and the new intermittent dosing strategy of echinocandins in critically ill patients are able to achieve the corresponding PK/PD targets. The published population PK models of antifungals in critically ill patients and a public data set from the MIMIC-III database (n = 662) were employed to evaluate PK/PD target attainment of different dosing regimens of an

Time efficiency analysis for undersampled quantitative MRI acquisitions.

To realize Quantitative MRI (QMRI) with clinically acceptable scan time, acceleration factors achieved by conventional parallel imaging techniques are often inadequate. Further acceleration is possible using model-based reconstruction. We propose a theoretical metric called TEUSQA: Time Efficiency for UnderSampled QMRI Acquisitions to inform sequence design and sample pattern optimisation. TEUSQA is designed for a particular class of reconstruction techniques that directly estimate tissue parameters, possibly using prior information to regularize the estimation. TEUSQA can be used to evaluate

Electron backscattering for signal enhancement in a thin-film CdTe radiation detector.

BACKGROUND: Thin-film cadmium telluride (CdTe) offers high average electron density, direct detection configuration, and excellent radiation hardness, making it an attractive material for radiation detectors. Although a very thin detector provides capabilities to conduct high-resolution measurements in high-energy radiation fields, it is limited by a low signal, often boosted with a front metal converter enhancing X-ray absorption. An extension of this approach can be explored through the investigation of electron backscattering phenomenon, known to be highly dependent on the material atomic n

Estimation of absorbed radiation doses to skin and S-values for organs at risk due to nasal administration of PET agents using Monte Carlo simulations.

PURPOSE: The intranasal (IN) administration of radiopharmaceuticals is of interest in being a viable route for the delivery of radiopharmaceuticals that do not ordinarily cross the blood-brain barrier (BBB). However, to be viable in a patient population, good image quality as well as safety of the administration should be demonstrated. This work provides radiation dosimetry calculations and simulations related to the radiation safety of performing such experiments in a human cohort. METHODS: We performed Monte Carlo (MC) simulations to estimate radiation dose to the skin inside a cylindrical

[Monte Carlo simulation of biometric effect sizes and their influence on the translational ratio of corneal astigmatism in the cylinders of toric intraocular lenses].

BACKGROUND AND OBJECTIVE: Toric intraocular lenses (IOL) provide a reliable and predictable option for permanent correction of corneal astigmatism. In order to determine the lens strength necessary for achieving the desired correction, the operator can either use the calculation mode implemented in the biometry device or the calculation service offered by the lens manufacturer; however, in many cases a classical lens calculation from biometric data is not carried out but only a simplified estimation, which translates the corneal astigmatism into the torus of the toric IOL. This translational r

Tabulation of organ dose conversion factors for terrestrial radioactivity monitoring program.

Terrestrial radioactivity monitoring of 238U and 232Th series, and 40K in soil is an essential practice for radioactivity and radiation measurement of a place. In conventional practice, only basic data can be in-situ measured using a survey instrument, for example radioactivity concentration in soil and ambient dose equivalent rate. For other physical quantities, for example organ absorbed dose and organ equivalent dose, the measurement is impossible to be performed and can only be computed using Monte Carlo radiation transport simulations. In the past, most of the works only focused on calcul

Understanding the role of hydrophobic patches in protein disaggregation.

Protein folding is a very complex process and, so far, the mechanism of folding still intrigues the research community. Despite a large conformational space available (O(1047) for a 100 amino acid residue), most proteins fold into their native state within a very short time. While small proteins fold relatively fast (a few microseconds) large globular proteins may take as long as several milliseconds to fold. During the folding process, the protein synthesized in the ribosome is exposed to the crowded environment of the cell and is easily prone to misfolding and aggregation due to interactions
