Special issue on microglia and chronic pain.

This special issue of Experimental Neurology is devoted to the role of Microglia and Chronic Pain. Chronic pain affects 116 million people per year in the United States, which is more than heart disease, cancer, and diabetes combined. Nervous system trauma and disease are principal contributors to the establishment of chronic pain in people and in animal models. Central nervous system (CNS) injury or tumor development, peripheral nerve injury, multiple sclerosis, diabetes and many other neurological disruptions can serve as the instigating pathophysiolgical conditions that lead to chronic pain

Recessive dystrophic epidermolysis bullosa associated with mesangioproliferative glomerulonephritis and multifocal necrotizing leucoencephalopathy of the pons.

We report a woman with recessive dystrophic epidermolysis bullosa (RDEB) in whom there was prolonged sepsis and death at age 22 years. Autopsy revealed multiple epidermolytic skin lesions with chronic ulceration, mesangioproliferative glomerulonephritis and multifocal necrotizing leucoencephalopathy (MNL) of the pons. The latter two conditions may have been mediated by sepsis-associated cytokines. Although mesangioproliferative glomerulonephritis has previously been described in association with RDEB, to our knowledge this is the first report of MNL in a patient with RDEB.

Maintaining health in older people with multiple chronic illnesses.

The effective management of the elderly patient who has multiple chronic conditions is an important clinical responsibility. Of special importance is an understanding and appreciation of the maintenance of function. A focus on the patient's needs and ability to function, rather than on the diseases from which he or she suffers, is more likely to improve productivity and quality of life. These goals can usually be achieved in the elderly patient without curing the underlying disease.

Medical disorders of suicides in Australia: analysis using a multiple-cause-of-death approach.

The impaired health of a person who has committed suicide is often suggested among the proximate causes of suicidal behaviour. The introduction in 1997 of multiple-cause-of-death coding by the Australian Bureau of Statistics provided an opportunity to examine health impairments recorded on the death certificates of suicides. Data for the quinquennium 1997-2001 revealed a high prevalence of mental and behavioural disorders, in particular among women and among young and adolescent suicides. Comparison of multiple causes of death attributed to those who died in accidents with those recorded as su

Bioinformatics-based identification of potential genes in the thalamus associated with chronic orofacial and somatic inflammatory pain.

BACKGROUND: The thalamus is one of the structures that receives projections from multiple ascending pain pathways. Although the transmission of orofacial and somatic pain involves different nuclear clusters and nerve pathways, the thalamic neuroimmune alterations across these different pain modalities remain poorly understood. This work aimed to investigate alterations in the neuroimmune mechanisms of the thalamus under chronic orofacial and somatic inflammatory pain conditions via bioinformatics analysis and to identify novel therapeutic targets for chronic pain management. METHODS: To illum

Autonomic neurophysiologic implications of disorders comorbid with bladder pain syndrome vs myofascial pelvic pain.

AIMS: The neuropathophysiology of a debilitating chronic urologic pain condition, bladder pain syndrome (BPS), remains unknown. Our recent data suggests withdrawal of cardiovagal modulation in subjects with BPS, in contrast to sympathetic nervous system dysfunction in another chronic pelvic pain syndrome, myofascial pelvic pain (MPP). We evaluated whether comorbid disorders differentially associated with BPS vs MPP shed additional light on these autonomic differences. METHODS: We compared the presence and relative time of onset of 27 other medical conditions in women with BPS, MPP, both syndr

A rare grade III symptomatic Tsunoda class III left lobe hepatolithiasis in a North American woman.

A 44-year-old Caucasian woman without any identified predisposing factors, presented to her local emergency room with multiple episodes of recurrent ascending cholangitis. She underwent a laparoscopic cholecystectomy several years prior to her admission to the hospital for parenteral antibiotic therapy and she had no personal or family history of hypercholesterolaemia. Transferred to a tertiary hospital for surgical therapy, she underwent a left hepatic resection for a chronically infected left lobe hepatolithiasis (HL) conditioning atrophy of the parenchyma due to obstruction of the biliary t

Multimorbidity in Chronic Overlapping Pain Conditions: From Burden to Integrated Care.

Chronic overlapping pain conditions (COPCs) refer to a set of chronic pain disorders that frequently co-occur and may involve partially overlapping mechanisms. The U.S. National Institutes of Health currently recognizes ten COPCs: fibromyalgia, painful temporomandibular disorders, chronic low back pain, chronic migraine headache, chronic tension-type headache, irritable bowel syndrome, endometriosis, interstitial cystitis/bladder pain syndrome, vulvodynia, and myalgic encephalomyelitis/chronic fatigue syndrome. When multiple COPCs coexist, they are associated with a disproportionate multimorbi

A clear look at the neuroimmunology of multiple sclerosis and beyond.

The term neuroimmunology was first coined to refer to a generic involvement of the immune system in the pathogenesis of neurological diseases, particularly of the central nervous system. Since then, the neuroimmunology spectrum has steadily grown and currently spans from classical autoimmune diseases of the central and peripheral nervous systems to previously unsuspected conditions such as autism spectrum disorders or chronic fatigue syndrome. Multiple sclerosis remains the predominant entity in terms of research efforts and social pressure as well as a good model of organ-specific autoimmune

Sex and gender differences in pain.

Chronic pain affects 20% of adults and is one of the leading causes of disability worldwide. Women and girls are disproportionally affected by chronic pain. About half of chronic pain conditions are more common in women, with only 20% having a higher prevalence in men. There are also sex and gender differences in acute pain sensitivity. Pain is a subjective experience made up of sensory, cognitive, and emotional components. Consequently, there are multiple dimensions through which sex and gender can influence the pain experience. Historically, most preclinical pain research was conducted exclu

Neurobiological aspects of pain in the context of alcohol use disorder.

Alcohol is an effective and widely utilized analgesic. However, the chronic use of alcohol can actually facilitate nociceptive sensitivity over time, a condition known as hyperalgesia. Excessive and uncontrollable alcohol drinking is also a hallmark feature of alcohol use disorder (AUD). Both AUD and chronic pain are typically accompanied by negative affective states that may underlie reinforcement mechanisms contributing to AUD maintenance or progression. Frequent utilization of alcohol to relieve pain in individuals suffering from AUD or other chronic pain conditions may thus represent a pow

The juvenile fibromyalgia syndrome (JFMS): a poorly defined disorder.

Juvenile fibromyalgia syndrome (JFMS) is a chronic condition characterized by symptoms of chronic diffuse musculoskeletal pain and multiple painful tender points on palpation. It is often accompanied by fatigue, disorders of sleep, chronic headaches, irritable bowel syndrome, and subjective soft tissue swelling. The complexity of the presenting clinical picture in JPFS has not been sufficiently defined in the literature. Similarities to adult fibromyalgia syndrome in JFMS are often difficult to compare, because many of the symptoms are "medically unexplained" and often overlap frequently with

The Interplay of Aging and the Immune System.

Aging is a genetically regulated biological process. We review recent studies that highlight the role of the immune system, specifically chronic inflammation, in contributing to age-related decline and frailty. Of clinical importance, aging is a risk factor for multiple conditions. During childbearing years, women are largely protected from diseases associated with aging. However, menopause, which marks the loss of ovarian function, introduces a significant shift. An unexpected and remarkable discovery is that the decline in estrogen levels triggers chronic inflammation, which in turn promotes

Intersectional health-related stigma in persons living with HIV and chronic pain: implications for depressive symptoms.

"Intersectional health-related stigma" (IHRS) refers to stigma that arises at the convergence of multiple health conditions. People living with HIV (PLWH) and chronic pain have two highly stigmatized health conditions, and thus may be at especially high risk for internalizing these stigmas and consequently experiencing depression. This study examined the intersectionality of internalized HIV and chronic pain stigma in relation to depressive symptoms in a sample of PLWH and chronic pain. Sixty participants were recruited from an HIV clinic in the Southeastern United States. Chronic pain was def

Physical exercise as a modulator of the sleep-obesity interaction: metabolic, neuroendocrine and circadian mechanisms.

Obesity and sleep disturbances are highly prevalent chronic conditions that share multiple pathophysiological mechanisms, including chronic low-grade inflammation, neuroendocrine dysfunction, circadian misalignment, autonomic imbalance, insulin resistance, and metabolic dysregulation. These conditions establish a multidirectional relationship that contributes to the development and progression of cardiometabolic, respiratory, and psychological comorbidities. In this context, physical exercise (PE) is widely recognized as a non-pharmacological strategy for the prevention and treatment of obesit

Postoperative hyperkalemia.

Hyperkalemia occurs frequently in hospitalized patients and is of particular concern for those who have undergone surgery, with postoperative care provided by clinicians of many disciplines. This review describes the normal physiology and how multiple perioperative factors can disrupt potassium homeostasis and lead to severe elevations in plasma potassium concentration. The pathophysiologic basis of diverse causes of hyperkalemia was used to broadly classify etiologies into those with altered potassium distribution (e.g. increased potassium release from cells or other transcellular shifts), re

Tobacco and alcohol use disorders: Evaluating multimorbidity.

There is growing interest in applying the multimorbidity model for mental health disorders - i.e. the interactive effects of co-occurring diagnoses. Alcohol use disorder (AUD) and tobacco use disorder (TUD) often occur together, but distinctive correlates of their co-occurrence have not been studied. Veterans treated by the Veterans Health Administration (VHA) nationally in FY 2012 with diagnoses of both AUD and TUD were compared to those with only AUD or only TUD on socio-environmental factors, medical and psychiatric diagnoses, and service use. Multimorbid effects were defined as those in wh
