BACKGROUND: Electromagnetic navigation bronchoscopy (ENB) is a useful method to obtain tissue for peripheral lung nodules. We aimed to understand the diagnostic yield and safety profile in high-risk pulmonary nodules that cannot be accessed by percutaneous transthoracic needle biopsy.
METHODS: In this single-center retrospective study, we reviewed patients who underwent ENB for high-risk pulmonary nodules. All procedures were performed under moderate sedation using intravenous midazolam and fentanyl.
RESULTS: A total of 100 pulmonary nodules in 90 patients were subjected to ENB between Octob

HISTORY AND CLINICAL FINDINGS: A 67-year-old woman with a history of chronic obstructive pulmonary disease and breast cancer developed an increase in size and number of pulmonary nodules that had previously been stable over the course of 9 years.
INVESTIGATIONS: Thoracic computed tomography revealed a well-defined pulmonary lesion with a diameter of 1,1 × 1,1 cm, accompanied by multiple bilateral pulmonary nodules with a maximum size of 5 mm. Thoracoscopic resection of the major nodule was extended to a completive resection of an adjacent lung area that operatively showed a macroscopically no

BACKGROUND: Pulmonary meningothelial-like nodules (PMNs) are benign proliferations of unclear clinical significance. They are mainly asymptomatic lesions that are usually discovered during the pathologic evaluation of resected pulmonary specimens or following post-mortem examination. Diffuse pulmonary meningotheliomatosis (DPM), which presents as bilateral multiple PMNs throughout the lungs, has been described less frequently. DPMs are benign lesions associated with both neoplastic and non-neoplastic pulmonary conditions.
CASE PRESENTATION: We report the case of a 59-year-old female patient w

BACKGROUND: Benign metastasizing leiomyoma (BML) is a rare condition described as multiple well-differentiated leiomyomas at sites distant from the uterus. Apart from lungs it has also been reported in lymph nodes, heart, brain, bone, skin, eye and spinal cord. We present a case of pulmonary benign metastasizing leiomyoma in a female patient admitted to our hospital with suspicion of left adnexal tumor.
CASE REPORT: A 45-year-old woman was referred to our hospital with suspicion of left adnexal tumor. The control transvaginal ultrasound examination performed at admission to the Gynecological

Adenoid cystic carcinoma (ACC) of the lung is a rare subtype of lung cancer characterized by tumor cells forming cystic or glandular structures. It predominantly arises in segmental or larger bronchi, typically presenting as solid nodules or masses on imaging. Previous studies describe it as a slow-growing, low-grade malignancy associated with favorable prognosis and low metastatic potential. A 29-year-old male underwent surgical resection for breast nodules, pathologically confirmed as ACC of the breast. During hospitalization, chest CT revealed multiple bilateral ground glass nodules (GGN),

Pulmonary brucellosis is a rare focal manifestation of human brucellosis with non-specific clinical features. Predominant imaging findings include pneumonia, pleural effusion, pulmonary nodules, abscesses, and interstitial changes. Multiple cavitary lesions are exceptionally rare. Herein, we report a case of bilateral multiple pulmonary cavities in a 76-year-old man with a 2-year history of intermittent cough, sputum production, and progressive dyspnea that acutely worsened 10 days prior to admission with intermittent fever, anorexia, and fatigue. Chest computed tomography (CT) revealed bilate

Malignant lymphoma has various pulmonary manifestations on chest CT, including nodules, masses, areas of consolidation, and ground-glass opacity. These presentations can pose a diagnostic challenge, as they mimic other disease patterns. Herein, we report a case of diffuse large B-cell lymphoma (DLBCL) manifesting as miliary nodules in a 67-year-old male initially presenting with dyspnea and fever. Radiologic findings included diffuse, bilateral, multiple tiny nodules consistent with metastasis, miliary tuberculosis, and fungal infection. However, further investigations, including laboratory te

BACKGROUND: Limited research exists on screening thresholds for low-dose CT in detecting malignant pure ground-glass lung nodules (pGGNs) in the Chinese population.
MATERIALS AND METHODS: A retrospective analysis of the Guangzhou Lung-Care programme was conducted, retrieving average transverse diameter, location, histopathology, frequency and follow-up intervals. Diagnostic performances for 'lung cancers' were evaluated using areas under the curve (AUCs), decision curve analysis (DCA), sensitivities and specificities, with thresholds ranging from 5 mm to 10 mm. We divide malignant pGGNs into

BACKGROUND: Prospective validation and comparison of the performance of nodule management protocols is limited. The aim of this study was to examine the performance of size and risk thresholds for assessing malignancy in solid nodules at baseline low-dose CT (LDCT) in a lung cancer screening (LCS) programme.
METHODS: This was an observational study using data from the SUMMIT Study, a prospective longitudinal study investigating LDCT for LCS. Participants were 55-77 years old and met either the United States Preventative Services Task Force (2013) criteria or had a PLCOm2012 risk of ≥1.3%. LDC

PURPOSE: This study evaluates the utility of gray-scale inversion imaging for the detection of pulmonary nodules on chest radiography, which are often difficult to appreciate in traditional imaging.
APPROACH: 100 paired chest radiographs and CT scans was provided to six radiologists (three attending and three residents) in a within-subjects study design. Each radiologist analyzed the radiographs with and without gray-scale inversion. Detection rates of pulmonary nodules were compared across conditions. Sensitivity and specificity were analyzed using statistical tests to determine differences

BACKGROUND: Navigational bronchoscopy and other imaging modalities have improved the ability to evaluate pulmonary nodules/mass. Many of these lesions are located outside the bronchial airway and are difficult to access even with these devices. The Transbronchial Access Tool (Medtronic, Minneapolis, MN) allows the bronchoscopist to create a pathway from the bronchial airway, across the lung parenchyma, and into the target lesion. We are reporting the feasibility and safety of this new device.
METHODS: Patients with peripheral pulmonary nodules/mass with an absence of an air bronchogram on tho

OBJECTIVE: Because of decreased tactile sensation with thoracoscopic approaches to biopsy, localization preoperatively and intraoperatively is important for successful biopsy. Our study evaluated the technique of combined computed tomography-guided hookwire and methylene blue localization.
METHODS: Seventy-five patients from November 2007 to August 2013 who underwent combined Hawkins hookwire and methylene blue localization of 76 total pulmonary nodules before video-assisted thoracic surgery-guided wedge resection were retrospectively reviewed. BACKGROUND: Pulmonary nodular lymphoid hyperplasia is a rare benign lymphoproliferative disorder. Patients are commonly asymptomatic. Its radiological presentation is characterized by peripheral single nodular lesion, multiple nodules or mass-like consolidation. Therefore, it can mimic other more frequent pulmonary diseases, such as lung cancer and pulmonary lymphoma. The diagnosis is often late, relying on histopathological findings.
CASE PRESENTATION: We report a case of a 66-year-old woman who presented with cough and upper respiratory tract symptoms. Chest CT revealed a mass in the right

We report a rare case of multiple primary pulmonary meningiomas in a 52-year-old woman who presented with lung nodules identified on abdominal computed tomography(CT). She had a history of surgery and chemotherapy for an immature ovarian teratoma 33 years prior to presentation. She underwent resection of a mature retroperitoneal teratoma 16 years prior and a peritoneal teratoma 6 months prior to presentation. Chest CT revealed 3 solid nodules in the right S3, right S8, and left S6 lung segments, and we performed thoracoscopic resection. Histopathologically, the 3 tumors were diagnosed as menin

Pulmonary amyloidosis can develop together with both primary or secondary amyloidosis, isolated or associated with amyloidosis of other organs. In the study a case of a 59 year old female with multiple nodules and diffuse amyloid infiltration in pulmonary parenchyma associated with Sjörgen's syndrome is presented. Amyloid deposits were also found in the surgically removed parotid salivary gland, mostly in its blood vessel walls.

Multiple peripheral pulmonary carcinoid tumors or their smaller counterparts (tumorlets of carcinoid type) are the most unusual form of carcinoids as a cause of ectopic corticotropin syndrome. Only three case reports were found in the literature. We describe a 35 year-old female patient with ectopic corticotropin secretion due to multiple peripheral pulmonary carcinoid tumors and tumorlets. A high-dose dexamethasone suppression test result led to the diagnosis of Cushing's disease in our case. But no tumor was identified on sella imaging and bilateral inferior petrosal sinus sampling was non-d

The CT halo sign was first described in immunocompromised patients with invasive pulmonary aspergillosis. Although the halo sign was originally thought to be specific for invasive pulmonary aspergillosis, it has been reported in a wide variety of pulmonary abnormalities in both immununocompromised and immunocompetent patients. We report a case of mucinous adenocarcinoma of the pancreas metastatic to the lungs in which there were multiple pulmonary nodules showing the halo sign. This case further illustrates the nonspecific nature of the CT halo sign and the need to consider malignancy as a cau…
