Outcomes in checkpoint inhibitor induced neuromuscular immune related adverse events: A case series.

Immune-checkpoint inhibitors, a mainstay of treatment of solid tumors, rarely lead to activation of the host immune system against another target, causing autoimmune inflammation of other end organs known as immune-related adverse events (ir-AEs). When ir-AEs are directed at the neuromuscular system, clinical consequences include myositis and myasthenic syndromes. In this article, we describe three cases presenting with neuromuscular ir-AEs. Two patients had lung adenocarcinoma and were treated with pembrolizumab. One patient had gastroesophageal junction adenocarcinoma treated with nivolumab.…

Association Between Statin Initiation and Myasthenia Gravis Crisis.

BACKGROUND AND PURPOSE: Statins are widely prescribed for preventing cardiovascular disease, but concerns remain that they may trigger or exacerbate myasthenia gravis (MG). We evaluated whether statin initiation increases the risk of an MG crisis using a self-controlled case series (SCCS) design. METHODS: This SCCS study analyzed nationwide Korean claims data from 2007 to 2023. We included patients in whom statin therapy was initiated after an MG diagnosis and who experienced at least one MG crisis during a 730-day period spanning from 365 days before to 365 days after statin initiation. The …

Efficacy and Safety of Nipocalimab in Adults With Generalised Myasthenia Gravis: An Asian Subgroup Analysis of the Vivacity-MG3 Study.

BACKGROUND AND PURPOSE: The phase 3, randomised, double-blind, placebo-controlled Vivacity-MG3 trial demonstrated that nipocalimab, a neonatal Fc receptor blocker, provided sustained disease control over 6 months when added to standard of care therapies in patients with antibody-positive generalised myasthenia gravis (gMG). Here we present the Vivacity-MG3 Asian subgroup analysis. METHODS: Recruited patients were randomised (1:1) to receive either nipocalimab (30 mg/kg loading dose, then 15 mg/kg every 2 weeks for maintenance) or placebo infusions every 2 weeks, both alongside standard of car…

Comparison of preoperative urinary Titin levels and serum Titin levels in patients with gastrointestinal malignancy.

BACKGROUND: In recent years, reports using urinary Titin (U-Titin) levels to evaluate patients with muscular dystrophy, myocardial infarction, myasthenia gravis, gastrointestinal malignancy (GIM), chronic liver disease, and sarcopenia have been sporadically observed; however, reports using serum Titin (S-Titin) levels are scarce. We report here on the measurement of S-Titin, which has now become feasible. METHODS: This study included 104 patients who underwent surgery for the diagnosis of GIM in our department between May 2024 and November 2025. Preoperative S-Titin and U-Titin levels were me…

Failure of early B cell tolerance and possible BCR signaling dysregulation underlie NF155-mediated autoimmune nodopathies.

Autoimmune nodopathies (AINs) are rare acquired autoimmune neuropathies with distinct clinical features and circulating autoantibodies, often of the immunoglobulin G4 (IgG4) subclass, targeting proteins at the node of Ranvier. Defects in B cell tolerance checkpoints have been implicated in several autoimmune diseases, including MuSK-positive myasthenia gravis, another IgG4 autoantibody-mediated disease. Here, we investigated whether tolerance defects exist in neurofascin-155-mediated AIN (NF155-AIN), using a well-established assay, by generating recombinant antibodies from new emigrant (NE) an…

Association Between the Hemoglobin-to-Red Blood Cell Distribution Width Ratio and Perioperative Mortality Risk in Myasthenia Gravis Patients with Sepsis: A Retrospective Cohort Study.

BackgroundThis study aims to explore the association between the Hemoglobin/Red Cell Distribution Width Ratio (HRR) and perioperative mortality in myasthenia gravis (MG) patients with sepsis based on the electronic ICU collaborative research database (eICU-CRD).MethodsWe retrospectively analyzed data of 151 patients from the eICU-CRD between 2014 and 2015. We performed multivariate logistic regression, stratified analysis with interaction terms, and restricted cubic spline regression to assess the linear relationship. Additionally, we applied the extreme gradient boosting (XGBoost) machine lea…

Investigating the Risk Factors for Medication Nonadherence in Myasthenia Gravis Patients: Establishing a Nomogram Model.

OBJECTIVE: Many studies have shown that MG patients often show poor adherence to their medications. Poor adherence may affect disease control, and also put a heavy burden on patients' physical functions and medical costs. Therefore, this study aimed to develop and verify a nomogram for predicting medication adherence in MG patients. METHODS: We prospectively reviewed clinical information, demographic data, and medication adherence records for MG patients at Shijiazhuang People's Hospital from January to December 2025. Logistic regression models were used to analyse risk factors related to med

Rituximab in Myasthenia Gravis: a real-world study using inverse probability of treatment weighting.

BACKGROUND: The role of rituximab in the treatment of myasthenia gravis (MG) remains uncertain due to limited randomized controlled evidence and heterogeneous observational data. While rituximab is often used in refractory MG, its comparative effectiveness against other non-steroidal immunosuppressive therapies (NSISTs) has yet to be fully clarified. OBJECTIVE: To evaluate the effectiveness of rituximab compared to a second NSIST in achieving a composite clinical outcome in acetylcholine-receptor-antibody (AChR-Ab) positive MG patients using causal inference methods to adjust for confounding.…

Improving outcomes in myasthenia gravis: uni-portal video-assisted thoracoscopic (UVATS) thymectomy in a single-centre retrospective cohort.

BACKGROUND: Thymectomy is a well-established treatment for myasthenia gravis (MG), with minimally invasive approaches increasingly preferred due to reduced morbidity. This study evaluates the safety and clinical outcomes of uni-portal video-assisted thoracoscopic surgery (UVATS) thymectomy in MG patients at a Malaysian tertiary centre. METHODS: We conducted a retrospective cohort study of 30 patients who underwent UVATS thymectomy for MG at Universiti Malaya Medical Centre between August 2016 and July 2025. Of these, 24 patients with complete perioperative and follow-up data were included. Ou…

Targeting BLyS and APRIL with Telitacicept versus Conventional Immunotherapy in Generalized Myasthenia Gravis: A Comparative Study.

BACKGROUND: Telitacicept is approved for acetylcholine receptor antibody-positive generalized myasthenia gravis (AChR-Ab+ gMG) in China. Although clinical trials have demonstrated its efficacy, real-world outcomes and safety require validation. This study compared telitacicept with conventional immunotherapy in a broader AChR-Ab+ GMG population, bridging the gap between RCT data and real-world practice. METHODS: This retrospective real-world study (ChiCTR2500109279) included 215 patients with AChR-Ab+ gMG, with 43 receiving telitacicept and 172 undergoing conventional immunotherapy. Propensit…

Improving sensitivity of autoantibody testing for myasthenia gravis using cell-based assays: an evaluation of strategies that may be used in clinical practice.

INTRODUCTION: Testing for acetylcholine receptor and muscle-specific kinase antibodies (anti-AChR and anti-MuSK) is of central diagnostic importance in myasthenia gravis (MG), yet practical evaluations of strategies to improve sensitivity using cell-based assays (CBAs) are lacking. We evaluated strategies to improve sensitivity of autoantibody testing for MG using CBAs at our center, including repeat fixed CBA at routine 1:10 dilution, send-out live CBA, and fixed CBA at lower 1:5 dilution. METHODS: Quality improvement/quality assurance (QI/QA) evaluation of all patients at London Health Scie…

Circular RNA circNUP214 serves as a microRNA-31 sponge to promote the progression of myasthenia gravis through NFAT5.

Myasthenia gravis (MG) is an autoimmune disease driven by autoantibodies targeting the neuromuscular junction, leading to muscle weakness. Although emerging evidence implicates circular RNAs (circRNAs) in MG, their specific regulatory mechanisms remain largely unknown. In particular, the role of circNUP214 in MG has not been characterized. CircNUP214 expression in PBMCs and CD4+ T cells from MG patients and healthy controls was determined by qRT-PCR. Its circular structure and cytoplasmic localization were verified. Furthermore, the interaction between circNUP214 and miR-31 was confirmed, and …
