PURPOSE: To correlate the MR findings in transverse myelitis secondary to systemic lupus erythematosus with clinical findings during disease exacerbation and remission.
METHODS: Four patients (ages 33 to 47 years) with episodes of transverse myelitis secondary to systemic lupus erythematosus were identified. Three patients had recurrent transverse myelitis episodes (one patient with two recurrences), for a total of eight episodes. MR examinations (six after contrast administration) were performed during each transverse myelitis episode, as well as during four periods of remission (in three pa

Post-infectious transverse myelitis is an important clinical entity in childhood, but the diagnosis is difficult to confirm by conventional radiologic investigations. We report a patient with acute, cervical, post-infectious transverse myelitis whose diagnosis had been established by the distribution and evolution of lesions on serial magnetic resonance imaging (MRI). Autopsy results confirmed the diagnosis of post-infectious transverse myelitis and the presence of MRI-identified lesions. To our knowledge, this patient is the first child reported with MRI findings in post-infectious transverse

PURPOSE: Varicella-zoster virus (VZV) belongs to the human neurotropic alpha-herpes virus group. VZV reactivation can lead to neurological complications, including transverse myelitis. However, transverse myelitis caused by VZV reactivation is rare in immunocompetent patients. A case of transverse myelitis caused by VZV in an immunocompromised patient is presented in this paper.
CASE DESCRIPTION: A 64-year-old female patient was admitted to our outpatient clinic with complaints of pain, numbness and loss of strength in her right arm, and decreased sensation of warmth, after suffering from zon

BACKGROUND: Differential diagnosis of non-compressive cervical myelopathy encompasses a broad spectrum of inflammatory, infectious, vascular, neoplastic, neurodegenerative, and metabolic etiologies. Although the speed of symptom onset and clinical course seem to be specific for certain neurological diseases, lesion pattern on MR imaging is a key player to confirm diagnostic considerations.
METHODS: The differentiation between acute complete transverse myelitis and acute partial transverse myelitis makes it possible to distinguish between certain entities, with the latter often being the onset

INTRODUCTION: We evaluated the spectrum of acquired demyelinating and inflammatory disorders in patients presenting with an acute transverse myelopathy. We also studied differences between an acute idiopathic transverse myelitis and myelitis resulting from other etiologies.
MATERIALS AND METHODS: Eighty consecutive patients with acute transverse myelopathy were included. At inclusion, clinical profile, serum and cerebrospinal fluid parameters, brain and spinal cord magnetic resonance imaging, and visual evoked potentials were obtained. All patients were given methylprednisolone therapy. Patie

We report the case of a 46-year-old female patient with transverse myelitis complicating mixed connective tissue disease (MCTD). She responded well to steroid and immunosuppressive therapy. Unlike in systemic lupus erythematosus (SLE), transverse myelopathy in association with MCTD is very rarely described. The mechanisms, diagnosis and treatment of transverse myelitis in MCTD are briefly discussed.

The rapid development of paraparesis or tetraparesis combined with a bilateral sensory deficit that has a clearly defined rostral border and bladder dysfunction are the principal features of acute transverse myelopathy. Acute partial transverse myelopathy is far much more frequent: its symptoms are asymmetric, sometimes unilateral, and sensory deficit may predominate. An urgent MRI is required to exclude acute spinal cord compression. Diagnosis of ischemic acute transverse myelopathy includes the following elements: sudden onset, neurologic symptoms compatible with infarction in the anterior s

A 12-year-old boy presented with typical features of short incubation tetanus. Spastic paraparesis with a mid-thoracic sensory level was present from the time of the initial examination. X-rays of the spine and lumbar myelogram were normal. The paraparesis improved rapidly and within a month the patient was free of neurological deficit. The case is possibly a rare example of tetanus involving the central nervous system other than the anterior horn cells.

BACKGROUND: Transverse myelitis is an urgent medical problem all over the world. Acute transverse myelitis is more common in children than in adults. Postvaccinal transverse myelitis is a rare complication of vaccination. In vaccine-associated paralytic poliomyelitis, the virus mosaically infects the motor neurons of the anterior horns of the spinal cord, resulting in asymmetric paralysis of predominantly proximal muscles.
CASE PRESENTATION: The case of acute transverse myelitis in a 12-year-old child corresponds to the existing definition of a vaccine-associated paralytic poliomyelitis case:

Acute transverse myelitis (ATM) with moderate symptomatology and smaller multiple magnetic resonance imaging lesions is often caused by multiple sclerosis. Severe ATM with extensive magnetic resonance imaging lesions with or without associated meningitis often has a viral cause, particularly in the younger age groups, whereas vascular disorders may prevail among older patients. Previously, one had to rely on indirect evidence such as viral serology or viral identification in throat washings to confirm a diagnosis of myelitis. Thus, mycoplasma myelitis may occur coincident with a mycoplasma pne

BACKGROUND: Atypical myelitis in multiple sclerosis (MS) is characterized by extensive myelitis in the longitudinal (longitudinally extensive transverse myelitis) or axial plane (transverse myelitis).
OBJECTIVE: To characterize a cohort of MS patients with atypical myelitis.
METHODS: Atypical myelitis was extracted from the French and Luxembourg MS databases and compared to two cohorts of MS patients with typical myelitis and neuromyelitis optica spectrum disorders (NMOSDs) patients with myelitis.
RESULTS: We enrolled 28 MS patients with atypical myelitis, 68 MS patients with typical myelit

Differential diagnosis of non-traumatic, acute transverse spinal cord syndromes should cover compressive myelopathy (mostly hematomas or tumors), inflammatory myelitis and vascular myelopathies. Since acute pathologies of the spinal cord primarily result in flaccid para- or tetraparesis accompanied by areflexia or hyporeflexia (spinal shock), acute polyradiculoneuritis and the cauda equina syndrome must also be weighed into the differential diagnosis. Paraplegia may ultimately also be of psychogenic origin. The clinical picture is characterized by the rapidity of progression, the possible invo

Neuromyelitis optica of Devic (NMO) is a syndrome which combines transverse myelitis and optic neuritis. For many years it was assumed that transverse myelitis and optic neuritis should be simultaneous, that optic neuritis should be bilateral, and that transverse myelitis must be complete. NMO was considered to be very rare. Through the 20th century there was a debate on whether NMO is a distinct nosologic entity or a form of MS. In 2005 it is now possible to distinguish NMO from MS on the basis of the clinical, imaging, serology and immunopathology profile. This distinction is necessary for t

This report presents a rare case of acute transverse myelitis (ATM) after mumps infection in a 33-year-old male. Symptoms included fever, parotid and scrotal swelling, and subsequent sensory-motor paraparesis. Magnetic resonance imaging revealed long-segment spinal cord hyperintensity, and cerebrospinal fluid analysis showed pleocytosis and elevated protein, with positive mumps virus antibodies. Treatment with high-dose pulse steroids resulted in significant improvement. This underscores the association between mumps and ATM in adults, suggesting an autoimmune-inflammatory mechanism. The favor

OBJECTIVES: Neuromyelitis optica (NMO) is an inflammatory demyelinating disease of central nervous system. We report a case of NMO spectrum disorder that developed acute cervical segmental denervation during an episode of transverse myelitis.
METHODS: Case report and a literature review.
RESULTS: A 69-year-old woman presented with right leg weakness, which improved in 2 weeks of intravenous methylprednisolone. Six weeks later, she returned with recurrent transverse myelitis followed by left hand weakness and atrophy. Electromyography showed active denervation on left C8/T1-innervated muscles
