Knowledge of anatomy and physiology of the nail unit is necessary to understand onychomycosis, their clinical patterns, and response to the different treatments. It was useful for the discovery of new transungueal products. In this chapter, we describe anatomy and physiology of the normal nail and the main routes of fungus entry that lead to the onychomycosis.

A case of a 54-year-old woman exhibiting the yellow nail syndrome with exudative pleural effusions, lymphoedema, pericardial effusion, rhinosinusitis and bronchiectasis is described. This case illustrates the rather benign course of the syndrome over ten years. A review of the literature is provided. The etiology of the syndrome is obscure, while the pathogenesis seems to involve impaired lymphatic drainage. Two cases of yellow nail syndrome were reported with a review of the literature. The yellow nail syndrome is a rare disease, typically characterized by the trias of scleronychia (yellow nails), peripherical lymphostasis and pleural effusions. In 25 % the cases are accompanied by sinusitis. In the beginning of the disease the affects of the sinus are the main problem. For this reason otolaryngologists are often the first medical doctors faced with the treatment of yellow nail syndrome.

Although nail abnormalities were the first sign to be noticed, this syndrome is now known to involve multiple organ systems and its association with other diseases is well described. Centromedullary nailing is a well-established method of treatment for diaphyseal long bone fractures. The indications have been broadened greatly since the introduction in 1974 of interlocking centromedullary nailing. The purpose of this paper is to review our first results with locked intramedullary nailing of the tibia. A composite of nail and its skin appendages can be transferred for the foot to the hand by microvascular anastomosis. For technical reasons, the most suitable donor nail is from the big toe, and this most closely approximates the thumb nail. Heritable nail diseases are associated with a great variety of inherited diseases and syndromes. Well over 60 HEDs and numerous other inherited nail abnormalities are known to exist.
