Uvular tip necrosis following routine diagnostic oesophagogastroduodenoscopy.

Uvular necrosis is an uncommon complication following oesophagogastroduodenoscopy (OGD). We report a case involving a male in his 20s who developed uvular necrosis following a routine OGD performed under conscious sedation. The patient presented three days postprocedure with a worsening sore throat and odynophagia. Examination revealed an erythematous uvula with a necrotic tip. He was managed conservatively with simple analgesia, topical lidocaine and oral antibiotics with eventual full resolution of symptoms with no lasting sequelae. Massive hepatic necrosis (MHN), characterised by extensive loss of hepatocytes, represents the most severe pathological lesion in acute liver failure (ALF). [Fatal epidermal necrosis due to methotrexate use].

A 53-year-old patient with known psoriasis presented with painful erosive plaques in existing psoriasis lesions, ulcerations in the mouth, and leukopenia. She used 10 mg of methotrexate daily without folic acid prophylaxis. During admission she developed pancytopenia, liver failure and acute kidney injury. She died after developing toxic shock syndrome. We aim to describe the use and toxic effects of methotrexate, how to recognise toxicity clinically and histologically, and treatment options.

OBJECTIVE: To evaluate the effectiveness of intraluminal drainage of acute fluid accumulations for infected pancreatic necrosis. Scrotal necrosis after cobra (Naja annulifera) envenomation.

A 47-year-old man received a snouted cobra (Naja annulifera) snake bite to his genitals while on holiday in South Africa. His penis and scrotum were noted to be swollen, deep purple in color, and painful on hospital admission. Scrotal necrosis was diagnosed, and he received multiple doses of a non-specific snake venom antiserum and broad-spectrum antibiotics. Although no neurological sequelae developed, he did require hemodialysis due to acute kidney injury. After stabilizing, he was repatriated to the Netherlands for further treatment and has since made a full recovery.

It usually presents with sore throat, fever, foreign-body sensation, and odynophagia following esophagogastroduodenoscopy.
