Therapeutic hypothermia is a standard of care for infants ≥36 weeks gestational age (GA) with moderate-to-severe hypoxic-ischemic encephalopathy. Because some studies included infants born at 35 weeks GA, hypothermia should be considered if they meet other criteria. Cooling for infants <35 weeks GA is not recommended. Passive cooling should be started promptly in community centres, in consultation with a tertiary care centre neonatologist, while closely monitoring the infant's temperature. Best evidence suggests that maintaining core body temperature between 33°C and 34°C for 72 hours, followe

In many preterm infants, the ductus arteriosus remains patent beyond the first few days of life. This prolonged patency is associated with numerous adverse outcomes, but the extent to which these adverse outcomes are attributable to the hemodynamic consequences of ductal patency, if at all, has not been established. Different treatment strategies have failed to improve short-term outcomes, with a paucity of data on the correct diagnostic and pathophysiological assessment of the patent ductus arteriosus (PDA) in association with long-term outcomes. Echocardiography is the selected method of cho

The article presents a review of the data, which reflects the relevance and modern views on the problem of retinopathy of prematurity, inclu … g in the office catamnesis are itemized. The importance of interaction between ophthalmologists and neonatologists at all stages of neonatal screening for providing timely specialized care in order to minimize the occurrence of retinopathy and prevent progression to disabling stages, as well as to improve the quality of life of these patients is shown.

Advances in the care of high-risk newborn babies have contributed to reduced mortality rates for premature and term births, but the survivin … Provision of dedicated neurocritical care to newborn babies requires a collaborative effort between neonatologists and neurologists, training in neonatal neurology for nurses and future generations of care providers, and the recognition that common neonatal medical problems and intensive care have an effect on the developing brain.

It is essential to start enteral nutrition early to preterm infants by giving small amounts of milk (preferably human milk) to ensure that metabolic homeostasis is kept stable and to limit postnatal growth retardation. Increasing feeding volumes to reach "full enteral feeding" is limited by individual feeding tolerance. Feeding intolerance is extremely common in premature infants. The most frequent signs of a suspect feeding intolerance are the presence of gastric residuals, abdominal distension and the onset of crises of apnea/bradycardia. Gastric residuals are probably a benign consequence o

Retinopathy of prematurity (ROP) is a vasoproliferative disorder affecting low birth weight infants. Although timely diagnosis and treatment can significantly reduce the risk of severe complications, ROP remains a leading cause of childhood blindness worldwide. Limitations of current disease management strategies include extensive travel and logistical coordination requirements for ophthalmologists and neonatologists, decreasing availability of adequately trained ophthalmologists at the point of care, variability in how retinal findings are diagnosed and documented, and a growing need for ROP …

Diabetological care in pre-gestational (preGDM) and gestational (GDM) pregnant diabetic is an indispensable factor for the reduction of risk of mother, fetus and newborn complications. The adoption of an intensified therapy, aiming at maintaining almost normal glycemic values, and the improvement in fetal monitoring have led to a marked reduction, for instance, in fetal intrauterine death and in perinatal mortality rate in general. Furthermore, pregnancy programming allows good glycemic control since conception and a decrease in malformations. It is therefore, necessary to create interdiscipli

To evaluate whether the dramatic rise in the cesarean section rate for breech presentation (from 22% in the decade 1963 to 1973 to 94% in 1978 and 1979) has been justified, 770 term breech deliveries at the Royal Victoria Hospital during the period 1963 to 1973 and during 1978 and 1979 have been reviewed. Neonatal outcome, as determined by one neonatologist during the entire period of this study, was analyzed according to vaginal or cesarean section breech deliveries of primiparous and multiparous women. Knowledge of the changes during transition from fetal to neonatal circulation is useful for the understanding of physiologic and pathologic variations in the neonatal cardiovascular system. Cyanosis as well as clinical symptoms of congestive heart failure in the neonate are unspecific and may represent cardiac and noncardiac disease (primary pulmonary disease, septicemia, intracranial haemorrhage or meningitis, polyglobulism). Besides detailed analysis of clinical symptoms, the examination of arterial bloodgases when breathing from room air and 100% oxygen, the electrocardiogram, the chest-roe

The analysis of gastric fluid from newborns for lecithin/sphingomyelin (L/S) ratio evaluation is useful in determining whether respiratory distress will follow an early delivery. Without exception, the L/S ratios obtained on 14 gastric aspirates paralleled and were slightly higher than intrapartum values obtained from corresponding amniotic fluids (5.6 vs 3.7). Of 46 normal infants, only 1 had a gastric aspirate L/S ratio less than 2:1. No false positive results were found. The obstetrician can easily obtain gastric fluid at delivery, and by ordering an L/S ratio determination, the neonatologi

METHODS: A web-based questionnaire was sent to the perinatal centres enrolled in the Japanese Neonatologist Association and the Japanese Society of Perinatal and Neonatal Circulatory Management. The enrollment window was between the 21st of July and the 31st of December 2021.
RESULTS: This survey covered 59% of the general perinatal centres in Japan. The study revealed that 64% of respondents would not attempt early continuous positive airway pressure strategy for extr

Newborns with hemophilia are at risk of intracranial hemorrhage, extracranial hemorrhage, and other bleeding complications. The safe delivery of a healthy newborn with hemophilia is a complex process that can begin even before conception, and continues throughout pregnancy, birth, and the newborn period. This process involves the expectant parents and a wide variety of health-care professionals: genetic counselors, obstetricians, neonatologists, pediatricians, radiologists, adult and pediatric hematologists, and nurses with expertise in hemophilia. Because of this multidisciplinary complexity,…

Physiologic alterations of the oral and vulvovaginal mucosal surfaces result from the profound hormonal and immunologic changes of gestation. High estrogen levels are responsible for the vascular changes noted on mucosal surfaces. Gingival hyperemia and edema, gingivitis and pyogenic granuloma are the most common alterations of the oral mucosa during gestation. Physiologic changes of the vulvovaginal area are mainly of vascular nature, and include among others varicose veins. The oral and vulvovaginal mucosal surfaces can be affected by diseases that can worsen or develop in pregnancy. Oral le

We present a case of a newborn from a remote, underserved area in the inland of Paraíba, a state from Northeast Brazil. She presented with clinical cyanosis at birth. With the aid of telemedicine, a neonatologist under online cardiology supervision performed a screening echocardiogram. The session established the diagnosis of simple transposition of the great vessels in the baby's first few hours of life. During the same telemedicine session, the necessary arrangements for transferal to a larger maternity center took place. The baby was maintained stable on prostaglandins and was subsequently
