Systemic mastocytosis (SM) is a hematopoietic neoplasm characterized by pathologic expansion of tissue mast cells in one or more extracutaneous organs. In most children and most adult patients, skin involvement is found. Childhood patients frequently suffer from cutaneous mastocytosis without systemic involvement, whereas most adult patients are diagnosed as suffering from SM. BACKGROUND: Basal cell carcinoma (BCC) is the most frequent cancer worldwide in humans. The risk factors reported in the literature encompass excessive sun exposure, genetic predisposition, irradiation, exposure to arsenic, and trauma. The exact role of trauma in the etiology of BCC remains unexplained.
OBJECTIVES: To analyze patients with BCC treated surgically in the Dermatosurgery Unit, looking for possible cases of BCC secondary to trauma.

We present an unusual case of basal cell carcinoma (BCC) arising from a non-sun-exposed area. The patient was 69-year-old male with an enlarging giant fungating mass protruding from his scrotum for which he did not seek medical treatment until recently. The mass did not involve the scrotum or epididymis and was confirmed on ultrasound. The patient underwent wide surgical excision and was diagnosed with BCC of the scrotum. Scrotal BCC appears to be more aggressive and more likely to metastasize compared with lesions arising from other areas of the body.

A case of basaloid carcinoma of the right epididymis in a 22-year-old Saudi man is described. The tumor presented as a painless swelling of 2 months' duration. Right inguinal orchiectomy has been performed. Microscopically, the stroma was widely infiltrated by tumor cells, with marked apoptosis and necrosis. The tumor cells were rather uniform and basaloid and contained many abnormal mitoses. The tumor was positive for keratin and contained tight intercellular junctions by ultrastructural examination. The patient was tumor free at last follow-up.
