Primary gingival leiomyosarcoma. A clinicopathological study of 1 case with prolonged survival.

BACKGROUND: Leiomyosarcoma is a relatively uncommon mesenchymal tumor that exhibits smooth-muscle differentiation. Only 3 to 10% of leiomyosarcomas arise in the head and neck, the nose and paranasal sinuses, skin and subcutaneous tissue and cervical esophagus being the most common localizations. Most leiomyosarcomas involving the oral tissues primarily affect the maxillary sinus, the maxillary or mandibular bone. A review of the English-language literature since 1908 revealed 30 reported cases of primary leiomyosarcoma of the oral mucosa and soft tissues. MATERIAL AND METHODS: We report on a

A Case Report on the Diagnostic Challenge of Mandibular Osteolipoma.

Osteolipoma is an extremely rare histological variant of lipoma characterized by the presence of mature adipose tissue admixed with well-formed lamellar bone. Its occurrence in the oral cavity is exceptional, often posing a diagnostic challenge due to its nonspecific clinical and radiographic features. We report a rare case of osteolipoma in a 40-year-old male who presented with a slow-growing, painless swelling on the right side of the mandible of six months' duration. Clinical examination revealed a firm, mobile, non-tender mass causing facial asymmetry and intraoral vestibular obliteration.

Tumor-like Lesions of Bone and Soft Tissues and Imaging Tips for Differential Diagnosis.

In the musculoskeletal system, tumor-like lesions may present similar imaging findings as bone and soft tissue tumors and can be defined as tumors on radiologic examinations. Misinterpretation of the imaging findings can lead to inappropriate clinical management of the patient. There is still some debate regarding the pathophysiology and origin of tumor-like lesions that include congenital, developmental, inflammatory, infectious, metabolic, reactive, posttraumatic, post-therapeutic changes, and some miscellaneous entities causing structural changes. Although tumor-like lesions are historically

MRI for Diagnosing Musculoskeletal Tumors: A Qualitative and Quantitative Analysis of Tissue Composition.

Musculoskeletal (MSK) tumors are neoplasms that arise in the bones, cartilage, muscles, tendons, nerves, and synovium. These lesions are highly diverse, comprising various tissue components such as mature fat, fluid, hematoma, and fibrous tissue. MRI is a crucial modality for diagnosing MSK tumors, providing both qualitative and quantitative assessments of tissue composition. The key tissue components of MSK tumors that are identifiable on MRI include mature fat and fluid. However, hyperintense mimickers-such as subacute hematomas on T1-weighted image (T1WI) and pseudocystic lesions on T2-weig

Bioengineeredin vitrobone scaffolds to investigate bone metastases: a systematic review of mechanical and biological model validation.

One of the most common sites of cancer metastasis is the bone, with a large proportion of both breast cancer and prostate cancer patients who develop metastases having involvement of the skeleton. The prognosis for patients with bone metastases is poor as there are limited effective treatment options. The lack of reliable models to recapitulate the native bone micro-environment during the drug discovery process, has resulted in a poor understanding of the biological processes that enable and drive metastases, and difficulty evaluating potential treatments. Animal models that have been successf

Skeletal involvement in tumor-induced osteomalacia.

Tumor-induced osteomalacia (TIO) is an ultrarare paraneoplastic syndrome of abnormal phosphate and vitamin D metabolism secondary to the overproduction of fibroblast growth factor 23 by small-sized mesenchymal tumors typically located in soft tissues and bone. The tumor has adverse effects on bone and patients complain of skeletal symptoms and, in severe cases, they suffer multiple devastating fractures. Specific features may characterize the histology of tumors located in bone with respect to those found in extra-skeletal sites. Indeed, the matrix may contain foci resembling primitive cartila

Nasal chondromesenchymal hamartoma of infancy: the first Japanese case report.

The first Japanese case of nasal chondromesenchymal hamartoma, a rare infantile nasal neoplasm, is presented. A 4-month-old Japanese boy was referred to our Centre because of intranasal mass and oculomotor disturbance. Radiological images showed the intranasal tumor extending to the paranasal sinus, orbit, and anterior frontal fossa. Subtotal resection and radiation therapy to residual tumor were performed. There has been no recurrence of the tumor for 13 years. Histologically, the lesion demonstrated admixture of various mesenchymal elements, including cellular spindle cell stroma with occasi

Pseudomalignant myositis ossificans. A pathological study of eight cases.

Eight cases of pseudomalignant myositis ossificans are presented. There was a male predominance and four patients were under 20 years of age. There was no history of trauma, but there was evidence of mild local or general inflammation with positive roentgenograms in two cases. Histologically there was active fibroblastic proliferation, appearing to develop essentially in fibrin deposits with metaplasia leading to the formation of an unusual osteoid. The newly formed mass was surrounded by bone and compressed the surrounding muscle fibers, which were either atrophic or trapped therein. The hist

Primary Bone Lymphoma: A Review of the Literature with Emphasis on Histopathology and Histogenesis.

Primary bone lymphoma (PBL) is a rare neoplasm of malignant lymphoid cells presenting with one or more bone lesions without nodal or other extranodal involvement. It accounts for approximately 1% of all lymphomas and 7% of malignant primary bone tumors. Diffuse large B-cell lymphoma (DLBCL), not otherwise specified (NOS) represents the predominant histological type and constitutes over 80% of all cases. PBL may occur at all ages with a typical diagnosis age of 45-60 years and a slight male predominance. Local bone pain, soft tissue edema, palpable mass and pathological fracture are the most co

A 50-Year-Old Man Presenting with Multiple Bone Lesions and a Diagnosis of Phosphaturic Mesenchymal Tumor of the Femur.

BACKGROUND Phosphaturic mesenchymal tumor (PMT) is an extremely rare mesenchymal neoplasm that is commonly seen in bone and soft tissue. It is associated with a paraneoplastic syndrome, oncogenic osteomalacia, due to tumor-induced urinary phosphate wasting. It is demonstrated to be predominantly mediated by fibroblast growth factor 23 (FGF23)/fibroblast growth factor receptor 1 (FGFR1) axis. Clinically, PMT usually presents as a solitary lesion in the bone. The diagnosis of PMT is challenging due to its non-specific clinical manifestation, radiologic findings, and morphological features. Immunohistochemical characterization of mast cell disease in paraffin sections using tryptase, CD68, myeloperoxidase, lysozyme, and CD20 antibodies.

To date, the diagnosis of mast cell disease (MCD) relied on routine plus histochemical stains. Its differential diagnosis, however, includes a variety of other hematopoietic and particularly B-cell lymphoid neoplasms that are best identified in paraffin sections using immunostains. To determine the paraffin-section immunoreactivity of MCD, 20 specimens from 14 patients with MCD and 1 bone marrow sample (from a patient with probable MCD) that showed equivocal metachromasia, were stained with antitryptase, CD68 (KP-1), CD20 (L26), antilysozyme, and antimyeloperoxidase antibodies. Angiomyolipomatous Lesions of the Nasal Cavity (Sinonasal Angioleiomyoma with Adipocytic Differentiation): A Multi-Institutional Immunohistochemical and Molecular Study.

PURPOSE: Mesenchymal neoplasms composed of vascular, smooth muscle, and adipocytic components are uncommon in the nasal cavity. While angioleiomyoma (AL) is a smooth muscle tumor in the Head & Neck WHO classification, it is considered of pericytic origin in the Skin as well as Soft Tissue and Bone classifications. For nasal AL with an adipocytic component, the terms AL with adipocytic differentiation and angiomyolipoma (AML) have been applied, among others. AML is a type of perivascular epithelioid cell tumor (PEComa), most often arising in the kidney, sometimes associated with the tuberous sc

Phosphaturic Mesenchymal Tumors: Histologic Mimics and Updates.

Phosphaturic mesenchymal tumor (PMT) is a rare bone or soft tissue tumor. It exhibits distinctive morphologic features, including bland spindled cells, highly vascularized stroma, "smudgy or grungy" calcified matrix, and osteoclast-like giant cells. It is characterized by paraneoplastic tumor-induced osteomalacia through producing phosphatonins, especially FGF23. Recent findings suggest that its tumorigenesis and FGF23 overproduction depend on the FGFR1 pathways, which may be activated via FN1::FGFR1 or FN1::FGF1 fusion, as well as by α-Klotho overexpression in the majority of fusion-negative

Rare Osteochondroma of the Posterior Talar Process: A Case Report.

Osteochondromas are benign bone tumors that arise from divergent cartilage formation, most commonly in childhood versus adulthood. We report the case of a healthy 42-year-old woman who presented with an unusual solitary posterolateral ankle mass with associated pain and ankle impingement with 6-week follow-up. The patient was successfully treated with open surgical excision, with bone sent for pathologic diagnosis of benign osteochondroma. The patient returned to normal baseline function with no pain at 6-week follow-up. An open posterior ankle incision approach was performed to remove suspici

[Biopsy of musculoskeletal tumors : Complications and avoidable errors].

Biopsy followed by histopathological assessment is the key procedure to establish the correct diagnosis of unclear bone or soft tissue tumors. There are several possibilities to obtain a biopsy specimen. The indication for biopsy should be established in a specialized center, as should the type of biopsy (fine needle, incisional, excisional, percutaneous CT-guided/navigated biopsy), which must be performed according to established guidelines. The tumor biopsy must be representative and adequate in terms of quantity, to enable a conclusive histopathological diagnosis and planning of appropriate

Oncogenic osteomalacia associated with soft tissue chondromyxoid fibroma.

Oncogenic osteomalacia is a rarely described clinical entity characterized by hypophosphatemia, phosphaturia, and a low concentration of 1,25-dihydroxyvitamin D(3). It is most often associated with benign mesenchymal tumor and can be cured with surgical removal of the tumor. In this paper, we present a case of oncogenic osteomalacia caused by chondromyxoid fibroma in the soft tissue of the sole of the foot in a 56-year-old woman.

Standards of Pathology in the Diagnosis of Systemic Mastocytosis: Recommendations of the EU-US Cooperative Group.

Pathology plays a central role in the diagnosis of systemic mastocytosis (SM), its delineation from other neoplasms and reactive conditions, and in monitoring of SM under therapy. The morphologic hallmark of SM is the accumulation of spindle-shaped, hypogranulated mast cells (MCs) in bone marrow (BM) and other extracutaneous tissues. Four of the 5 World Health Organization-defined diagnostic criteria (ie, compact MC aggregates [=major criterion]; atypical MC morphology; activating KIT point mutations; aberrant expression of CD25 and/or CD2 and/or CD30 in MCs [=minor criteria]) can be addressed
