Nephrology's Next Frontier: Expanding the Reach of CAR T-Cell Therapy for Refractory Lupus Nephritis and Beyond.

Chimeric antigen receptor (CAR) T-cell therapy has recently evolved beyond cancer therapy's boundary to treating autoimmune diseases such as lupus nephritis. In CAR T-cell therapy, the genetically engineered patient's T cells express a receptor specifically targeting antigens such as CD19, a protein found on the surface of B cells. By directing the immune system to eliminate B cells, which play a central role in the pathogenesis of systemic lupus erythematosus, CAR T-cell therapy offers a novel and potent approach to resetting the immune system and achieving remission in difficult-to-treat lup

Introducing Exome Sequencing as Part of the Diagnostic Algorithm for Pediatric Nephrology Patients in Bulgaria: A Single-Center Experience.

INTRODUCTION: In pediatric kidney patients, where clinical presentation is often not fully developed, and renal biopsy is too risky or inconclusive, it may be difficult to establish the underlying pathology. In cases such as these, genetic diagnosis may be used to guide treatment, prognosis, and counseling. Given the large number of genes involved in kidney disease, introducing next-generation sequencing with extended gene panels as part of the diagnostic algorithm presents a viable solution. METHODS: A cohort of 87 consecutive independent cases (83 children and 4 terminated pregnancies) with

Nephrological Detrimental Impacts Resulting From Novel Immunotherapy Drugs Used in the Treatment of Cancer: A Systematic Review.

The most recent advancements in cancer therapy center on efficiently and conveniently enhancing a patient's natural immune system. Immune checkpoint inhibitors (ICIs) are antibodies that target cytotoxic thymus (T) lymphocyte antigen-4 (CTLA-4) and its receptor. They function by stimulating T-cell activity against malignancies. Immune-related adverse events (irAEs) are a distinct class of inflammatory side effects that are specific to a given organ. Antineoplastic medications can impact any part of the kidney, leading to the development of proteinuria, hypertension, electrolyte abnormalities,

Diabetic kidney disease in type 2 diabetes: a consensus statement from the Swiss Societies of Diabetes and Nephrology.

Diabetic kidney disease is highly prevalent in patients with type 2 diabetes and is a major cause of end-stage renal disease in Switzerland. Patients with diabetic kidney disease are among the most complex patients in diabetes care. They require a multifactorial and multidisciplinary approach with the goal to slow the decline in glomerular filtration rate (GFR) and cardiovascular morbidity. With this consensus we propose an evidence-based guidance to health care providers involved in the care of type 2 diabetic patients with diabetic kidney disease.First, there is a need to increase physician

The impact of disease duration on quality of life in children with nephrotic syndrome: a Midwest Pediatric Nephrology Consortium study.

BACKGROUND: The Patient Reported Outcomes Measurement Information System (PROMIS) II is a prospective study that evaluates patient reported outcomes in pediatric chronic diseases as a measure of health-related quality of life (HRQOL). We have evaluated the influence of disease duration on HRQOL and, for the first time, compared the findings of the PROMIS measures to those of the PedsQL™ 4.0 Generic Scales (PedsQL) from the PROMIS II nephrotic syndrome (NS) longitudinal cohort. METHODS: This was a prospective study in which 127 children (age range 8-17 years) with active NS from 14 centers wer

Canadian Society of Nephrology Commentary on the 2012 KDIGO clinical practice guideline for glomerulonephritis: management of glomerulonephritis in adults.

The KDIGO (Kidney Disease: Improving Global Outcomes) clinical practice guideline for management of glomerulonephritis was recently released. The Canadian Society of Nephrology convened a working group to review the recommendations and comment on their relevancy and applicability to the Canadian context. A subgroup of adult nephrologists reviewed the guideline statements for management of glomerular disease in adults and agreed with most of the guideline statements developed by KDIGO. This commentary highlights areas for which there is lack of evidence and areas in need of translation of evide…

Studies on preventive nephrology: pattern of the subsets of hypertension in the paediatric, adolescent and adult population of Gassim, Saudi Arabia.

As part of our studies in preventive nephrology, we have recorded the casual blood pressure during a total population cross-sectional survey of the Faizia East Primary Health District (FEPHD). A total of 5671 subjects had adequate records. Of these 2222 were adults (> 19 years). The prevalence of systemic hypertension (HPN) was calculated, using as cut-off levels, > or = 140/90 for the adult population and > or = 95th percentile as recommended by the Task Force for Blood Pressure Control in Children (1987) for the paediatric and adolescents (3-18 years). Using these definitions without modific…

[Prenatal ultrasound diagnosis in congenital abnormalities of the kidneys and efferent urinary tract from the pediatric nephrology viewpoint].

Antenatal foetal abnormalities are found in 0.5% of all pregnancies. 30-50% of these abnormalities are related to the urinary tract. Although congenital abnormalities of the urinary tract can be detected as early as in the 15th-18th weeks of gestation, most abnormalities are detected between the 28th-30th weeks of gestation. At this time irreversible damage of the kidneys has taken place and the foetus will not benefit from antenatal therapy. On the other hand, a foetus with late onset of obstruction will benefit from antenatal intervention. Repeated ultrasonography is crucial for initiating f

Renal disease in adolescents with type I diabetes mellitus: a report of the Southwest Pediatric Nephrology Study Group.

Although 30% to 40% of patients with type I diabetes mellitus develop diabetic nephropathy, the usual signs of clinical nephropathy are often thought to be delayed until adulthood. We studied 13 adolescents with type I diabetes mellitus for 5 to 14 years who had renal biopsies completed because of clinical problems, including proteinuria, hematuria, or hematuria plus proteinuria. Changes typical of diabetes were seen by light and immunofluorescence microscopy; evidence of other renal diseases was not noted. On electron microscopy evaluation, glomerular basement membrane width was increased in

Focal segmental glomerulosclerosis in children with idiopathic nephrotic syndrome. A report of the Southwest Pediatric Nephrology Study Group.

Clinicopathologic correlations were examined in 75 children with focal segmental glomerulosclerosis (FSGS) associated with idiopathic nephrotic syndrome. The biopsy specimens of all patients were examined by electron microscopy (69 patients) or immunofluorescence microscopy (67 patients) in addition to light microscopy. Fifty-three patients (group A) had FSGS diagnosed on their first biopsy; 22 patients (group B) had one to three previous biopsies showing minimal glomerular changes or mesangial hypercellularity prior to the demonstration of FSGS on a subsequent biopsy. Considerable homogeneity

Comparison of Urinary Isolate Distribution and Antimicrobial Resistance Profiles in Kidney Transplant Recipients and Non-Transplant Nephrology Patients.

Background/Objectives: Urinary tract infections (UTIs) are common infectious complications in kidney transplant recipients (KTRs) and non-transplant nephrology patients. However, direct culture-based comparisons of urinary isolate distribution and antimicrobial resistance profiles between KTRs and non-transplant nephrology (NTN) patients remain limited. This study compared urinary isolates recovered from urine cultures with significant growth and their in vitro antimicrobial resistance profiles in hospitalized KTRs and NTN patients. Methods: In this retrospective culture-based study, urine cul…
