Immunoglobulin G4-related Pleuritis Complicated with Minimal Change Disease.

A 70-year-old woman with bilateral pleural effusion and respiratory failure was admitted to our hospital. Nephrotic syndrome due to minimal change disease had been diagnosed four months before admission. Because blood tests and a pleural fluid analysis did not reveal the etiology of her condition, we performed a video-assisted thoracoscopic pleural biopsy. No specific thoracoscopic findings were noted. The pathological findings revealed an increase in immunoglobulin G4 (IgG4)-positive cells; IgG4-related pleuritis was diagnosed. Her pleuritis improved with oral corticosteroid therapy. A furthe

Executive summary of the KDIGO 2021 Guideline for the Management of Glomerular Diseases.

The Kidney Disease: Improving Global Outcomes (KDIGO) Clinical Practice Guideline for the Management of Glomerular Diseases is an update to the KDIGO 2012 guideline. The aim is to assist clinicians caring for individuals with glomerulonephritis (GN), both adults and children. The scope includes various glomerular diseases, including IgA nephropathy and IgA vasculitis, membranous nephropathy, nephrotic syndrome, minimal change disease (MCD), focal segmental glomerulosclerosis (FSGS), infection-related GN, antineutrophil cytoplasmic antibody (ANCA) vasculitis, lupus nephritis, and anti-glomerula

Nephrotic syndrome associated with Kimura's disease: a case report and literature review.

BACKGROUND: Kimura's disease (KD) is a rare chronic inflammatory disorder with a high incidence of renal involvement. In this report, we present a case study of KD-associated nephrotic syndrome combined with minimal change disease (MCD) and acute renal tubular injury. Meanwhile, the clinical and histopathological characteristics of 26 patients with KD presenting with renal involvement were retrospectively evaluated. CASE PRESENTATION: Here, we report a case study of a 59-year-old male patient with KD confirmed by a lymph node biopsy. He developed widespread edema and decreased urine output. A

[Incidental mesangial IgA deposition in minimal change nephrotic syndrome(MCNS)].

The frequency of incidental mesangial IgA deposition and its clinicopathological features were investigated in patients with minimal change nephrotic syndrome(MCNS). Mesangial IgA deposition was present in 15/63 patients(23.8%), and co-deposition of IgA and C3 was present in 10/63 patients(15.9%). The serum IgA concentration was significantly higher in IgA(+) patients than in IgA(-) patients(341 +/- 79 mg/dl vs. 252 +/- 99 mg/dl, p = 0.034). The urinary red blood cell count tended to be higher in IgA(+) patients than in IgA(-) patients (12.8 +/- 24.9 vs. 5.0 +/- 7.9 counts/HPF, p = 0.58). Hist

Pulmonary thrombosis in a 10-year-old child with minimal change disease and nephrotic syndrome. A clinical, radiologic, and pathologic correlation with literature review.

The nephrotic syndrome has long been recognized as a hypercoagulable state. Arterial thrombosis is a rare complication of the syndrome. Diuretics and steroids, standard treatment for exacerbations, have been implicated as contributing to the development of arterial thrombosis. The authors present the pathologic, clinical, and radiologic findings of a patient with nephrotic syndrome and minimal change disease. The patient died of pulmonary thrombosis while on high-dose steroid therapy for an acute exacerbation of proteinuria following a recent hospital admission for chest pain and dyspnea.

Rheumatoid factors in patients with minimal change nephrotic syndrome.

Several immunological aberrations, especially T-cell abnormalities, have been shown to be involved in the development of the minimal change nephrotic syndrome (MCNS) although the responsible factors have not been identified. We studied rheumatoid factors (RF), thought to be a consequence of polyclonal B-cell hyperactivation, in sera from patients with MCNS using a sensitive fluorescence immunoassay (FIA). Significant increases of IgG and IgM RFs and marginal increase of IgA RF were observed during the nephrotic stages. Some patients still showed high titers of IgM RF in the early remission pha

[An aged woman with minimal change nephrotic syndrome complicated with reversible acute renal failure].

Reported was an aged woman (80-year-old) of minimal change nephrotic syndrome which was complicated with reversible oliguric acute renal failure. The patient presented massive proteinuria, anasarca, and severe azotemia. She recovered conservatively from the acute renal failure and subsequently remitted from the nephrotic syndrome after the treatment which comprised albumin infusion, diuretics, adrenocorticosteroid hormones (including the pulse therapy), antiplatelet drug, and anticoagulants. The histopathologic findings of renal biopsy were compatible with minor glomerular abnormalities and ac

[Comparison of automated micro-disc electrophoresis for the analysis of urine proteins with a conventional system].

Recently a new system for analyzing urinary proteins due to their molecular weight distribution has been introduced. The whole system is automatically driven and uses a silver staining. Due to the small amount of protein necessary for analysis native urine samples can be used. The data presented here show that the automatic micro-disc-electrophoresis (Phast-system) is very useful for any routine testing due to the quick (2 hours) and easily obtainable results with a very high reproducibility. The clinical relevance of urinary protein analysis seems to be much higher using this system.

[Will cyclosporin A be used in the treatment of refractory nephrotic syndromes?].

In the treatment of nephrotic syndromes complicating primary glomerulonephritis anti-inflammatory and immunosuppressive action of glucocorticoids is used most frequently. Cases refractory to corticotherapy are a difficult clinical problem and lead, most frequently, to renal failure. Recently trials have been carried out of replacing glucocorticoids with the immunosuppressant which is devoid of the dangerous side effects of cytostatics, i.e. cyclosporin A. Theoretical and experimental data relevant to the use of cyclosporin A in the treatment of glomerulopathies are presented and the results of

Steroid-dependent nephrotic syndrome with minimal glomerular changes and mesangial IgA deposits.

We report 3 children with steroid-dependent nephrotic syndrome, without hematuria. The renal biopsies showed minimal-change disease with mesangial IgA deposits. Their clinical presentation and therapeutic responses were similar to our cases of biopsied nephrotic syndrome with minimal glomerular abnormalities. We believe that our cases have minimal-change disease and not Berger's disease with nephrotic syndrome.

Waldenström macroglobulinemia and nephrotic syndrome with minimal change lesion.

A case of Waldenström macroglobulinemia with nephrotic syndrome is reported. Kidney biopsy, revealing only foot process fusion and response to steroid therapy, fits minimal change disease.
