Background: Spinal cord injury (SCI) often results in upper extremity paralysis, significantly limiting independence and quality of life. Su … performed due to heterogeneity. Results: Thirty studies met the inclusion criteria: 15 focussed on nerve transfers (208 patients, 319 limbs) and 15 on tendon transfers (360 patients, 447 limbs). The most common nerve transfers were brachialis to the anterior interosseous nerve (AIN) and supinator to the posterior interosseous nerve (PIN). Tendon transfers most …

PURPOSE: The motor branch to the brachialis muscle has been described as a versatile donor in upper limb nerve reconstruction. This includes … er limb involvement, in which selective denervation of spastic elbow flexors could be combined with nerve transfer to paretic wrist and/or finger extensors.
METHODS: Sixteen fresh-frozen cadaveric upper limbs were dissected to establish the feasibility of transferring a motor branch of either the biceps or brachialis muscle onto the branch of the extensor carpi …

This study aimed to assess the outcome of supinator to posterior interosseus nerve transfer, followed by tendon transfer, to restore grasp function in patients with mid-to-high-level cervical spine injuries. Fifteen upper limbs in 11 patients with these injuries were included. The outcomes were then compared with a historical control group consisting of 16 upper limbs that received tendon transfers alone. The outcome was assessed using the Canadian occupational performance measure, ability to open the first web space, and grip and pinch strength 12 months after surgery. The Canadian occupation…

There are very few descriptions of tendon transfers designed specifically to address the reconstruction of posterior interosseous nerve palsy (PINP). Unlike a radial nerve palsy (RNP), a patient with a PINP is able to extend their wrist but in radial deviation, because of the preserved innervation of the extensor carpi radialis longus (ECRL). Tendon transfers to restore finger and thumb extension in PINP have been extrapolated from tendon transfers to restore these functions in RNP, specifically using flexor carpi radialis, not flexor carpi ulnaris, so as not to further exacerbate the distinct

Acute flaccid myelitis (AFM) is a polio-like condition predominantly affecting children that is characterized by acute-onset, asymmetric flaccid paralysis, often preceded by a prodromal fever or viral illness. With prompt diagnosis and early surgical referral, nerve transfers may be performed to improve function. Highly selective nerve transfers are ideal to preserve existing functions while targeting specific deficits. In this report, we present a case of a double fascicular nerve transfer of median and ulnar nerve fascicles to the axillary nerve, combined with selective transfer of the spina…

BACKGROUND: Up to 10% of cervical spine surgeries are complicated by postoperative weakness. Although many patients recover with nonoperative management, some require surgery for restoration of function.
OBJECTIVE: To present the indications and outcomes of patients undergoing nerve transfers after developing weakness secondary to cervical spine decompression.
METHODS: A retrospective review of patients from 2 academic medical centers who underwent nerve transfer for C5-6 root injury after cervical spine surgery was performed.
RESULTS: Of the 10 treated patients, 9 experienced recovery at l…

Closed brachial plexus lesions (BPLs) are generally associated with a traumatic mechanism of forced traction between the neck and the shoulder-arm complex. For brachial plexus reconstruction different techniques have been proposed with donor motor nerves like intercostal nerves, or the ipsilateral cervical plexus, the phrenic nerve, the contralateral C7 root, and many others. Despite all these surgical possibilities, the overall recovery is generally poor and not satisfactory. The principal drawback is linked to the loss of upper limb proprioception, in a way that dramatically influences even

OBJECTIVE Transfer of the spinal accessory nerve to the suprascapular nerve is a common procedure, performed to reestablish shoulder motion in patients with total brachial plexus palsy. However, the results of this procedure remain largely unknown. METHODS Over an 11-year period (2002-2012), 257 patients with total brachial plexus palsy were operated upon in the authors' department by a single surgeon and had the spinal accessory nerve transferred to the suprascapular nerve. Among these, 110 had adequate follow-up and were included in this study. Their average age was 26 years (SD 8.4 years), …

BACKGROUND AND AIMS: Various techniques have been investigated to enhance peripheral nerve regeneration including the application of low-intensity electrical stimulation (ES) and the administration of growth factors, especially brain-derived neurotrophic factor (BDNF). The purpose of this study was to investigate the effects of combining short-term (ES) and recombinant adenoviral vector-mediated BDNF (BDNF-Ad) transfer, in comparison to each sole modality, on peripheral nerve regeneration in a rat model with crush-injured sciatic nerve.
METHODS: Sixty male Sprague-Dawley rats (250-300 g) were

In spinal cord injuries at the C6 level, elbow extension is lost and needs reconstruction. Traditionally, elbow extension has been reconstructed by muscle transfers, which improve function only moderately. We have hypothesized that outcomes could be ameliorated by nerve transfers rather than muscle transfers. We anatomically investigated nerve branches to the teres minor and posterior deltoid as donors for transfer to triceps motor branches. In eight formalin-fixed cadavers, the axillary nerve, the teres minor branch, the posterior deltoid branch, the triceps long and upper medial head motor b…

BACKGROUND: Restoration of elbow flexion is commonly achieved with nerve transfer to the musculocutaneous nerve branches after upper trunk brachial plexus injury. It is unknown if double nerve transfer to the biceps and brachialis nerve branches results in greater strength than single nerve transfer to the biceps branch alone.
METHODS: A retrospective review was performed of all patients receiving nerve transfer to restore elbow flexion. Single and double nerve transfers were compared in regard to severity of injury, elbow flexion, supination, grip strengths, and Disabilities of the Arm, Shou…

The purpose of this study in rats was to identify whether a minimal dose of FK506 could enhance nerve regeneration along a 4-cm cross-chest saphenous nerve graft. Our center established a cross-chest nerve regeneration model previously using the contralateral C7 root transfer to the musculocutaneous nerve. Using this model, 10 adult male Sprague-Dawley rats were divided into two groups: group 1 (N = 5) consisted of animals that did not receive any further treatment, and group 2 (N = 5) consisted of animals that received a daily subcutaneous dose of 0.7 mg/kg FK506 for a period of 4 weeks. We have assessed the anatomical feasibility of a transfer of the first intercostal nerve to the supra- and infraspinatus muscles and report on the first clinical application. Ten fresh cadavers were dissected for this study. Histomorphometric analysis showed the fascicular surface area of the first intercostal nerve at its origin (0.38 mm(2)) to be comparable to the suprascapular nerve (0.81 mm(2)). The first intercostal nerve is usually a pure motor nerve. Preservation of the spinal accessory nerve, lack of donor site morbidity and direct suture without nerve graft are the other advantages of

Subcutaneous transfer of the ulnar slip of the extensor digiti minimi (EDM) to the adductor tubercle across the dorsum of the hand restores pinch, and index finger abduction is reproduced by re-routing extensor indicis proprius around the thumb extensor tendons. Six patients with post-traumatic ulnar palsy have been treated by this method with a minimum follow-up period of 40 months. Pinch was improved from an average of 5% to 40-50% of the normal side, and index abduction to 30-40%. There was no donor morbidity.

Author describes two cases: in the first, in consequence of a lesion of the deep peroneal nerve during operation, the development a paralytic drop foot, in the second, in consequence of a lesion of unknown etiology of the common peroneal nerve, paralytic club foot could be observed. In both cases the intraosseal transposition of the tendon of the posterior tibial muscle on the second cuneiform bone was performed with good result.

Nerve transfer surgery may be performed to restore sensorimotor function following peripheral nerve trauma or dysfunction. There is biological plausibility and limited correlative evidence that donor-to-recipient axon count ratios may impact functional outcomes. Collating anatomical studies that report mean axonal count ratios could guide surgical practice. This systematic review aimed to collate anatomical studies reporting mean axonal counts (MAC) of nerve branches used in upper limb nerve transfer surgery, and calculate MAC ratios for donor-to-recipient nerves and pooled MAC ratios between …

PURPOSE: Restoring shoulder function after axillary nerve injury is always a challenge. Transferring a branch of the radial nerve destined to the triceps onto the anterior division of the axillary nerve has become the preferred technique. However, this is not always possible, especially when the axillary nerve is severely injured around the posterior part of the humeral neck. The purpose of this cadaver study was to assess the feasibility of transferring the nerve of the long head of the triceps through an anterior and lateral humeral neck tunnel, directly onto the branch of the anterior divis

BACKGROUND: Results of a single stage technique combining cross facial nerve graft(s) (CFNG) with an ipsilateral end to side nerve to masseter transfer (NTM) in incomplete facial paralysis are assessed in a retrospective cohort study. The hypothesis is that the technique can safely improve the quality of smile in these patients. End to side coaptations for the recipient facial nerve minimise the risk of iatrogenic function loss, contrasting with the end to end neurorrhaphies used in conventional babysitting procedures.
METHODS: A series of 27 patients was studied through case note review and
