'Translation' in medicine immediately suggests 'translational research', but there are many other varieties of 'translation'. I have selected 4 translations in the field of vascular neurology in which I have been involved in different respects: (1) the translation of results from men to women, taking the example of aspirin which, in primary prevention, decreases the risk of myocardial infarction in men and the risk of cerebral infarction in women, the reason for this sex difference being so far unknown; (2) the 'inverse translational research', from bedside to bench, taking the example of the …

BACKGROUND AND OBJECTIVE: The number of neurological second opinions (SO) and tertiary referrals (TR) is increasing. The main purpose of this study was to assess whether a day-care admission made a meaningful contribution to standard neurological outpatient care, for a wide range of second opinions and tertiary referrals.
METHODS: All new patients attending an academic neurological day-care clinic in a 6-month period were investigated. Before admission, all previous medical correspondence and ancillary investigations were reviewed. On the day of admission, extensive time was available for cli

INTRODUCTION: Cochrane Collaboration (CC) provides growing and readily accessible resource to help that decision-making care is based on detailed, critical, and current reviews of the best available evidence. There are many clinical questions for which there is no good evidence on which to base clinical practice.
AIM: To analyse the bibliometric characteristics of the systematic reviews (SR) about neonatal neurology published in Neonatal CC.
MATERIALS AND METHODS: Bibliometric analysis of The Cochrane Database Systematic Reviews in Neonatal CC, Issue 1, 2004 (n = 169 SR). The dependent varia…

'Nurturing the brain' is an emerging research field integrating brain science, child care and education, which also involves child neurology. It has emerged from the recent remarkable progress in brain science and strong social demands for improvements in child care and education. This article reviews the current status of three major research themes in this field. First, developmental disorders represented by attention deficit/hyperactivity disorder, autism and Asperger syndrome often introduce difficulties in child care and education, which are to be addressed by appropriate assessment and t…

The paper outlines the results of mesodiencephalic modulation (MDM) as therapy in adults and children with neurological and mental diseases in the out- and inpatient setting. MDM is a method of transcranial electric therapy that normalizes the performance of the hypothalamohypophyseal and opioid systems of the brain, which ultimately improves the body's adaptive response in disease.

OBJECTIVE: This article examines the historical basis for the divergence of neurology and psychiatry over the past century and discusses prospects for a rapprochement and potential convergence of the two specialties in the next century.
METHOD: The author presents a brief historical overview of developments in neurology and psychiatry from the late 19th century. The histories of research and prevailing scientific opinion on two neuropsychiatric disorders, Alzheimer's disease and Tourette's syndrome, are compared to illustrate the effects of viewing a disease process from, respectively, the ne…

OBJECTIVE: To contribute to a better understanding of a poorly appreciated pioneer of therapeutic neurology, Mary Broadfoot Walker, MD.
BACKGROUND: At a time when the treatment of myasthenia gravis (MG) was "a source of discouragement to the patient and a cause of nightmare for the physician," Mary Walker demonstrated that temporary relief of myasthenic symptoms could be produced by subcutaneous injection of physostigmine or neostigmine (Prostigmin; Roche, Basel, Switzerland). She also pioneered the concept of a circulating factor as the etiology of myasthenia and was the first to report hypo…

We attempted to classify, according to the 1989 International Classification of Epilepsies and Epileptic Syndromes, 1076 patients examined during a 4-year period (1 January 1988 to 31 December 1991) in the Child Neurology Unit. We aimed to determine the proportion of the clearly defined epileptic syndromes or the non-specific categories of the International Classification of Epilepsies and Epileptic Syndromes, to estimate the relative incidence of different categories and to review the criteria for defining epileptic syndromes. The proportion in categories as defined by the International Class…

Painstaking realism is an essential feature of the fiction of Count Leo Tolstoy. One example of Tolstoy's attention to detail is the description of the death of Prince Nicholas Bolkonski in War and Peace. The information provided in War and Peace allows the identification of the prince's terminal illness as a brain-stem stroke and is probably the first description of the one-and-a-half syndrome. Prince Bolkonski is also portrayed as suffering from a dementing process. Tolstoy used the character of Prince Bolkonski to exemplify the rationalistic, Western-influenced aristocracy that dominated Ru

A prospective study of 100 consecutive admissions to a university neurology inpatient service was done to assess the prevalence and spectrum of autoimmune rheumatic disorders, most specifically Sjögren's syndrome (SS), systemic lupus erythematosus (SLE), and lupus anticoagulant (LA)-associated disorders. All patients underwent a physical exam (PE), a rheumatic disease questionnaire, and serologic testing (ANA, RF, and LA). The 100 patients consisted of 36 men and 64 women, aged 17 to 98 (mean 60) years and included 47 white, 45 black and eight other. Definite rheumatic diseases were found in 1…

The authors have elaborated a program of selective screening of hereditary metabolic defects (HMD) ensuring the identification of over 100 disease entities as well as a program of the biochemical diagnosis and prophylaxis of mucopolysaccharidoses. More than 3000 patients who applied for help to the medical-genetic consultative centre were examined. Data on the incidence and genogeography of HMD were obtained.

In times of increasing "sparetime pathology" sequels of sports are getting a new meaning in medicine. Surgeons are mainly involved in this issue but it also contacts neurologists. We differentiate between direct sequels of sports and concomitant incidents of sports. Besides the typical head injuries we saw a number of atypical traumatic damages such as a hit by a golf ball, fall on the buttocks and the header in football. Discprotrusions are mainly understood as concomitant incidents at sports because in most cases one has to consider preexisting disc lesions. Though there are typical motions

INTRODUCTION: The increased survival of adolescent patients with neuromuscular disease (NMD) entails the transition of these patients to adult healthcare. The process of care transition seeks to guarantee the continuity of healthcare, although it is not always well planned. This consensus statement aims to provide a conceptual framework for the care transition of young patients with NMD.
METHODS: A review of the literature on the subject was performed by NMD experts from the Spanish Society of Paediatric Neurology and the Spanish Society of Neurology, and a final consensus statement was draft…

This panel had the objective of recommending evidence-based guidelines for the clinical diagnosis of Alzheimer's disease (AD) in Brazil. Guidelines from other countries and papers on the diagnosis of AD in Brazil were systematically evaluated in a thorough research of PUBMED and LILACS databases. The panel concluded that dementia diagnosis should be based on the DSM criteria and AD diagnosis, on the McKhann et al. criteria (NINCDS-ADRDA). The recommended auxiliary tests are: blood cell count, blood urea nitrogen, serum levels of creatinine, free-thyroxine, thyroid-stimulant hormone, albumin, h

Biological barriers, including the tumor microenvironment and the blood-brain barrier, represent the primary bottleneck for the clinical translation of nanomedicines in oncology and neurology. Ultrasound, characterized by noninvasiveness, deep tissue penetration, and high spatiotemporal precision, provides a transformative platform to address this challenge. This review systematically presents sononanomedicine as an integrated paradigm that couples acoustic physics with nanoplatform engineering to overcome these physiological barriers. We delineate the core physical mechanisms─cavitation, sono…
