Transcranial electric motor evoked potential monitoring during scoliosis surgery in children with cerebral palsy and active seizure disorder: is it feasible and safe?

PURPOSE: Use of spinal cord monitoring in children with cerebral palsy (CP) and neuromuscular scoliosis is challenging. The previous reports suggest low success rates in the setting of CP, and it is unclear if transcranial electric motor evoked potentials (TcMEP) monitoring is contraindicated in patients with an active seizure disorder. The purpose of this study was to determine (1) are patients with CP able to be appropriately monitored with TcMEP? and (2) does TcMEP cause an increase in seizure activity? METHODS: This was an institutional review board-approved retrospective cohort study obs

What do we know about perioperative hypersensitivity reactions and what can we do to improve perioperative safety?

Hypersensitivity reactions are an important aspect of perioperative care and are a crucial interdisciplinary issue in anaesthesiological practice, as well as allergological and laboratory diagnostics. This phenomenon was observed as early as the 1980s and 1990s in Western European countries, and knowledge on this subject has grown significantly over time. Although hypersensitivity reactions are not frequent events (the incidence of perioperative hypersensitivity reactions ranges from 1:386 to 1:13 000 procedures, with higher frequency - 1 per 6500 general anaesthesias with neuromuscular blocki

Homocysteine sensitizes the mouse neuromuscular junction to oxidative stress by nitric oxide.

Homocysteine (HCY), a redox-active metabolite of the methionine cycle, is of particular clinical interest because of its association with various neurodegenerative diseases including amyotrophic lateral sclerosis. It has been previously established that HCY exacerbates damage to motor neurons from reactive oxygen species (ROS) such as hydrogen peroxide. To assess the role of HCY at the mammalian neuromuscular junction, neurotransmission was monitored by electrophysiology at the mouse epitrochleoanconeus muscle. Preparations were preincubated in HCY before inducing ROS and recordings were taken

β2-Adrenoceptor agonists as novel, safe and potentially effective therapies for Amyotrophic lateral sclerosis (ALS).

Amyotrophic lateral sclerosis (ALS) is a chronic and progressive neuromuscular disease for which no cure exists and better treatment options are desperately needed. We hypothesize that currently approved β2-adrenoceptor agonists may effectively treat the symptoms and possibly slow the progression of ALS. Although β2-agonists are primarily used to treat asthma, pharmacologic data from animal models of neuromuscular diseases suggest that these agents may have pharmacologic effects of benefit in treating ALS. These include inhibiting protein degradation, stimulating protein synthesis, inducing ne

Physiological, Neuromuscular, and Perceptual Responses to Matched High-Intensity Intermittent Exercise in Soccer Players Under Fed and Ramadan-Type Fasting Conditions.

Ramadan-type diurnal fasting, which restricts both food and fluid intake from dawn to sunset, may influence physiological, metabolic, and perceptual responses during high-intensity exercise. However, its acute effects under matched external workload conditions in field-based soccer training remain unclear. This study examined the physiological, neuromuscular, metabolic, and perceptual responses to a matched high-intensity intermittent training session performed under fed and Ramadan-type fasting conditions. Twenty male university-level soccer players completed two counterbalanced sessions. Ext

Effect of neuromuscular blockade on opioid consumption in high-risk children undergoing adenotonsillectomy: A randomized-controlled trial.

BACKGROUND: Protective reflexes like coughing occur frequently during pediatric adenotonsillectomy (AT) and are difficult to blunt with anesthetics, potentially leading to higher than necessary opioid administration. Our objective was to evaluate if nondepolarizing neuromuscular blockade reduces opioid consumption and postoperative respiratory events, compared to no blockade, in children at high-risk for respiratory complications after AT. METHODS: We conducted a patient- and assessor-blinded, superiority, randomized, controlled trial at a pediatric hospital between April and September 2024.

Intraoperative use of lateral spread response measurement in the upper orbicularis oculi and mandibular muscles in patients with hemifacial spasm after botulinum toxin treatment.

BACKGROUND: Following repeated botulinum toxin (BTX) treatments, patients with hemifacial spasm (HFS) are recommended to undergo microvascular decompression (MVD) for a permanent cure. Intraoperative lateral spread response (LSR) monitoring is important to improve surgical outcomes. However, LSR monitoring during MVD surgery in HFS patients who have previously received BTX is challenging, since the muscles typically used to assess the LSR are paralyzed. Here, we describe our use of the upper part of the orbicularis oculi and mentalis muscles for intraoperative monitoring of the LSR during MVD

Reducing Perioperative Neurocognitive Disorders (PND) Through Depth of Anesthesia Monitoring: A Critical Review.

General anesthesia has been administered for over 150 years, and in that time, has become progressively safer. Improvements in outcomes have been driven by multiple advances, including the use of non-invasive monitors to assess cardiovascular and respiratory status. More recent advances have included the development and use of monitors to measure neurologic status by means of "processed" electroencephalography (pEEG), wherein the frontal EEG signal is analyzed by proprietary algorithms to produce a dimensionless number (scaled from 0 to 100), wherein low values are associated with deepening le

Feasibility of mechanomyography-based fatigue classification for passive lower-limb exoskeleton evaluation: A pilot study.

Mechanomyography (MMG) enables non-invasive monitoring of muscle mechanical activity, while its utility in time-resolved fatigue detection during dynamic human-exoskeleton interaction remains underexplored. This pilot study explored the feasibility of combining MMG with machine learning to characterize neuromuscular fatigue and evaluate passive lower-limb exoskeleton assistance during repetitive 10 kg squat-lifting tasks. MMG signals from five lower-limb muscles were extracted for time-domain, frequency-domain and nonlinear features, and fatigue identification was implemented via a spectral-ba

An ultrathin gas-permeable imperceptible electronic skin enables maxillofacial electrophysiological monitoring.

Maxillofacial electromyographic (EMG) signals serve as crucial indirect indicators of occlusal function, playing a significant role in clinical oral diagnosis. However, conventional gel electrodes with occlusive nature induce skin irritation and wearer discomfort, which limits long-term monitoring. Here, we developed an ultrathin, gas-permeable, and flexible electronic skin (e-skin) designed for imperceptible, long-term maxillofacial EMG monitoring. This e-skin comprises a stretchable substrate formed by core-shell elastic fibers fabricated via coaxial electrospinning, coated with PEDOT:PSS@MX

Detection of early nocturnal hypoventilation in neuromuscular disorders.

Objective Nocturnal hypoventilation (NH) is a complication of respiratory involvement in neuromuscular disorders (NMD) that can evolve into symptomatic daytime hypercapnia if not treated proactively with non-invasive ventilation. This study aimed to assess whether NH can be detected in the absence of other signs of nocturnal altered gas exchange. Methods We performed nocturnal transcutaneous coupled (tc) pCO2/SpO2 monitoring in 46 consecutive cases of paediatric-onset NMD with a restrictive respiratory defect (forced vital capacity < 60%). Nocturnal hypoventilation was defined as tcPCO2 > 50 m

Statins, neuromuscular degenerative disease and an amyotrophic lateral sclerosis-like syndrome: an analysis of individual case safety reports from vigibase.

BACKGROUND: The WHO Foundation Collaborating Centre for International Drug Monitoring (Uppsala Monitoring Centre [UMC]) has received many individual case safety reports (ICSRs) associating HMG-CoA reductase inhibitor drug (statin) use with the occurrence of muscle damage, including rhabdomyolysis, and also peripheral neuropathy. A new signal has now appeared of disproportionally high reporting of upper motor neurone lesions. AIM AND SCOPE: The aim of this paper is to present the upper motor neurone lesion cases, with other evidence, as a signal of a relationship between statins and an amyotro

[Visual hallucinations and unusual pain related to hypomagnesemia in an advanced cancer patient].

Hypomagnesemia has been related to high accumulated doses of cisplatin, and its clinical presence is characterized by neuromuscular and cardiovascular alterations. We present the case of an advanced cancer patient who had received successive lines of chemotherapy and who was receiving opioid treatment for cancer pain. During his hospital stay, he experienced visual hallucinations and very intense, unexplained pain in one shoulder. Symptoms reverted with the normalization of his plasma magnesium levels, which were low. We consider that plasma magnesium levels should be monitored in oncology pat

Acute and subacute weakness.

Respiratory failure is a common complication of acute neuromuscular disease and high cervical cord lesions and should be monitored by measuring forced vital capacity and respiratory rate. Urgent imaging is mandatory if there is any clinical suspicion of spinal cord disease. Treatment of Guillain-Barré syndrome with plasma exchange or intravenous immunoglobulin (IVIg) speeds up the rate of recovery. Treatment of patients in a myasthenic crisis with plasma exchange or IVIg often results in significant short-term improvement. A drug side effect or metabolic disturbance should be considered in a p

Sleep disordered breathing assessment in patient with slowly progressive neuromuscular disease.

BACKGROUND: Sleep-disordered breathing (SDB) is common in patients with neuromuscular diseases (NMD). Focusing on hypercapnia may lead to the neglect of other SDB such as obstructive and/or central sleep apnea syndrome (SAS). Our objectives were to assess the risk of inappropriate SDB management according to different screening strategies and to evaluate the prevalence and determinants of isolated and overlapping sleep apnea in patients with slowly progressive NMD. METHODS: This monocentric, cross-sectional, retrospective study analyzed medical records of adult NMD patients referred to a slee
