The nervous system has been involved in the majority (at least 75%), of cases of acquired immune deficiency syndrome (AIDS) examined postmortem, but the pathogenetic mechanisms involved are not well understood. The predominant pathological process is opportunistic infection secondary to the decrease of T-helper (T4) cells and includes toxoplasmosis, encephalitis due to cytomegalovirus and progressive multifocal leucoencephalopathy. On the other hand, mycoses (mainly cryptococcosis) are relatively uncommon. Primary lymphomas are three times more common than secondary lymphoma spreading from oth

Anti-IgLON5 disease is a unique condition that bridges autoimmunity and neurodegeneration. Since its initial description 10 years ago, an increasing number of autopsies has led to the observation of a broader spectrum of neuropathologies underlying a particular constellation of clinical symptoms. In this study, we describe the neuropathological findings in 22 patients with anti-IgLON5 disease from 9 different European centers. In 15 patients (68%), we observed a hypothalamic and brainstem-predominant tauopathy of varying severity in which the original research neuropathological criteria were r

Huntington's disease is a hereditary neurodegenerative disorder caused by the abnormal expansion of CAG repeats in the huntingtin (HTT) gene … icine, and its purified active component, Lycium barbarum glycopeptide, has been shown to alleviate neuropathology in various neurodegenerative diseases. Here, we evaluated the therapeutic potential of Lycium barbarum glycopeptide in Huntington's disease using both in vitro and in vivo models. We found that Lycium barbarum glycopeptide treatment effectively redu …

Alcohol use disorder (AUD) is characterized by an inability to stop consuming alcohol. Neuroimaging studies of patients with AUD show mild grey and white matter atrophy, while pathological studies suggest that atrophy is restricted to the white matter. The effects on individual brain cells are largely unknown. Mild neuronal loss has been described in the prefrontal cortex but this has not been consistent. Studies quantifying oligodendrocytes, astrocytes, and microglia are rare. These knowledge gaps impede therapeutic advancements. Here, we piloted the use of tissue microarrays, immunohistochem

A reduction in the number of neurons and an increased susceptibility to apoptosis are one of the characteristic features observed in Down syndrome (DS). However, the multifaceted actions of chromosomal aneuploidy hinder the elucidation of the underlying mechanism. Here, using neurons and astrocytes differentiated from patient-derived induced pluripotent stem cells (iPSCs), we aimed to clarify the neuropathology of DS by focusing on aneuploidy-associated stress and the dosage effects of specific genes. Human chromosomal trisomies 13, 18, and 21 exert a stress response on neurons, resulting in t…

Chronic cerebral hypoperfusion (CCH) is a key pathophysiological substrate of vascular cognitive impairment, and the reorganization of neuronal activity across disease progression underlies network dysfunction and intervention strategy. Using immediate-early gene imaging with whole-brain mapping, we characterized spatiotemporal activation patterns in a bilateral common carotid artery stenosis mouse model at early (day 7) and chronic (day 40) stages across 225 brain regions. Weighted gene co-expression network analysis identified the paraventricular thalamus (PVT) as a persistently hyperactive

Freezing of gait (FoG) is an episodic, debilitating motor phenomenon defined as paroxysmal episodes wherein there is an inability to step effectively, despite attempting to do so. Although FoG is most commonly associated with Parkinson's disease, it manifests across a spectrum of progressive neurodegenerative and potentially transient non-neurodegenerative conditions that affect overlapping locomotor circuits. The episodic nature of FoG implies transient disruptions of a distributed locomotor network rather than fixed structural lesions, rendering its neuropathological basis particularly chall

The role of neuroinflammation is becoming increasingly prominent as our understanding of the mechanisms underlying central nervous system disorders advances. What makes this phenomenon particularly intriguing from a clinical perspective is that neuroinflammation is consistently present, regardless of the specific neuropathology. One of its major consequences is the recruitment of immune cells from the circulation into the brain parenchyma, leading to chronic damage. Therefore, uncovering the molecular mechanisms driving this process may provide valuable therapeutic insights. Among the numerous…

INTRODUCTION: Whether Alzheimer's disease (AD)-associated plasma biomarkers reflect cognitive performance and neuropathology in the oldest old remains unclear.
METHODS: In plasma samples from 255 centenarians from the longitudinal 100-plus Study (median age 101.2 years), we quantified biomarkers amyloid beta (Aβ)42/40 ratio, Aβ40, Aβ42, phosphorylated tau 181 (pTau-181)/Aβ42 ratio, pTau-181, neurofilament light (NfL), and glial fibrillary acidic protein (GFAP) concentrations. These were associated with same-day measures of cognitive performance and, for centenarians who donated their brain (n…

Alzheimer's disease (AD) is neuropathologically characterized by the extracellular deposition of the amyloid-β peptide (Aβ) and the intraneuronal accumulation of abnormal phosphorylated tau (τ)-protein (p-τ). Most frequently, these hallmark lesions are accompanied by other co-pathologies in the brain that may contribute to cognitive impairment, such as vascular lesions, intraneuronal accumulation of phosphorylated transactive-response DNA-binding protein 43 (TDP-43), and/or α-synuclein (αSyn) aggregates. To estimate the extent of these AD and co-pathologies in patients, several biomarkers have

Medical science has made revolutionary discoveries around the nosology and aetiology of the neurodegenerative diseases. Dementia is the second leading cause of death in Australia. My colleagues and I are now looking at therapeutics which potentially can delay or prevent the onset of Alzheimer's disease (AD). Advances in diagnosis allow detection of preclinical AD. Neuropathologists working closely with chemical and genetic pathologists have a major role to play in pushing diagnostics and therapeutics to the forefront of clinical management of these diseases.

Alzheimer's disease (AD) is the most common form of age-related dementia worldwide. Although the neuropathology of AD is clear, its pathogenesis remains unclear. Recently, conceptualising AD as brain-centred has reoriented many scientists because the close functional relationship between the peripheral and central nerves is increasingly recognised. Recently, various studies have focused on the crosstalk between peripherals and centrals. A new hotspot of research and new therapeutic strategies have emerged from this great progress. This mini-review is an overview of the potential molecular mech…

OBJECTIVES: Loneliness in the aging population is associated with decreased cognitive function and increased neuropathology; less is understood about the association of loneliness and cognitive resilience (CR), defined as the discordance between a person's actual and expected cognition given their neuropathology. Here we assess the effect of loneliness and change in loneliness on CR at end of life and across older adulthood.
METHODS: Data were combined from 2 longitudinal studies of older adults. CR proximate to death (CRlast_level) and across time (CRslope) was obtained by independently regr…
