[Neurophysiological background of pain in the orofacial area: review of the literature].

The article presents an overview on the peripheral and central neural mechanisms underlying pain in the orofacial area. First a definition of pain and a description of general aspects of orofacial pain are presented. Characteristics of acute and chronic pain are also described. The study highlights the sense organs, the molecular mechanisms and categories of primary afferents involved in peripheral events of orofacial pain. After describing the brain-stem nuclei participating in trigeminal pain and their functional role, primary afferents involved in nociceptive sensation from the tooth pulp,

[Neurologic and neurophysiologic examinations of workers occupationally exposed to aluminium].

BACKGROUND: Studies conducted to assess aluminum (Al)-induced toxicity to humans and environment. It has been implicated that Al is involved in the etiology of Alzheimer's disease and other neurodegenerative disorders, though this is controversial. The aim of the present study was to assess Al effects on the nervous system functions in workers chronically exposed to this metal. METHODS AND MATERIALS: The study covered a selected group of 67 male workers (mean age, 38.7 +/- 10.3 years; range 23-55 years) involved in aluminum production. Their employment duration ranged between 2 and 34 years (

The use of neurophysiological endophenotypes to understand the genetic basis of schizophrenia.

Specifying the complex genetic architecture of the "fuzzy" clinical phenotype of schizophrenia is an imposing problem. Utilizing metabolic, neurocognitive, and neurophysiological "intermediate" endophenotypic measures offers significant advantages from a statistical genetics standpoint. Endophenotypic measures are amenable to quantitative genetic analyses, conferring upon them a major methodological advantage compared with largely qualitative diagnoses using the Diagnostic and Statistical Manual of Mental Health, 4th Edition (DSM-IV). Endophenotypic deficits occur across the schizophrenia spec

Neurophysiologic findings in early acute inflammatory demyelinating polyradiculoneuropathy.

BACKGROUND: Patients with early acute inflammatory demyelinating polyradiculoneuropathy (AIDP) may not meet the current neurophysiologic criteria. OBJECTIVE: To document neurophysiologic findings in early AIDP. METHODS: Clinical and neurophysiologic data from 38 AIDP patients, assessed within 10 days of symptom onset were reviewed. RESULTS: In addition to absent H reflexes and abnormal F-wave responses in the acute stage of AIDP, abnormalities of blink reflexes, upper limb sensory responses abnormalities with spared sural responses, presence of distal CMAP dispersion, and A-waves were frequ

Neurophysiological correlates of habituation during exposure in spider phobia.

Imaging studies using symptom-provocation paradigms in specific phobia have yielded contradictory results, possibly reflecting a failure to account for habituation processes. Given that a single session of exposure in vivo can result in significant improvement in specific phobia, we used prolonged exposure to phobic stimuli to identify CNS regions showing habituation. Eighteen subjects (12 with spider phobia, 6 healthy controls) underwent H(2)(15)O-positron emission tomography while being continuously presented with pictures of spiders or butterflies. Results showed main effects (spiders>butte

[Neurophysiological pain and sensory disorders correlates in patients with syringomyelia].

Short-latent and pain-related long-latent somatosensory evoked potentials (SEP) in response to electrical stimulation of n. medianus, n. tibialis and n. mandibularis were studied in 28 patients with syringomyelia. During stimulation in the area of pain-temperature sensitivity disturbance, reduced amplitude of early components N13, N18 and late components P3, N3, P4, N4 (100-500 ms) was detected. In patients with neuropathic pain, increased amplitude of the middle latency waves P2, N2 (40-100 ms) and low amplitude ratio P4/P2 (normal values = 1.5) were found. Patients with stimulus-evoked pain

[Neurophysiological and clinical aspect of patients with carpal tunnel syndrome].

In the group of patients with carpal tunnel syndrome (CTS), besides paresthesias in their fingers, we can see paresthesias in forearm; pain in elbow, sholder and neck, which is a reason for complicated diagnosis of it. This kind of complexnost of differential diagnosis pain in hand, is a reason for complet and strict diagnostic procedure. There are 30 patients with clinical and neurophysological findings for CTS in this paper who made provocative tests. Analising their results we made diagnosis of KTS.

Leg length inequality in humans: a new neurophysiological approach.

In conditions where there is moderate or severe pain, such as in low back pain, sciatica and osteoarthritis affecting the hip, the patient often reports the sensation that the two legs are of unequal length. This sensation cannot be explained as a result of fracture or other trauma that might cause a real change in length of the legs. There is confusion as to whether this 'leg length inequality' causes the pain or vice versa. It is argued here that the sensation occurs when there is pressure on the sensory nerves, especially the dorsal spinal roots. This causes pain but, more relevantly, cause

Neurophysiological prediction of outcome in obstetric lesions of the brachial plexus.

Seventy-three children with slow recovery after obstetric lesion of the brachial plexus (biceps function returning after 3 months of age) and with relatively favourable neurophysiological investigation were followed until a mean age of 4.3 years. Predictions for C6 and C7 were confirmed in 92% and 96% of the cases, respectively. Predictions for C5 were confirmed in a smaller proportion of cases (78%). The inability to record nerve action potentials for C5, and the high frequency of secondary shoulder pathology are reasons for this. In all, 33 of the 73 children came to operation for medial rot

Carpal tunnel syndrome despite negative neurophysiological studies.

The purpose of the present study was to compare the results of conservative and operative treatment for patients with carpal tunnel syndrome having normal neurophysiological studies. We studied 125 patients with normal neurophysiological studies and analysed eight symptoms and signs as "prognostic factors". Ninety-six patients were treated conservatively (splintage, steroid injection, antiinflammatory medications, activity modification) and 29 were treated surgically (open decompression). One year after initiation of treatment we assessed the outcome and statistically analysed (chi-square test

Excessive fragmentary hypnic myoclonus: clinical and neurophysiological findings.

BACKGROUND: Brief involuntary sleep-related twitches occurring asymmetrically throughout the body define physiologic hypnic fragmentary myoclonus. An abnormal intensification of this entity identifies excessive fragmentary hypnic myoclonus (EFHM), a 'proposed sleep disorder' in the International Sleep Disorders Classification. METHODS: We describe two patients with EFHM, one associated with a REM behaviour disorder. RESULTS: EFHM activity was restricted to sleep prevailing during NREM sleep stages and the second part of the night. EMG was normal and EEG-EMG back-averaging did not show any co

Symptoms and neurophysiological picture of carpal tunnel syndrome in pregnancy.

OBJECTIVE: To evaluate the incidence of carpal tunnel syndrome (CTS) in pregnancy through a validated and multiperspective assessment of CTS. METHODS: During 2000, the Italian CTS study group focussed on the occurrence of CTS in women during the final stages of pregnancy, enrolled in 7 Italian centers. In addition to the physician-centered and neurophysiologic traditional evaluations, we used a validated patient-oriented measurement to obtain more comprehensive and consistent data for severity of symptoms and functional impairment. RESULTS: In our study, CTS was clinically diagnosed in more

Clinical, neurophysiological and immunological correlations in classical Rett syndrome.

Rett syndrome (RTT) is neurodevelopmental disorder with the onset at critical period of postnatal ontogenesis and age dependent occurrence of clinical manifestations. The aim of the present study was to investigate possible correlations of the age of disease onset with clinical manifestations at the stage 3 of illness and neurobiological parameters. The study was carried out in 38 girls with classical RTT, aged from 3 to 7 years, and twenty and eighteen patients with the disease onset before and after the age of one year were divided into the groups 1 and 2 (Gr1 and Gr2), respectively. Quantit

Neurophysiological brainstem investigations in isolated Pierre Robin sequence.

Polysomnography, electromyography (EMG) of the face, tongue, and soft palate, blink reflexes (BRs), EMG during bottle-feeding, and brainstem auditory evoked responses (BAERs) were performed in 25 newborn babies with isolated Pierre Robin sequence (PRS) to aid in evaluation and management. Obstructive apneas were found in 23/24 patients (the 25th having undergone tracheotomy). Number and duration of central respiratory pauses were always normal, as well as electroencephalographic and clinical organization of sleep stages. EMG recruitment pattern in facial and lingual muscles, and BRs were norma

Mild ciguatera poisoning: Case reports with neurophysiological evaluations.

Ciguatera poisoning causes mainly gastrointestinal and neurological effects of variable severity. However, symptoms of peripheral neuropathy with paresthesias and paradoxical disturbance of thermal sensation are the hallmark. Electrophysiological studies are often normal, except in severe cases. We report four people who developed mild ciguatera poisoning after barracuda ingestion. Electrophysiological studies documented normocalcemic latent tetany. These findings are consistent with ciguatoxin's mechanism of toxicity, which involves inactivation of voltage-gated Na(+) channels and eventually

Clinical, radiological, neurophysiological, and neuropathological characteristics of gluten ataxia.

BACKGROUND: Ataxia is the commonest neurological manifestation of coeliac disease. Some individuals with genetic susceptibility to the disease have serological evidence of gluten sensitivity without overt gastrointestinal symptoms or evidence of small-bowel inflammation. The sole manifestation of disease in such patients may be ataxia. We describe the clinical, radiological, and neurophysiological features of this disorder. METHODS: Patients with ataxia attending the neurology outpatient clinics at the Royal Hallamshire Hospital, Sheffield, UK, were screened for gluten sensitivity as shown by

Neurophysiologic evaluation of lower motor neuron damage in tetraplegia.

We quantitatively investigated the extent of damage to motor neurons in tetraplegic subjects. Numbers of motor units in the patients were significantly lower for thenar, wrist extensor, and biceps brachii as compared to controls. Reduction in counts occurred even when M-response amplitudes were normal. Standard electromyography suggested a surprising frequency of lower motor neuron dysfunction below the level of injury. These results confirm previous reports and add data on motor units in the biceps brachii.

Focal subcortical reflex myoclonus. A clinical and neurophysiological study.

BACKGROUND: Patients with progressive myoclonus epilepsy or progressive myoclonus ataxia often show a focal myoclonus, both spontaneous and reflex to somatosensory stimuli. Myoclonus is time-locked to large ("giant") electroencephalographic potentials. Previous authors have classified it as a "cortical reflex myoclonus," with the assumption that it invariably arises from an abnormal corticifugal neuron discharge. OBJECTIVE: To identify the myoclonus source, using various neurophysiological techniques, in 5 patients with progressive myoclonus epilepsy/ataxia. METHODS: Extensive investigations
